A Traditional Healer provides holistic traditional care to those with diabetes in the areas of doctoring (including medicines), ceremonies, spiritual advice and counseling. teaching circles are provided both on and off site. Classes are available to anyone either by referral from a health care or service provider, or by self-referral. Individual diet counseling is available to any resident of the River East area (postal codes R2K, R2L, R2G, R2E) with a family physician referral. Description of Care, and Support Service: Classes are held every 4-6 weeks (except in July, August, and December due to historically poor attendance). People can choose to attend either four Monday evenings or four Tuesday mornings. Two classes are facilitated by a dietitian and two classes are facilitated by a nurse. Individual counseling with a Registered Dietitian is available to any resident of the River East area (postal codes R2K, R2L, R2G, R2E) with a family physician referral. Clinic is run on Thursdays in our clinic (and as needed), and is staffed by a Nurse and a Dietitian. People must be clients of our clinic to attend. Care is done in consultation with the client s health care provider. Services: Learning Series Presentations This suite of modules is targeted towards people diagnosed, undiagnosed, as well as those at risk for developing diabetes. It provides consistent, up-to-date referenced diabetes information. Volunteers and employees have been trained to deliver these presentations that include the following topics: : What you need to know; Living Well with About the CDA for Healthcare Workers Aboriginal and Low literacy versions are also available. Presentations can be tailored by adding information about Making Healthy Food Choices, Becoming More Active, Maintaining a Healthy Weight and Preventing the Complications of. Support Services: The Association provides empathetic and supportive staff to those who call the toll-free Information and Support Line at BANTING. They are specially trained to listen to callers concerns, identify needs, refer to organizations and services if necessary, and mail out information. These staff also has access to broader resources, both within the Association and outside. The Canadian Association also offers: a user friendly web site; printed resources and information packages on diabetes prevention and management; diabetes education events; public awareness displays; travel health insurance and credit life insurance; publications such as Dialogue; cookbooks for sale

The following topics are discussed: what is diabetes, including discussion about how did it happen, management of diabetes through healthy eating and lifestyle adjustments, blood sugar values and control, and complication screening. R2M 5M3 Phone (204) Fax (204) Web site Population Served (include catchment area): R2N, R3M Description of Care, and Support Service: Centre Youville Centre a community-based health resource, offers coordinated approach to the delivery of self-management for adults with diabetes education at two sites. Living Well with is a health promotion education series that is part of the Chronic Disease Management Program provided through Youville Community Health Resource Centre, St. Vital. Living Well with provides diabetes selfmanagement education for adults affected by diabetes and their family and support persons. The program provides interactive group and individual sessions. Participants are invited to attend follow-up sessions. Nurses, dietitians and a counselor collaborate with community pharmacists and other agencies to provide local and timely diabetes education, care and support in the community of St. Vital. Foot care nurse services are also available for a fee. Ask a Nurse program is a primary health care initiative through Youville Centre Community Health Resource Centre, St. Vital. Ask a Nurse program can be a point of entry and provides assistance to navigate access to diabetes education, care support and referral within the larger community of Winnipeg. The community has access to nurses who can address immediate concerns related to the individual s health and diabetes. A partnership is maintained with the Youville Centre nurses and dietitians for expert consultation and/or referral. Youville Centre is accredited by the Canadian Council on Health Services Accreditation. This organization meets national standards of quality for health services set by Council. This accreditation award has been achieved through the combined efforts of the entire organization and those it serves

R2H 0S8 Phone (204) Fax (204) Web site Population Served (include catchment area): Winnipeg Region Description of Care, and Support Service: The Youville Centre (YDC) is an accredited community Health Centre, servicing the Winnipeg area. The Centre offers a self-management education program for adults affected by type 1 and type 2 diabetes, as well as women pregnant with diabetes. YDC also offers a unique clinic with access to Endocrinology for young adults less than 25 years old with type 1 diabetes, in partnership with the Maestro project. The education team includes certified diabetes educators (nurses, and dietitians), a counselor and foot care nurse (). The personalized care begins with initial contact from referral, and may include individual and/or group education sessions. Bilingual services and evening access are available. YDC mentors health professionals and encourages industry partnerships and student practicum. The team actively partners with other organizations to ensure access to healthcare education and services, such as Home Care nursing, Income Security and the Canadian Association. YDC is accredited by the Canadian Association Standards Recognition Program and the Canadian Council on Health Services Accreditation. Youville Centre and the Canadian Association partner to provide the public with information that can help prevent the onset of type 2 diabetes. Canadian Association staff/volunteers deliver a one-time group presentation, directed to adults at risk and their families. Options to attend grocery store tours or cooking classes through the Youville Center program are available and clients may register for Eating for A Healthy Heart or Lose Inches Feel Energized (LIFE) at St Vital
