Transportation is also available to governmental agencies such as the Social Security Office, Veteran's Services Office, and the Housing Authority. Eligibility: Must be a senior citizen (60 or older) living in the cities of Westminster and Midway City. Additional cities are covered under certain circumstances. Reservations are required and appointments are limited by availability., donations accepted City of Aliso Viejo Contact: (949) Transportation is provided to AV residents age 60 years and older with on demand door to door service to and from Sea Country Senior and Community Center in Laguna Niguel and the Florence Sylvester Senior Center in Laguna Hills for senior nutrition and activity programs. Service is available from 9:00 a.m. to 4:00 p.m. Monday through Friday, excluding holidays. Vehicles are wheelchair accessible. Interested residents must apply for program prior to scheduling. Eligibility: Aliso Viejo residents only, age 60 years and older Cost: Free City of Anaheim Contacts: Program Information: Trip Reservations: Transportation service is available for active adults age 60 plus to locations inside the City of Anaheim s city limits within a 10 mile radius of your home. You may use the Senior Wheels Program from any residence located in the City of Anaheim to the nearest City of Anaheim Community Center or Library for the purpose of using the site or connecting with the OCTA ACCESS program or the Congregate Meal Program offered at Brookhurst Community Center. Non-emergency medical appointments transportation within the CITY limits are also available. The Senior Wheels program will provide transportation to St. Jude Medical Center 101 E. Medical appointments for residents can be arranged (Tuesdays-Thursdays) up to three miles outside the Buena Park city limits. Eligibility: Buena Park residents, 60 years and older. Cost: Cost for medical appointment is $1 per person each way and the cost for the (ST*R) Program is on a donation basis. City of Costa Mesa

City of Cypress Contact: Joan Bardin (714) Nutrition Taxi - The Nutrition Taxi program provides transportation to the Cypress Senior Center for their Nutrition Program between 9:00 a.m. 1:00 p.m., Monday - Friday. Eligibility: Cypress Residents age 60 or older Cost: Free 2. Taxi Vouchers - The Taxi Voucher Program is a curb-to-curb taxi service providing transportation to medical appointments, grocery shopping, banking centers, etc. Each participant may receive up to six (6) taxi vouchers with a value of $11.00 each, per month. Eligibility: Cypress residents, age 60 or older, meet income requirements Cost: Free City of Dana Point Contact: Dana Point Senior Center (949) Vanna Murphy Door-to-door service to and from the Dana Point Senior Center for the congregate meal program. Eligibility: Must be a senior citizen (60 or older) residing in the city of Dana Point City of Fountain Valley Contact: Christy Borack (714) Eligibility: Fountain Valley residents 60 years and older Days/Hours of Service: Seven (7) days a week from 8:30 am to 4:30 pm a. Transportation is provided to Fountain Valley residents within the city of Fountain Valley and approximately up to one mile outside the boundaries; for non-emergency medical, nutrition, recreation and social activities, shopping, and other miscellaneous trips to meet the daily needs of participants.

Cost: $2.00 per ride City of Fountain Valley s Senior Transportation link: City of Fullerton Contact: Fullerton Community Center, (714) Program Description: Transportation service to and from the Fullerton Community Center, subsidized taxi vouchers and OCTA 30-day bus passes. Transportation service is limited to Fullerton city limits and up to 5 miles beyond the city limits. Eligibility: Fullerton residents age 60 and older. Cost: 30-day bus pass cost $4; $29 value taxi voucher cost $6 and $50 value taxi voucher cost $10. City of Garden Grove Contact: H. Louis Lake Senior Center (714) The H. Louis Lake Senior Center provides free transportation services to seniors who attend the nutrition program, educational classes, and activities at the H. Louis Lake Senior Center. This service is available Monday - Friday between the hours 8:00 a.m. to 3:30 p.m. Advance reservations are required. For more information about the H. Louis Lake Senior Center, please call Jackie McIntosh at (714) or for additional details on the Congregate Lunch Program you may contact Lori Ochoa at (714) Eligibility: Requirements for the Senior Mobility Program are that you must be a senior citizen (60 or older) and live in Garden Grove. Cost: The nutrition program is only a suggested donation of $3.00 for any senior 60+; other trips offered at no cost. City of Huntington Beach Contact: Surf City Seniors on the Go! (714) The Huntington Beach Senior Services Mobility Program (Surf City Seniors on the Go!) provides transportation to medical appointments, grocery shopping, senior center classes/nutrition site, special events, the Alzheimer s Family Services Center and life supporting medical trips, such as dialysis and chemotherapy within the City, as well as to Hoag, Orange Coast Memorial and Fountain Valley Hospitals. The service is available 8:30 a.m. to 4:30 p.m. Monday through Friday. Reservations are required five days in advance.

