2 If you need transportation, and you live in Washington County, you have a variety of options available to you. Transportation fees and fares will vary with each service provider and many require some advance notice to schedule a ride. Other transportation services exist that may not be included here. Check your local phone directory or the internet for more information. This information is subject to change. Allegiance Transportation At Allegiance Transportation, our goal is to try and ease those concerns, and replace them with the comfort of knowing you or your loved ones are in the very best of hands while traveling with one of our professionals. Prior to your first riding experience with our company, we offer to our clients a meeting with one of our staff to introduce you to our company. We ll take time to answer any questions you may have about us, our company, or our drivers. We ll also have some questions for you such as any regular appointments, therapy you re undergoing, or any other transportation needs you may have. We offer the safest, most courteous, respectful, and comfortable transportation experience for seniors, disabled persons, or anyone else who has decided to leave the driving to a professional transportation provider. Days/hours of service: Monday through Sunday, 7:00am 7:00pm. Length of advance notice required: 24 hours preferred. Wheelchair lift: Yes- 800 lb. capacity, Braun rear mount lift. Price: Floating scale for pick-ups/drop-offs, and mileage. Please call for quote very competitive rates. Payer sources: Private Pay. Always Best Care Transportation service with a special touch. Caregivers escort patient to appointments and are available to remain with patient during appointments, also assist with meals and light housekeeping. Days/hours of service: 7 days a week, morning through evening. Length of advance notice required: 24 hours advance notice. Wheelchair lift: No. Price: $66 for 3 hours, plus $0.50 per mile paid directly to the caregiver. Payer sources: Private Pay/cash or check, no MA or insurance. 2
3 American Cancer Society Transportation provided to cancer patients who are ambulatory and need rides to doctor appointments and medical treatments. The Road to Recovery Program uses volunteer drivers. Five business days advance notice is encouraged. Days/hours of service: Monday Thursday, (8:00am -5:30pm); Friday 8:00am 4:30pm (varies based on patient need). Length of advance notice required: At least 5 days if possible; the more notice given, the better chance to find an available screened & trained volunteer driver. Wheelchair lift: No patients must be ambulatory. Price: Free service available to all cancer patients based on availability of drivers. Payer sources: Free through American Cancer Society s Road to Recovery Program Blue Cross Blue Shield A service to BluePlus members who are ambulatory. Transportation provided to medical, dental, physical therapy or eye exams. Days/hours of service: Office hours are 8:00am 4:30pm, Monday Friday. Length of noticed required: 3 business day advance notice Wheelchair lift: No. Price: Free with plan. Payer sources: Insurance. BrightStar Care St. Croix Valley /7 phone support We accompany the client the entire time with a Companion, Certified Nursing Assistant, Licensed Practical Nurse, or Registered Nurse, depending on the care level needed for each situation. Call for more information on services and charges. Days/hours of service: 7 days a week, 24 hours a day. Length of advance notice required: 2 days. Wheelchair lift: No client must be ambulatory. Price: Dependent on the level of care required, plus $0.56 per mile. No mileage charge if we use the client s car. Please call for more information. Payer sources: We accept Private Pay, Medical Assistance, Veterans Administration, Medica, and Long Term Care Insurance. 3
4 Canvas Health (formerly Human Service, Inc.) For passengers not ADA certified, our private service is a system for those individuals preferring additional privacy time. Our second option is designed for groups wanting to share a common origination point and a common destination point while enjoying the fellowship of others. Days/hours of service: Monday through Friday, between 8:00am and 4:30pm. Length of advance notice required: 4 days. Price: For individuals: The cost of this service is $25 per ride ($50 round trip) within our service area, if available, Monday through Friday. For groups: The cost of this service is $70 per hour with a two hour minimum. Payer sources: Private pay Community Thread Transportation provided by volunteer drivers to medical appointments for persons who are live in the Stillwater School District (ISD 834), are ambulatory and have with no other means of transportation. There is a pre-screening process for riders. Qualified riders need to complete a pre-service application form prior to start of service. Days/hours of service: Monday Friday, 8:00am - 4:00pm (varies depending on the availability of volunteer drivers, distance to and from appointment, and city of rider s residence); no holiday, evening or weekend service provided. Office hours: 9:00am 1:00pm, Monday Friday. Length of advance notice required: minimum of 3-5 full business days prior to the day of the appointment. Will take ride requests up to 30 days before day of appointment. Wheelchair lift: No, riders must be ambulatory. Price: Fares apply for non-contract rides. Fares start at $15.00 for a round trip ride and $10.00 for a one-way trip. The cost is priced below commercial taxi services. Call for more information. Payer sources: Additional payer sources only for contract rides. Call to inquire. Disabled American Veterans of Minnesota The Disabled American Veterans of Minnesota strives to provide Minnesota veterans in need with transportation to VA Medical Center appointments through our DAV Volunteer Transportation Program. Disabled veterans who are unable due to financial or medical hardship to secure transportation to their medical appointments in the St Cloud and Twin Cities metro area. Days/hours of service: As volunteers are available. Length of advance notice required: at least a week. 4
