6 Footnotes to Table 1A and 1B 5 * Years shown represent when time limited certifi cates were fi rst issued by the ABMS Member Board and their duration before recertifi cation. With the implementation of the ABMS Program for Maintenance of Certifi cation (ABMS MOC ) the certifi cation process is expanded to include assessment and learning activities that help physicians continually evaluate and refi ne their clinical judgment, procedural skills, and service to patients. Since the program requires ongoing professional activity, particular requirements must be completed before a physician receives an updated certifi cate. Certifi cate issued prior to 1972 when ABMS subspecialty recognition procedures were established. Certifi cates not yet issued. (1) First year certifi cate issued by the Member Board. (2) Founding member of ABMS. (3) In 2014, the ABMS Board of Directors approved a name change for the American Board of Medical Genetics (ABMG) to American Board of Medical Genetics and Genomics (ABMGG). (4) ABMGG certifi cates established prior to becoming an ABMS Member Board. (5) The following certifi cates were issued only during years indicated below. The Board renamed the certifi cates in 2011, however the requirements for certifi cation were not changed. Current Certificate Name Years Issued Under Previous Name Diagnostic Medical Physics (formerly Diagnostic Radiological Physics) Nuclear Medical Physics (formerly Medical Nuclear Physics) Therapeutic Medical Physics (formerly Therapeutic Radiological Physics) (8) The American Board of Surgery initially offered a Certifi cate of Special Qualifi cations in 1982 and a Certifi cate of Added Qualifi cations in The Special Qualifi cations pathway was closed in 1989; both certifi cates were given through In 1998, the designations of Added and Special Qualifi cations were dropped and all specialty certifi cates were designated as Certifi cation in Vascular Surgery. Effective July 1, 2006, the American Board of Surgery re-designated its certifi cate in Vascular Surgery from that of subcertifi cation to one of general certifi cation. The certifi cates of surgeons certifying in Vascular Surgery by this new pathway are identical to those previously issued for successful certifi cation/recertifi cation in Vascular Surgery. (9) The American Board of Obstetrics and Gynecology offers a voluntary annual recertifi cation. For more information, contact the Member Board or visit their Web site. (10) For physicians certifi ed by the American Board of Family Medicine entering the board s MOC program in 2012 or later, and for physicians initially certifi ed by the American Board of Internal Medicine, the American Board of Pediatrics, the American Board of Psychiatry and Neurology, or the American Board of Radiology, maintaining certifi cation is contingent upon their meeting the requirements for the specifi c board s MOC program, and therefore no certifi cation end date is provided. To maintain certifi cation from their specialty board, physicians must successfully complete specialty-specifi c requirements throughout their ongoing MOC cycles.

8 Table 2: ABMS Member Board Requirements for General Certifi cation 7 General certification is first awarded to candidates who meet the requirements in a specified field of medical practice. This table identifies specific requirements for initial general certification from each Member Board, but as published in this form, the table is necessarily over-simplified. The board eligibility time limits set by each of the boards are shown in this table.

(1) Accreditation Council for Graduate Medical Education (ACGME) is the accrediting body for post-md medical training programs in the United States. (2) Royal College of Physicians and Surgeons of Canada (RCPSC) is the organization which oversees the medical education of specialists in Canada. (3) Accepted by the Canadian College of Medical Genetics. (4) The ABMS Board Eligibility Policy, effective January 1, 2012, limits the period of time that may elapse between a physician s completion of training and achievement of initial certifi cation in a specialty. In implementing this policy, the boards that did not have an existing board eligibility policy in place established a board eligible period for candidates to achieve initial certifi cation following the successful completion of accredited training. This period can be 3 to 7 years plus time (if any) in practice as required by the board for admissibility to the certifying examination. All other candidates who have not completed their required training by January 1, 2012 have a board eligible period as indicated by their specialty board to complete the certifi cation process. Additionally, the boards established transition dates for physician candidates who had completed training, but not yet achieved initial certifi cation in a specialty as of the ABMS Board Eligibility s effective date. Those boards chose a date between January 1, 2015 and January 1, 2019 for candidates to achieve initial certifi cation. (5) These Member Boards had board eligibility policies in place prior to the effective date of the ABMS Board Eligibility Policy, and therefore no future transition date is provided. They aligned their board eligible periods with the principles of the ABMS policy. (6) Two pathways for certifi cation may be pursued. Candidates may either complete fi ve years of prerequisite training (General Surgery) and two or three years of Thoracic Surgery, or six years of an integrated Thoracic Surgery program with no prerequisite training. The American Board of Thoracic Surgery will accept a Vascular Surgery residency in lieu of a General Surgery residency as long as the Vascular Surgery training leads to primary certifi cation by the American Board of Surgery.

10 Table 3: Requirements for the ABMS Program for Maintenance of Certifi cation (ABMS MOC ) 9 Through ABMS MOC, physicians have the opportunity to maintain their certification by keeping pace with the latest advances in their specialties and demonstrating proficiency in six specific competency areas adopted by ABMS and the Accreditation Council of Graduate Medical Education. Particular requirements are determined by each ABMS Member Board and may change periodically. These activities are based on evidence-based guidelines, national clinical and quality standards, and specialty best practices. The information shown here is for maintaining a general certificate. Requirements may differ for a subspecialty certificate. Contact the particular board for the most current information. Footnotes can be found on page 10. (2) In most cases, this refers to the certifi cate duration length during which time certain requirements are to be completed. For physicians certifi ed by ABFM entering it s ABMS MOC program in 2012 or later, and for physicians initially certifi ed by ABIM, ABP, ABPN, or ABR, maintaining certifi cation is contingent upon their meeting the requirements for the specifi c board s MOC program, and therefore no certifi cation end date is provided. To maintain certifi cation from their specialty s board, physicians must successfully complete specialtyspecifi c requirements throughout their MOC cycles. (3) Requirements could include CME that is practice-relevant such as literature reviews, conference participation, online tests and/or particular board-or society-developed self-assessment modules. Milestones for the type and amount of CME credits varies among the boards. (4) CME - Continuing Medical Education, consisting of educational activities which serve to maintain, develop or increase the knowledge, skills and professional performance and relationships that a specialist uses to provide services for patients, the public or the profession.
