1 ROLE DESCRIPTION POSITION TITLE: Planned Activity Group (PAG) Activities Assistant DATE OF EFFECT: January 2015 TYPE OF EMPLOYMENT: Volunteer REPORTING TO: Recreation Officer & PAG Team Leader Advisor, Engagement GENERAL RESPONSIBILITY STATEMENT To work within the PAG to help with social and recreational activities LIAISES WITH: Patients and staff Revised: Jan 2014

ORGANISATION INFORMATION 1.1 Introduction The Northern Health Service provides a diverse range of acute, sub-acute and community health services to the large and diverse population in the regions north of Melbourne. There are five campuses; Broadmeadows Health Service, Bundoora Extended Care Centre, The Northern Hospital at Epping and Panch Health Service at Preston, and Craigieburn Health Service. 1.2 Strategic Plan The Strategic Plan for Northern Health includes the following elements Our vision Outstanding health care for our community. These priorities are inter-related in that success in one goal is dependent on success in another goal. Each goal is detailed in this plan, with a statement as to why it is a priority, what Northern Health has achieved already, key challenges that Northern Health is facing and the intended three year outcome we are anticipating with implementation of the strategies and actions. ROLE STATEMENT Requirements: On arrival sign in the volunteer book Wear a you identification badge at all times Meet and greet clients on arrival Assist in the set up and pack of activities Assist and participate in activities (including cultural) Assist in the moving of clients in wheelchairs Assist in the visual supervision of clients whilst in the program Work one on one assisting high care clients with activity participation Farewell clients on departure On departure sign out in the volunteer book Activities include: Physical Games (such as basketball, shuffleboard etc.) Craft Activities Board Games Cooking Gardening External Outings Old Blokes Shed Accompanying clients to do Woodworking 3. KEY RESULT AREAS and MAJOR RESPONSIBILITIES General Comply with all of the By-Laws, Regulations and Policies that are in place at Northern Health from time to time, including those relating to; Privacy and Confidentiality, Occupational Health and Safety, Performance and Development Management, Harassment in the Workplace. Comply with all relevant Legislation. Contribute to quality improvement and sustainability of the organisation by participating in quality activities and ensuring flexibility within the role in order to respond to the changing needs of our customers. 4 4. OCCUPATIONAL HEALTH & SAFETY The following potential Occupational Health & Safety risks are associated with this position. Physical Risks Repetitive tasks > 2 hours at a time. Heavy lifting. 5 IMPORTANT INFORMATION: Volunteers must not - Attempt to assist patient in or out of his or her wheelchair seek nursing assistance for patient Attend their shift if they are unwell Express personal opinions regarding a patients medical status or treatment Provide advice regarding a patients medical status or treatment Assist clients with food or drink provision Assist unsteady clients when walking Assist clients with toileting Leave clients unattended Volunteers must - Follow infection control guidelines Practise safe sun exposure; ie, wear hat, sunscreen Alert staff when patients require attention Attend training as required Notify the volunteer coordinator of any planned absences Communicate closely with appropriate members of staff Support Services- Should you require assistance or support, contact your direct supervisor or the volunteer coordinator who may direct you to an appropriate support service for your needs.