5 Price: Free to disabled veterans with financial or medical hardship. Payer sources: N/A; donations are accepted to DAV of Minnesota Disability Linkage Line and A free statewide information and referral resource for persons with disabilities and chronic illness. Business hours are 8:30am 5:00pm, Monday Friday. Discover Ride office@discoverride.com Discover Ride is a medical transport service company providing a full range of medical transportation services in Minneapolis, MN. Please contact the office to inquire about various medical transport services provided by the company. Days/hours of service: Monday Thursday, 6:00am -6:00pm; Friday, 6:00am - 5:00pm. Length of advance notice required: at least 1 day. Price: $30 round trip plus $1.85 per mile Payer sources: Private Pay by check prior to ride or cash; also accept Medical Assistance, Insurance call for details. Driving Miss Daisy or Driving Miss Daisy is a local specialized Transportation company serving the twin cities metropolitan area and south central Minnesota. We offer a complete line of wheelchair accessible vans with rear entry ramps, para-lift equipped, and regular sedan vehicles to transport our clients. We provide transportation and companion services for seniors and those with disabilities in the greater Twin Cities Metro area and south central Minnesota. Days/hours of service: Monday Friday, 6:00am 6:00pm Length of advance notice required: at least 1 day. Price: Quote request available online Payer sources: Private Pay ( Personal and Corporate checks, Credit cards accepted: Visa, MasterCard, Discover & American Express); Medicaid STS Certification Required, MNET, most long-term care insurance plans; third party private payment and workman s compensation. 5
6 Elmo Special Transportation Services or Provides medical and non-medical transportation services in the Twin Cities area. Door-todoor service by appointment. Days/hours of service: Call for information. Length of advance notice required: Prefer 2 days notice. Wheelchair lift: No Price: Call for rates Payer sources: Call for information Fairview Senior Clinic Transportation Fairview Senior Transportation makes rides available to any Fairview clinic, Fairview-affiliated clinic or Ebenezer Adult Day program. All riders must be age 55 or older. Additionally, riders must be temporarily disabled and unable to drive themselves. (Note: Service is not available to Assisted Living or Care Center sites that already provide transportation.) Days/hours of service: Monday-Thursday, 7:00am to 5:30pm.; Fridays, 7:00am to 3:30pm. Length of advance notice required: Rides must be scheduled 24 hours in advance and within 20 miles of the East Bank address Harvard St. SE. Price: $5.00 per round trip (prices subject to change). Payer Sources: Private Pay. Handi-Lift Transportation or Wisconsin based company providing door-to-door non-emergency wheelchair lift transportation in western Wisconsin and Minnesota. First aid and CPR certified drivers. Can do Saturday dialysis transportation. Days/hours of service: Monday Saturday, 5:30am 5:30pm. Length of advance notice required: Prefer 2 days notice. Price: $48 round trip plus $1.47 per mile Payer sources: Private Pay, no Minnesota MA, call for insurance payment details. HealthEast Medical Transportation Lift-equipped van for medical appointments. Call for more information and rates. Medical assistance reimbursement may be available. 6
7 HealthPartners Ride Care Provides transportation to medical appointments to enrollees to qualifying insurance plans. Call for more information and rates. Home Helpers and Direct Link of the Twin Cities Providing both transportation and in-home compassionate companion care to seniors, and others including post surgery recuperative care and respite care. Our goal is to make life easier by providing services that help keep seniors and those with needs independent and in their homes. In addition to transportation and in-home services from Home Helpers, we also offer Personal Emergency Response Systems (PERS) through Direct Link. Providing help with just the click of a button. Days/hours of service: 7 days, 6:00am 10:30pm. Length of advance notice required: 48 hours notice is preferred. Wheelchair lift: No. Price: $24 per hour plus $0.56 mileage roundtrip from client s residence. Payer sources: Private Pay. No long term commitment, however a contract for services is required in advance of first service assignment. Home Instead Senior Care Provides friendly and personal door-thru-door transportation services for seniors in Washington County and surrounding areas. Our reliable, trained and insured CAREGivers SM can transport ambulatory individuals to physician appointments, religious services, sporting events and other social outings, as well as escort for all shopping and errand needs. A free face-to-face consultation is required before transportation services can be provided. Please contact our office for more details and rate information. Days/hours of service: 7 days per week, anytime. Length of advance notice required: at least 24 hour notice. Wheelchair lift: No client must be ambulatory. Price: $26 per hour and $0.55 per mile (mileage charge waived if our CareGiver drives the client s vehicle). Payer sources: Private Pay, no MA or insurance. Lionmobile Transportation Transportation for ONLY White Bear Lake Area School District 624 residents age 60 and older. Door-to-door assistance, wheelchair accessible. Days/hours of service: Monday-Friday from 8:15am 4:15pm. Length of advance notice required: at least 48 hours. 7
