2 Ophthalmic Plastic Surgery Complete eye health includes having healthy eyes and healthy eyelids. Common eyelid problems include excess skin and droopy eyes. They cause eye discomfort and can limit vision and affect appearance. Fortunately, such eyelid problems can be corrected with surgery. As we age our skin begins to lose its elasticity and facial lines or wrinkles become more predominant. We may appear older than we actually are. The oculoplastic surgeons at Bascom Palmer offer a variety of cosmetic surgeries and non-surgical aesthetic enhancements which may correct these problems. Cosmetic surgery is generally perceived as an aesthetic procedure rather than the repair of a physical flaw. 3 Surgical Cosmetic Enhancements The eyes and eyebrows are the aesthetic centers of the face. As aging occurs, the skin loses its elasticity resulting in a downward gravitational pull on the facial structures. Consequently, the position of the eyelids and eyebrows can dramatically change and alter the characteristics of mood and emotion. Excess skin of the upper eyelids may produce a heavy feeling and may hide the natural skin crease creating a false sense of aging. Excess skin and bulging fat of the lower eyelids produce bags underneath the eyes and give the impression of fatigue. Poor position of the eyebrows may be misinterpreted as an angry expression, whereas a more arched, higher set brow tends to be associated with a sense of awareness and energy. Ptosis (pronounced toe-sis ) is a condition in which the upper eyelid droops. Ptosis may limit your peripheral or central vision. If ptosis occurs in one eye, it may create an uneven appearance. Surgery restores the upper eyelid to its normal position and will often lead to better vision and improved appearance. Before blepharoplasty After blepharoplasty Excess Eyelid Skin is quite common and develops in many people as they get older. Because eyelid skin is the thinnest skin of the body, it tends to stretch with age. In the upper eyelid, this skin may limit your side or upper vision and create a heavy sensation in the eyes. The excess skin also causes bags to form in the lower eyelids. The excess skin in the eyelids can be removed surgically by a procedure called blepharoplasty. Through this procedure the excess skin, muscle and fat of the upper and/or lower eyelids are removed. 2 3

4 The procedure in the upper lids results in a more apparent lid crease and a more open appearance of the eyes. In the lower eyelid, the presence of bags is minimized and a more energetic appearance is restored. If excess fatty tissue is present, it may be removed at the same time, with or without the removal of excess skin. Eyelid plastic surgery is almost always performed on an outpatient basis using local anesthesia. The improvement in your vision, comfort and appearance can be extremely gratifying. Brow and Forehead Lift A forehead or brow lift is a procedure that restores a more youthful, refreshed look to the area above the eyes. This lift improves the horizontal lines in the forehead that make a person appear older, angry or tired. Several approaches to lifting the brows and forehead are offered at Bascom Palmer, including the small incision endoscopic technique, which requires only a few small incisions hidden behind the hairline, and the transblepharoplasty technique using the endotine, an absorbable implant. Another approach is the coronal brow lift, a more invasive yet effective procedure to lift the forehead with a large incision behind the hairline. These brow lift procedures aim to lift the eyebrows to a higher position, allowing the surgeon to tailor the arch and shape of the brow to fit the patient s desires. Certain approaches also work to smooth out the entire forehead and can be combined with Botox treatments to minimize the appearance of wrinkles, resulting in a more refreshed appearance or positive expression. 4 5

5 Non-Surgical Cosmetic Enhancements older they become deeper and more noticeable. The most common areas where we develop dynamic wrinkles are around the lips, the corners of the eyelids ( crows feet ), between the eyebrows and the forehead. These wrinkles often make people look older and more tired than they actually are. Botox can be used to treat these dynamic wrinkles and make them less noticeable. The first signs of aging can often appear as wrinkles around the eyes, forehead, cheeks and lips. While wrinkles are normal defining features of the human face, they can sometimes falsely portray a tired or more aged appearance. We are born with dynamic wrinkles which are associated with the muscles we use for our facial expression. We continuously use our facial muscles to smile, laugh and squint and as a result wrinkles appear. When a person is young wrinkles appear mild, but as we grow Botox Cosmetic Botox works to reduce or eliminate wrinkles on the face. By temporarily relaxing the muscles, Botox reduces the appearance of fine lines around the lips, across the forehead, around the corners of the eyes and between the eyebrows. It can also provide a small amount of lift to the lateral brows, providing a non-surgical option of brow lifting. Only FDA-approved Botox manufactured by Allergan, Inc. is used at Bascom Palmer. 6 7

6 Laser Skin Resurfacing In laser skin resurfacing a laser is used to remove areas of damaged or wrinkled skin. The procedure is most often used to minimize the appearance of fine lines around the eyes and the mouth, and can also be used to smooth wrinkles, remove brown spots and other pigmentation, and treat facial or acne scars. Additionally, laser resurfacing may be performed on the hands, neck and chest, as well as, stretch marks and nonfacial scars. This treatment works by stimulating collagen regeneration. Laser skin resurfacing is often done in conjunction with another cosmetic procedure. When combined with a blepharoplasty, even more pronounced results can be achieved. It is also a non-invasive alternative method of facial rejuvenation for those patients who opt for non-surgical treatment. A series of treatments results in decreased wrinkling of the eyelid skin, thickening and tightening of the tissues, and a reduced appearance of dark circles around the eyes. The surgeon can tailor each treatment according to the extent of skin damage, skin type and individual desires. Dermal Fillers Another effect of aging is the loss of volume in the face. A dermal filler, or substance injected into the skin, may restore volume and fullness to the skin to correct facial wrinkles and folds. There are many types of dermal fillers offered as an injection to replace this volume loss, many of which are used at Bascom Palmer. These include Restylane, Juvederm and Radiesse. 7 Not only do the dermal fillers enhance volume, especially in the lips and lower face, but they in turn decrease the appearance of lines and wrinkles and can also be used to fill in acne scars, producing a smoother appearance of the skin. In addition to enhancing the appearance of the lip border, fillers can also be used to correct hollowness around the eyes and decrease the appearance of bags under the eyes, again offering a nonsurgical alternative to a blepharoplasty. More recently, the dermal fillers have been used to non- surgically lift the lateral brows. In conjunction with Botox, dermal fillers may further enhance the aesthetic outcome. Intense Pulsed Light Age spots and sun damage can be treated non-invasively with a new treatment using the process called photorejuvenation. Light is delivered to the skin surface in precise pulses that stimulates the formation of new collagen. It is effective on red blemishes that originate from blood vessels, brown pigmentation, birthmarks, suninduced freckles, age spots, acne scars, and redness due to rosacea. This treatment may be applied to the face, neck, chest, arms and hands, where sun damage is often most visible. Another benefit of the Intense Pulsed Light (IPL) is permanent hair removal. IPL eliminates hair follicles in the active growth phase. This reduces the density of hair present on the skin. Intense Pulsed Light treatments provide gradual, natural improvement with little discomfort and an immediate return to work or social activities. This treatment may be applied to the face, neck, chest, arms and hands where sun damage is often most visible

With nightly application of the Latisse to the upper lash line, studies report a 106% increase in lash thickness, a 25% increase in eyelash length, and an 18% increase in eyelash darkness. Results may be seen in as little as eight weeks with maximum effect achieved by 16 weeks. As Latisse is available by prescription only and is not appropriate for all patients, a thorough eye exam and initial close follow-up are recommended prior to first use.