5 Eligibility: Must be 55+ and reside in the City of Huntington Beach. (Service not available for assisted living or skilled nursing facilities.) City of Irvine Contact: The TRIPS office at (949) for more information between 8:00 a.m. and 2:00 p.m., Monday through Friday, or visit the City of Irvine website at TRIPS program provides low cost, door-to-door, wheelchair-accessible transportation to Irvine seniors and adults with disabilities. All participants must be unable to drive due to a physical or cognitive disability. TRIPS supports independent living and community involvement through safe, reliable and professional paratransit service. Eligibility: Must be a current full time Irvine resident, 60 years of age or older who is unable to drive due to a physical or cognitive disability. This program also includes services for other populations - please call TRIPS for more information. Cost: $1.90 each way within the City of Irvine and $3.80 or more depending on location to neighboring cities. Trips to cities outside Irvine are limited to specific areas. Jewish Federation and Family Services of Orange County Contact: Sandy Baren at (949) or sandy@jffs.oc Transportation services are provided by the Silver Streak Senior Transportation Program to Orange County residents over 60 for non-emergency medical visits, grocery shopping, visits to senior and community centers, and other activities. The program offers volunteer rides, shuttles, group bus trips, taxi-vouchers (restricted to riders enrolled in volunteer driver program) and mobility management services. Eligibility: Orange County seniors age 60+ who are unable to drive Cost: $2.00 each way for volunteer rides of 12 miles or less. Rides over 12 miles require an additional $2 voucher each way. There are variable costs for shuttles, group trips, and taxi rides. Korean American Senior Association Contact: (714) Transportation services are provided to Korean-language dependent seniors living in the cities of Garden Grove, Buena Park, Anaheim, Santa Ana, Huntington Beach, Fountain Valley, Westminster and Orange. Transportation is available for non-emergency medical

6 visits, grocery shopping, and visits to senior and community centers. Service is available Monday through Friday 9:00 a.m. - Noon and 2:30 p.m. - 4:30 p.m. with some Saturday services available. Eligibility: Seniors age 60+ Cost: No charge to Korean American Senior Association members City of La Habra Contact: Call (562) for general information about the program or (866) (toll free) for reservations. La Habra Shuttle provides curb-to-curb van transportation anywhere within the City of La Habra and within one-half mile outside the city limits, Monday - Friday, 7:30 a.m. - 4:30 p.m., excluding some holidays. Twenty-four hour advance reservation required - same-day reservations allowed as space is available. Eligibility: Must be a resident of La Habra and a senior citizen. Application forms may be obtained from and returned to the La Habra Community Services Department, 101 W. La Habra Blvd., La Habra. Cost:.50 cent donation each way to and from all locations including the senior center. City of Laguna Hills Contact: Transportation Coordinator at (949) Eligibility: Service is provided to Laguna Hills residents who are 60 years of age or older. Application Required: Yes; you will need to bring proof of age and Laguna Hills residency. Cost: $5.00 per one-way trip paid for by resident (except for airport travel); For airport travel, participants pay $16.00 per one-way trip to John Wayne Airport. Travel is permitted anywhere within the combined city limits of Laguna Hills and Mission Viejo for shopping, errands or doctor s appointments. Travel to and from select satellite locations outside of Laguna Hills, Laguna Woods or Mission Viejo is also available at the following locations: Kaiser Hospital, 6640 Alton Pkwy, Irvine Irvine Spectrum, 71 Fortune Dr., Irvine Lake Forest Nursing Center, Old Trabuco Rd., Lake Forest Freedom Village El Toro Rd., Lake Forest Laguna Niguel/Mission Viejo Metrolink Station, Forbes Rd., Laguna Niguel Irvine Metrolink Station, Barranca Parkway, Irvine

