3 What side effects can I expect after treatment? Immediately following treatment, you will have some degree of redness and swelling. Other temporary side effects may include minor itching, dry skin, peeling, flaking, and a bronzed skin appearance. We provide each patient with specific instructions about what to expect for their particular treatment and how to minimize side effects. What areas can I treat? Face, neck, eyelids, chest and most other body areas for improvement of texture, wrinkles and scars. This laser can safely treat non-facial areas, a truly new advance in the improvement of non-facial wrinkles, scars, and texture. Treatment of stretchmarks is also very good with this laser. How painful is the treatment? Mild treatments can be performed with topical anesthesia only. For more aggressive treatments, oral sedation and tumescent local anesthesia is used to make the skin completely numb. We do everything necessary to make you completely comfortable during your treatment. How do I take care of my skin after treatment? Dark Circles Under the Eyes Most people think that dark under-eye circles are caused by staying up late watching monster movies, having that last drink the night before, or sitting up with your laptop trying to finish your quarterly report. Most people believe that their behavior is somehow linked to dark undereye circles. The truth is, most people are dead wrong. If nothing else, I want you to know that dark under-eye circles are not your fault. They don t mysteriously appear when you misbehave or are stressed out, only to vanish when you get 8 straight hours of sleep. Dark under-eye circles are a byproduct of aging. Changing your behavior will, in almost every case, NOT get rid of your dark under-eye circles. Tear troughs are semi-circular creases or depressions which start at the nose and run along the bottom of the lower eyelid just above the cheek bone. These depressions can produce the illusion of darkening around the eyes, resulting in dark circles or tired eyes. As we age, the eyelids and cheek begin to sag. The tear troughs, however, cannot sag with the rest of the facial skin because of ligaments that attach to the bone of the eye socket. This results in the deepening depressions which make the shadow and dark circle more noticeable. If you have deep tear troughs, an injectable dermal filler, such as Restylane, can be used to correct them. The filler is injected with a very fine micro needle under the skin s surface along the line of the hollow. An anesthetic cream may be applied to the area for 30 minutes prior to the treatment (although often no anesthetic cream is required). Results with dermal fillers are instantaneous. After the procedure, the treated area may be slightly red and swollen; however, you can resume normal activities, drive yourself home or return to work immediately after the treatment. The results typically last 6-9 months, although the exact length of time will depend on the structure of your skin and other factors such as lifestyle, age and the degree of perfection you demand. 11 Wrinkle Treatments Botox Safe & Effective Botox was approved by the FDA-in 2002 for the treatment of facial wrinkles (specifically the frown lines between the eyebrows) and is now also approved for the treatment of underarm sweating and some severe neurological disorders. Medications such as Botox are tested rigorously for years to document safety and efficacy before they can be FDA approved. Millions of patients have been treated with Botox without any serious side effects. Despite the product s overwhelming success and acceptance, the media hasn t stopped creating news by reporting on Botox stories with negative spin. What they typically don t include when documenting side effects, however, is that they occurred when Botox was used in huge amounts, injected inappropriately by an inexperienced user, or in laboratory animals. At the Gateway Aesthetic Institute & Laser Center, we have used Botox on many people every day with tremendous satisfaction rates. Botox works by relaxing the facial muscles that cause expression lines. It must only be injected by an experienced professional who is familiar with facial anatomy so that it is not injected into the wrong muscle. It looks the most natural if it is used in the correct amount to simply relax the muscle and not completely paralyze it. That way, you can still have normal facial expressions, but the lines and wrinkles are softened. The treatment usually has to be repeated 3-4 times a year to maintain your results. Another less-known but successful use of Botox is for excessive sweating under the arms, a problem unrelated to ambient temperature that can t be well controlled with antiperspirants. Injecting Botox can decrease the nerve signals to the sweat glands and thereby decrease or eliminate sweating for up to 6 months (and even longer in some people). It is a quick and easy procedure with a very high rate of satisfaction.

