Stowe, Director RE: Organization Assessment The Department of Student Life conducts program assessment and evaluation of all registered clubs and organizations each year. The reason for this assessment is to determine the level of involvement of organizations over the academic year. Please complete the enclosed evaluation form and attachments and return to the Department of Student Life, c/o Ms. Ebony King, Coordinator of Student Activities, P. O. Box SSU, by 5:00 p.m. on THIRD MONDAY IN APRIL. Failure to complete the required evaluations will result in NON-RECOGNITION status for your organization for the next academic year. 2 Attachment 1 Department of Student LIfe Savannah State University Organization Evaluation & Annual Report Name of Organization Address Name of President Advisor Name or Web Address Directions The Organizational Evaluation and Annual Report is an opportunity for campus clubs and organizations at Savannah State University to share with the Department of Student Life and other administrators the success and strength of their organizations over the academic year. This report should be prepared in narrative form, as concisely as possible and you may include any supporting materials [copies of flyers, programs, etc.] within this report. Organizations annual reports are due the last Monday in the month of April. Please return all typed reports to: DEPARTMENT OF STUDENT LIFE C/O MS. EBONY KING, COORDINATOR, STUDENT ACTIVITIES SAVANNAH STATE UNIVERSITY P.O. Box Savannah, Georgia Section A: Organizational Administration Identify and briefly provide the current mailing address [campus] for your organizations along with the name(s) and contact information of campus advisors. You should also provide the name(s) and address of any local, regional and national structure in which this organization is affiliated with. Section B: Membership & Meetings Briefly describe the membership make up of the organization. Please indicate the total number of members, how many membership drives where sponsored over the year and what methods of recruitment were conducted. Also state the total number

3 of non-traditional, minority (Non African-American), disabled students that participated with your organizations this academic year. Also in this section please indicate the leadership makeup of your organization and their participation within the organization. State the level of involvement or participation of the advisor and if the organization regularly held meetings and the participation of the members, leadership and advisors with the meetings. Section C: Events Summary of Major Organizational Accomplishments & Special Identify and briefly describe the major accomplishments made by the organization over the academic year. Indicate any programs or activities sponsored by the organization and the level of participation of the members of the organizations and the campus community at large. You may include copies of any flyers, programs and other supporting materials to help demonstrate the your involvement in campus life. Section F: Finance Briefly describe the financial satiability of the organization and indicate if the organizations received any type of funds [students activities fees, SOAR, etc] from the university over the academic year. Please provide information on any type of fundraisers conducted and indicate if the particular fundraisers were productive or not. Section G: Overall Learning Outcome Identify and briefly describe the outcome of your organization learning objective based on the goals and objectives of your organization. Please indicate if the members where able to learn to work cooperatively, any leadership skills gained, etc.

The organization has a permanent Post Office Box on campus. 3. The constitution and by-laws are current and on file with the Department of Student Life. 4. The organization has two full-time faculty or staff employees serving as advisors. [Please provide names and contact information] 5. The organization is affiliated with a state, regional and/or national organization. If so, please provide contact information. 6. The organization s cumulative GPA is a 2.50 or better. 7. Members understand the purpose of the organization. 8. Members have read and understand the organization s constitution and by-laws. 9. Members understand how to amend the constitution and by-laws.