7 Airport Travel: John Wayne Airport, Airport Way, Santa Ana City of Laguna Niguel Contact: Sea County Senior & Community Center - (949) Door-to-door taxi service from home to Sea Country Senior & Community Center. Individualized round trip taxi transportation service operates Monday-Friday 9:00 a.m. to 4:00 p.m. for Laguna Niguel residents to participate in activities offered at the Center. Call the Senior Center (above number) for application or go to Eligibility: Laguna Niguel residents only, age 55 and over. Must not own a car or be able to drive. City of Laguna Woods Contact: Taxi Voucher Program (949) or website at cityhall@lagunawoodscity.org. After vouchers have been purchased, riders may call the California Yellow Cab Company directly at (877) Taxi Voucher Program provides taxi service 7 days a week - 24 hours a day. Number of vouchers required for a taxi trip will depend on trip distance. Special reduced fare taxi vouchers are available: Special Destination Voucher: $6.00 each way to the Irvine Spectrum or Irvine Transportation Center. General Travel Voucher Books $50 ($100 value) $25 ($50 value). Non-Emergency Medical Transportation: The City s non-emergency medical voucher program provides services to medical appointment-doctors, dentists, vision care specialists, physical therapy, medical treatments, etc. at deeply discounted fares. It is available to all residents 60 years and older. Costs: General Travel Voucher Books $50 ($100 value) or $25 ($50 value) available for residents 60 years and above. City of Lake Forest Contact: Senior Scene Clubhouse at (949) or Reservations: (949) The City of Lake Forest offers door-to-door transportation service to Lake Forest seniors participating in the Senior Scene Clubhouse activities. Transportation is available Monday through Thursday from 10:00 a.m. to 2:00 p.m. Reservations must be made 24 hours in advance. Eligibility: Must be a Lake Forest resident and 60+ years of age.

8 Cost: $0.50 cents each way City of Mission Viejo Contact: Barb Cassidy, Contract Manager (949) or Mark Nix, RCS Director (949) or visit: for guidelines about the program. The City of Mission Viejo offers curb-to-curb cab service 24 hours/day and 7 days/week. A lift equipped van is available with 48 hour advanced notice. All participants must be selfsufficient in all aspects of travel or they may provide a personal care attendant. Service is provided within the city limits of Mission Viejo and Laguna Hills and to select satellite destinations. This program does not offer medical therapy transportation (i.e. dialysis, chemotherapy, radiation treatments, etc.) Eligibility: Mission Viejo residents 60 years and older with approved application and City of Mission Viejo issued photo identification card. Cost: $5.00 one way and $10 per round trip. City of Newport Beach Contact: OASIS Senior Center (949) The City of Newport Beach provides curb-to-curb transportation through its Care-A-Van and Shuttle programs. The Care-A-Van program provides transportation to medical appointments, grocery shopping and other errands within city limits. The Shuttle program provides transportation to the OASIS Senior Center for classes and activities. Service is available Monday - Friday, 8:00 a.m. - 4:30 p.m. Reservations recommended 2-3 days in advance. Eligibility: Must be 60+, live at home, reside within Newport Beach, Corona del Mar or Newport Coast and be unable to drive. Cost: $3 each way for the Care-A-Van program; $1.50 each way for the Shuttle program. City of Orange Contact: (714) Limited transportation services are provided to the Orange Senior Center for the congregate meal program. Transportation service to the Center is available Monday through Friday 9:00 a.m. -1:00 p.m. Eligibility: Seniors age 60+; Must live in the City of Orange. Application required.

City of Placentia Contact: Placentia Senior Center (714) The senior transportation program provides door-to-door transportation to the Senior Nutrition Program and Senior Center, Monday - Friday, 9:00 a.m. to 1:00 p.m. Eligibility: Must be a senior citizen (60 or older) living in the City of Placentia. Cost: No charge. City of Rancho Santa Margarita Contact: Community Services Division at (949) to register. Taxi Voucher Program provides taxi service 7 days a week, 24 hours each day. Vouchers may be purchased at Bell Tower Regional Community Center or via mail. Currently vouchers are not available, please check back in July Eligibility: Must be a resident of the City of Rancho Santa Margarita and 60+ years of age. City of San Juan Capistrano Contact: San Juan Capistrano Community/Senior Center (949) On demand door-to-door transportation service to and from the San Juan Capistrano Community/Senior Center, Monday Friday 8:00 a.m. 4:00 p.m. with the exception of holidays and city office closures. Eligibility: Must be a senior citizen (60 or older) residing in the City of San Juan Capistrano. City of San Clemente Contact: Laura Ferguson at (949) or Age Well Senior Services Transportation Division at (949) The Shopper Shuttle Service provides curbside or door-to-door (upon request) fixed-route transportation four days per week (Monday Thursday) to shopping destinations within the City of San Clemente (no service on holidays or weekends). Also available is daily roundtrip transportation to the Dorothy Visser Senior Center where seniors can enjoy meals, activities and classes. Eligibility: San Clemente resident seniors (age 60 and older). for the transportation