14 Understanding Skin Cancer Warning Signs and Prevention Few words conjure up more images of concern and desperation than that of cancer and rightly so. But did you realize that of the many forms of human cancer, skin cancer is the most common? The National Cancer Institute notes that skin cancers will affect nearly 1 million Americans this year and the number of new cases is increasing. Equally concerning is the fact that nearly half of all Americans who live to the age of 65 will develop skin cancer at least once. Despite the gloomy numbers, there is hope. Unlike cancers that grow internally, skin cancer has the greatest potential for early detection and treatment if we are vigilant to its warning signs. Types of Skin Cancer To understand what to look for, one should first understand the basic skin cancer types. They fall into three groups: precancers, nonmelanomas, and malignant melanoma. Warning signs for basal cell carcinoma may include an open sore that doesn t heal, a growth with an elevated border and central indentation, or a scar-like bump on the sun-exposed skin of the head, neck, chest, arms or legs. The most common treatment for this type of cancer is surgical excision. Up a notch on the danger pole, above basal cell carcinoma, is the next most common kind of skin cancer: squamous cell carcinoma. Like basal cell carcinoma, this cancer is generally not considered a threat to life. However, unlike its cousin above, squamous cell carcinoma, if not treated, may potentially spread to other body organs (including lymph nodes) and ultimately cause death. This type of cancer appears frequently as a red, scaling, thickened patch on the arms, legs, lips, face or ears. Ulceration and bleeding may also occur. Precancers When dermatologists diagnose precancerous lesions, they are most likely referring to actinic keratosis (AK), also known as solar keratosis. While AK is not officially cancerous, it has the potential to develop into more serious forms of skin cancer. AK is usually found in the outer layer of the skin, most commonly on the face, lips, ears, neck and scalp. It may cause the skin to become scaly, rough, red or discolored, and may be tender to the touch. Treatment options vary and are best discussed with one of our board-certified dermatologists. Common treatments for AK include freezing with liquid nitrogen, chemical peels, topical chemotherapy creams and laser resurfacing therapy. Nonmelanomas The most common form of skin cancer, and thankfully the least dangerous, is basal cell carcinoma. It accounts for more than 90% of all skin cancer in the U.S. Basal cell carcinoma grows slowly and rarely spreads beyond its original location. While this cancer is seldom lifethreatening, if left untreated, it can grow into underlying tissue and cause serious damage. Treatments include Mohs micrographically controlled surgery, surgical excision, lasers, or radiation therapy. Malignant Melanoma The most dangerous form of skin cancer is malignant melanoma. Thankfully it is also the least common, though incidences are rapidly increasing. If detected early, malignant melanoma can be completely cured. If it is not treated quickly, however, it will spread throughout the body s organs and ultimately be fatal. While anyone can get melanoma, fair-skinned people are at greater risk. It can appear anywhere on the skin, but is most common on the backs of men and on the legs of women. Melanoma almost always begins on the skin s surface, where it s easily treated (generally via surgical removal). If given time to grow, it will move into the blood and lymphatic vessels, making full recovery difficult. Melanoma s warning signs include spots or moles that change in size, color or feel (often with an irregular border). The cancer may also appear as a small lesion containing red, white, blue or black spots; or shiny, firm, dome-shaped bumps (located anywhere on your body). 14 gateway aesthetic Institute & Laser Center
15 If you are unsure about a spot or mole, it s always best to see your doctor. Prevention Several factors (many beyond our control) may increase the risk of getting skin cancer. These include genetic predisposition, the altitude at which we live, and age. That said, our risk may substantially decrease by doing the following: Reduce exposure to ultraviolet (UV) radiation, (especially important for children) Avoid sunburns and sun exposure during the day (usually from 10 a.m. to 3 p.m.), when the sun is highest in the sky Wear protective clothing (long sleeves and hats) when outdoors Use a broad-spectrum sunscreen that blocks both UVA and UVB radiation Reapply sun block every 2 hours and after swimming Check your skin regularly and report any changes to your doctor Like everything in life, our skin s health offers no guarantees. That said, if we remain aware, proactive, and diligent with our skin, we stand a much better chance of living a happy, healthy, and cancer-free lives. Fall/winter

18 A Note from Your Doctor Patients often ask for advice regarding the timing of when it s best to have a cosmetic procedure. Recently I performed a lower eyelid blepharoplasty on a 16 year old boy because he was born with puffy lower eyelids and his friends were teasing him. The procedure turned out great and the teasing stopped. For him, the timing was perfect. Children with birthmarks are often made fun of when they start kindergarten. Treating their birthmarks early may help them avoid growing up with a damaged self-image. Teenagers who suffer from problem acne may also be vulnerable. Early and aggressive treatment will not only help immediately with the acne, but it may also help them to avoid severe acne scarring that could bother them well into adulthood. Once again, timing is critical. Certainly not all cosmetic procedures are as time sensitive as in the examples above. Treatments that reduce wrinkles or other signs of aging are usually sought after when a person becomes self-conscious about a problem. It may be early in life or later. Many patients, for example, come in to the Gateway Aesthetic Institute to get their inherited baggy eyelids fixed when they are just 35. Others may wait until they are nearly twice as old. The point is, no matter your age, if something about your appearance is bothering you (wrinkles, spots, moles, lumps, bumps or any other skin condition or abnormality), then it may be time to seek treatment.
