4 Glossary ACGME Active Applicant Applicant Type NRMP RC SMS Accreditation Council for Graduate Medical Education (ACGME) An active applicant is one who submits a certified rank order list of programs. The NRMP classifies SMS applicants into 6 types: Graduates of U.S. allopathic medical schools: a graduate of a U.S. allopathic school of medicine accredited by the Liaison Committee on Medical Education (LCME). Also referred to as U.S. allopathic graduates. Graduates of Canadian medical schools: a graduate of a Canadian school of medicine accredited by the Committee on Accreditation of Canadian Medical Schools (CACMS). Also referred to as Canadian graduates. Graduates of osteopathic medical schools: a graduate of a medical school accredited by the Association of n American Colleges of Osteopathic Medicine (AACOM). 6 About the Specialties Matching Service The National Resident Matching Program (NRMP) is a private, not-for-profit corporation established in 1952 to optimize the rank-ordered choices of applicants and program directors. The NRMP is not an application processing service; rather, it provides an impartial venue for matching applicants' and program directors' preferences for each other consistently. The NRMP Specialties Matching Service (SMS ) conducts Matches for advanced residency and fellowship positions. The NRMP conducted its first Fellowship Match in 1984 for Colon and Rectal Surgery, and in 215 provided services for 56 subspecialties within 25 separate Matches. For the 215 appointment year, the Pediatric Matches experienced addition and regrouping: the Pediatric Specialties Fall Match was expanded with the addition of Pediatric Hospital Medicine, Neonatal Perinatal Medicine (originally a single-specialty Match), and Pediatric Infectious Diseases (originally in the Pediatric Specialties Spring Match). Also, the Pediatric Hematology/Oncology Match joined the Pediatric Specialties Spring Match. A complete list of the subspecialties participating in the SMS for the 215 appointment year can be found in Figure 1. The Fellowship Matches occur throughout the year, and each requires its own registration process with the NRMP. Fellowship positions generally begin in July. Because Fellowship Match appointments may begin anywhere from 6 to 17 months after their Match Days, data are reported for the appointment year instead of the Match Year. 9 Introduction This report summarizes all Fellowship Matches conducted by the NRMP Specialties Matching Service (SMS ). Data are provided for Matches conducted in 214 and early 215 for appointments beginning in July 215. Graphs display five-year trend data by specialty on numbers of programs, positions, and applicants as well as match rates. Numbers of positions offered and filled for each participating program are listed at the end of this report. The 215 report includes one new specialty: Pediatric Hospital Medicine. The 215 appointment year was the largest in the history of the SMS. The Matches encompassed: 3,674 programs within 56 subspecialties offering 8,53 fellowship positions Of those positions, 7,469 (87.8%) were filled Of the 3,674 participating programs, 2,938 (8.%) filled all positions 9,538 applicants participated in at least one of the Fellowship Matches and 7,469 (78.3%) obtained a position Numbers of applicants, matched applicants, positions, and programs offering positions between 1993 and 215 are shown in Figure 2 below. 11 Table 1 Table 1 provides a summary of the SMS Fellowship Matches for appointment year 215. Using Allergy and Immunology as an example: The first two columns ( No. of Applicants ) show that 15 U.S. allopathic medical school graduates and 155 total applicants ranked at least one Allergy and Immunology program. The next two columns ( Positions Offered and No. of Programs ) show that a total of 126 positions were offered in this specialty by 82 programs. The No. of Matches columns show that, of the 126 positions offered in Allergy and Immunology, 9 were filled by U.S. allopathic graduates, and 117 were filled overall. The percent of positions filled (calculated as No. of Matches divided by Positions Offered ) can be found in the % Filled columns. Of the fellowship positions offered in Allergy and Immunology, 71.4 percent were filled by U.S. allopathic graduates and 92.9 percent were filled overall. The Ranked Positions columns show that the 82 Allergy and Immunology programs were ranked 731 times by U.S. allopathic graduates and 1,71 times by all applicants (including U.S. allopathic graduates). The last column ( Unfilled Programs ) indicates that 8 Allergy and Immunology programs had at least 1 unfilled position after the matching algorithm had been processed. Of the 56 participating subspecialties, 35 filled more than 9 percent of the positions offered and 16 filled less than 75 percent of the positions offered. Eight specialties filled 75 percent or more of their positions with graduates of U.S. allopathic medical schools, and 23 specialties filled 5 percent or less of their positions with U.S. allopathic graduates. 