10 City of Santa Ana Contact: Rocio Meza at (714) The Senior Transportation Program provides shuttle service for two senior centers: Santa Ana Senior Center, 424 W. Third St., (714) ; and Southwest Senior Center, 2201 W. McFadden Ave., (714) The Santa Ana Senior Center and Southwest Senior Center programs include senior special events and activities, classes, lunch, special monthly excursions and shopping trips within the City of Santa Ana for all participants. Eligibility: Must be 60+ and a resident of Santa Ana Cost: No charge. City of Seal Beach Contact: For general information about the program contact Heather Nichols at (562) ext. 1350, or visit the website at and search on Public Works, Traffic and Transportation; For scheduling, call Keolis at (562) The City of Seal Beach offers three senior mobility programs: Congregate Meal Program transportation provides service to Senior Meals Monday through Friday. Schedules are coordinated with each registered senior participant. Pre-registration is required. Shopping Shuttle provides trips on Monday through Wednesday from Leisure World to Shops of Rossmoor/Target Center from 2:00 p.m. 4:30 p.m. Also on every Thursday, except for holidays, from Leisure World to Shops of Rossmoor/Target Center only, from 9:00 a.m. to 1:00 p.m. and from Leisure World to Old Town only, from 1:00 p.m. to 4:30 p.m. Carry-on restrictions apply. The Dial-A-Ride Program provides trips for senior residents to any location within City limits for doctor s appointments, and retail/grocery shopping. The program also provides trips within Orange County up to three miles outside the city for doctor s appointments only. Dial-A-Ride service is available Monday through Friday from 8:00 a.m. to 4:30 p.m., and can be utilized by calling (562) at least 24 hours in advance. Eligibility: Must be a senior citizen (60 or older) and a resident of Seal Beach. 11 On demand, door-to-door van service to and from the Stanton Senior Center. Eligibility: Must be a senior citizen (60 or older) residing in the City of Stanton. City of Tustin Contact: (714) The Tustin Area Senior Center provides curb to curb transportation for medical appointments, grocery shopping and Senior Center activities, Monday through Friday from 8:00 am to 4:00 pm. Registration is on a first come, first served basis and reservations are required. A five day advanced reservation is recommended. Transportation is limited to all of Tustin and three road miles outside of the city limits, as well as St. Joseph Hospital. Eligibility: Tustin residents 55 years and older. Cost: $1.00 suggested donation Vietnamese Community of Orange County, Inc. Contact: (714) The Vietnamese Community of Orange County provides transportation for seniors to a congregate meal program, the Asian Senior Center, medical, dental, and other physician s offices. Service is available Monday through Friday 7:30 a.m. - 4:30 p.m. Please call 3 days in advance of your anticipated appointment. Eligibility: Seniors age 60+. Please call the VNCOC to see if the program is currently accepting new participants. Villa Park Contact: (714) The Villa Park Taxi Cab Program is a curb-to-curb service for Villa Park residents 60 years of age or older. Riders are allowed 8 one-way trips per month (or 4 round trips) and are responsible for a $5 co-pay per one-way trip. Travel is limited to 10 miles per one-way trip. Overages are to be paid by the rider. Cost: $5.00 co-pay City of Westminster Contact: Transportation Coordinator (714) , ext. 3675

12 The Westminster Senior Center Transportation Program provides two services: The Nutrition Van provides curb-to-curb transportation to the Senior Center Luncheon Program and is designated for participants who attend the luncheon program at least once per week. The Shuttle provides a route from the Senior Center, mobile home parks and senior housing complexes to designated shopping destinations within the City of Westminster. Scheduled pick up times are 1-1/2 to 3 hours apart. Eligibility: Must be a senior citizen (60 or older) living in Westminster. Cost: $0.50 suggested donation per one-way trip for the Nutrition Van; other trips are offered at no cost. City of Yorba Linda Contact: For information about the program please call (714) ; for reservations, please call (714) 528-RIDE (7433). TRAILS (Taking Retired Adults Into Local Services) provides door-to-door transportation from 8 a.m. to 4 p.m., on Tuesday, Wednesday, Thursday and Friday throughout the City of Yorba Linda and within a one-mile radius outside the city to local services, including Placentia-Linda Hospital. A 24-hour advance reservation is required: however, reservations can be made 7 days in advance and 24 hour advance reservation is required. Participants may complete their application and obtain an identification card at the Yorba Linda Community Center, 4501 Casa Loma Avenue, Yorba Linda, CA or by calling (714) Eligibility: For seniors 60 years and older. Participants must be able to be picked up and dropped off within with City of Yorba Linda boundaries. An application and a TRAILS identification card must be obtained prior to scheduling a ride. Cost: $1 each way