8 Price: $7 round trip within school district; $10.50 round trip to Maplewood Mall, Beam Avenue medical clinics, Maplewood Dialysis, and Salvation Army Adult Day Care; $18 round trip for trips to St Paul and surrounding communities. Payer sources: Private Pay. Maximal Care Mobility, LLC Provides medical and non-medical transportation for residents of Dakota, Ramsey and Washington Counties. Days and hours of service: Monday Sunday, 5:00am 10:00pm Length of advance notice required: At least 24 hours notice required. Wheelchair lift: Yes Price: Call for information about rates. Payer sources: call for information. Med-Van Handicapped equipped serving the metro area. Please call for more information and rates. Medica Provide-A-Ride For Medica Choice Care enrollees only. Metro Mobility or Serves people with disabilities with limited services in Washington County. First Transit provides Metro Mobility services in Washington County. Communities served include: Bayport, Baytown Township, Birchwood, Cottage Grove, Dellwood, Lake Elmo, Landfall, Mahtomedi, Newport, Oak Park Heights, Oakdale, Pine Springs, St. Paul Park, Stillwater, Willernie and Woodbury. Days/hours of service: Services hours vary by community. Length of advance notice required: Varies depending on off-peak ($3.00 one way trip) or peak hours ($4.00 one way trip) and day of the week. Call for more information. Metro Mobility also offers a downtown fare zone ($1.00) for trips beginning and ending in the downtown areas. Price: Please call for more information and rates. 9 Metropolitan Area Agency on Aging/ Senior Linkage Line and Provides linkage and referral to transportation services available to community members, not direct transportation. Free linkage service by telephone available Monday Friday, 8 am -4:30 pm, please leave a message after hours. Many helpful links for senior services on both websites. Metro Transit Provides bus service from Washington County to other cities in the metro area. Fares and hours of service vary please call for more information and rates. Minnesota Non-emergency Transportation (MNET) or MNET provides transportation to medical appointments for people on Medical Assistance and some pregnant women and children on MinnesotaCare who have no other transportation available to them. Days/hours of service: Monday Friday, 7:00am 6:00pm. Weekend/after hours for hospital discharges only. Length of advance notice required: 3 days for scheduled trips, and urgent trips as needed. Price: Per Medical Assistance rates. Payer sources: Medical Assistance, FFS and Waiver. Northland Transportation Special needs transportation to medical appointments or other destinations. Stretcher service available. Days/hours of service: Monday Friday, 7:30am - 4:30pm. Length of advance notice required: at least 48 hours. Price: please call for details. 10 Oromiya Transportation Service or OTS provides non-emergency transportation business in the Twin Cities metropolitan and surrounding areas. OTS is committed to providing services to clients with diverse needs and backgrounds. Our drivers are trained in cultural sensitivity and respect for diversity. We aim to provide drivers who are fluent in multiple languages. OTS provides special transportation services for a variety of occasions including trips for: doctor s appointments, hospitals, prescription drug pick-ups, religious services, long distance trips, schools, events and outings and much more. Days/hours of service: 5:00am to 9:00pm Length of advance notice required: two days (working days) Wheelchair lift: Yes Price: Prices vary based on the transportation service needed Payer sources: Private pay and other. Call for more information. Premier Handicap Services, LLC ; after hours call Locally owned provider of private non-emergency transportation services (STS) for individuals with special needs. Door-to-door service with handicapped-equipped van. Offers translation in French, Laotian and Vietnamese. Days/hours of service: Monday Friday 5:30am 11:00pm; Saturday-Sunday 6:00am 8:00pm; holidays by appointment only. Length of advance notice required: at least 24 hours. Wheelchair lift: Yes, including pull out bed for long travel. Price: $48 round trip plus $1.55 per mile. Payer sources: Cash or check, Medical Assistance, and insurance payment with prior authorization. Seniors Helping Seniors North Provides personalized, non-shared transportation for ambulatory seniors for medical appointments, shopping, church, visiting family and friends, special events and more. Can provide basic curb-to-curb service, door-to-door service, or even have someone remain with you during the duration of your visit if desired. Mostly services residents of Washington, Ramsey and Dakota Counties, and can transport to any location, even locations outside these counties. Please call for more details. Days/hours of service: 24/7 capability. Length of advance notice required: at least 2 days. Wheelchair lift: No, riders must be ambulatory. 10

Senior Linkage Line Provides information and referral to a wide range of services for seniors, including transportation. Not a direct transportation provider, referrals only. See also Metropolitan Area Agency on Aging listing. TLC Special Transportation TLC is a state certified special transportation company servicing the entire metro area. Vans and buses are available for private trips and outings as well as group transportation events. Days/hours of service: Monday Friday, 5:30am 5:00pm. Length of advance notice required: 3-4 days Wheelchair lift: Yes, if needed. Price: $48 round trip plus $2.50 per mile. Payer sources: Private Pay; and Blue Plus, Health Partners, Medica Choice, Medical Assistance and UCare insurance. Transit Link Provides shared ride, curb-to-curb service for medical and non-medical needs within the seven-county metro area. Fares are based on the distance traveled.