15 Table 2 Table 2 shows the number and percent of matches in each specialty by applicant type. Of the specialties offering at least 3 positions: Gynecologic Oncology had the highest proportion filled by U.S. allopathic medical school graduates (92.2%) and Nephrology had the lowest (31.1%). The top five specialties in the percentage of positions filled by graduates of U.S. allopathic medical schools are Gynecologic Oncology (92.2%), Pediatric Surgery (89.6%), Hand Surgery (89.3%), Interventional Radiology (85.2%), and Neuroradiology (84.3%). The top five specialties in the percentage of positions filled by osteopathic medical school graduates are Primary Care Sports Medicine (31.2%), Developmental and Behavioral Pediatrics (2.%), Child and Adolescent Psychiatry (17.%), Pediatric Hematology/Oncology (16.3%), and Pain Medicine (14.3%). The top five specialties in the percentage of positions filled by U.S. international medical graduates are Sleep Medicine (2.2%), Developmental and Behavioral Pediatrics (2.%), Child and Adolescent Psychiatry (18.3%), Primary Care Sports Medicine (17.6%), and Nephrology (17.3%). The top five specialties in the percentage of positions filled by Non-U.S. international medical graduates are Abdominal Transplant Surgery (45.6%), Nephrology (42.1%), Endocrinology, Diabetes, and Metabolism (39.7%), Rheumatology (37.4%), and Pediatric Nephrology (33.3%). 18 Table 3 The ratios of fellowship positions to active applicants are provided in Table 3 for years 211 through 215 and shown graphically in Figure 3 for years between 22 and 215. Active applicants are defined as those who submitted certified rank order lists of programs. The red line in Figure 3 indicates the point at which there are equal numbers of positions and applicants. The number of fellowship positions per active U.S. allopathic graduate was 1.64 in 215. Across all five years, there were more fellowship positions offered than there were U.S. allopathic graduate applicants. The ratio of fellowship positions to all active applicants was.89 for the 215 appointment year, the same as in 214. Results and Data 215 Specialties Matching Service 12

20 Table 4 Table 4 displays applicants by applicant type in the SMS Fellowship Matches between 211 and 215. Applicants are grouped based on citizenship and medical school of graduation. For example, U.S. allopathic graduates are prior graduates of U.S. allopathic medical schools regardless of citizenship. U.S. international medical graduates are defined as U.S. citizens who are graduates of international medical schools, and non-u.s. international medical graduates are non-u.s. citizen graduates of international medical schools. The total number of registrants for the 215 appointment year was 1,36, an increase of 116 or 1.2 percent over 214. The number of graduates of U.S. allopathic medical schools rose to 5,343 in 215, a 3. percent increase. The number of non-u.s. citizen graduates of international medical schools declined for the third year in a row, and is down more than 4 applicants since 212. With the exception of non U.S. citizen graduates of international medical schools and graduates of Fifth Pathway programs, registration numbers for all applicant types increased from 214 to 215. An active applicant is one who submits a certified rank order list of programs. Between 211 and 215, the number of active applicants rose for graduates of U.S. allopathic medical schools, U.S. citizen graduates of international medical schools, and most notably for graduates of osteopathic medical schools (68.4% increase). Active applicant groups that increased faster than the overall rate of 22.7 percent between 211 and 215 are identified in Table 4 with green arrows. 25 Table 5 Table 5 displays Fellowship Match results by specialty and applicant preferred choice. Applicant choice is determined by the placement of programs on applicants' rank order lists. Preferred specialty is defined as the specialty ranked first on the applicant's rank order list. For single-specialty Matches (top half of table), the data presentation and interpretation are straightforward. For multi-specialty Matches (bottom half of table) they are slightly more complicated. Single Specialty Matches Using Allergy and Immunology as an example, 117 of 155 applicants matched. Of the 155 matched applicants, 43.2 percent matched to the program that was listed first on their rank order list, 14.2 percent matched to their second choice, 5.8 percent to their third choice, and 12.3 percent to a choice lower on their list. Thirty-eight applicants in the Allergy and Immunology Fellowship Match did not match, representing 24.5 percent of the total. Combined Specialties Matches To count unique applicants for the multi-specialty Matches, each applicant is included in the counts for the specialty ranked first on the rank order list (preferred specialty). Because applicants can match to other specialties within the same Match, "Matched in Another Specialty" data also are provided. For example, Endocrinology, Diabetes, and Metabolism participated with 11 other specialties in the Medical Specialties Matching Program (MSMP). Of the 324 applicants who ranked Endocrinology, Diabetes, and Metabolism, 33 ranked it first. Overall, 252 applicants matched to Endocrinology, Diabetes, and Metabolism; of those who matched, 244 listed it as the preferred specialty. Of the 324 applicants who ranked Endocrinology, Diabetes, and Metabolism, 143 (44.1%) matched to the program that was listed first on the rank order list, 45 (13.9%) matched to the second choice, 23 (7.1%) to the third choice, 41 (12.7%) to a choice lower on their list, and 9 (2.8%) matched to another specialty within the MSMP.
