1 A HEALTHIER The District s Clinic will be hosting free flu shots and no cost or low-cost wellness screenings at various District locations in November! Employees are encouraged to take advantage of this special, convenient opportunity to participate in this District wellness initiative to protect against the flu and to participate in a personal wellness screening to assure a healthier U. It s fast, easy and convenient to participate! The attached flyer outlines the services that will be available for employees who wish to receive a free flu shot and participate in a wellness screening at free or low cost, depending on insurance enrollment. (See attachment for details). The attached flyer will also provide details regarding the specific dates, times, locations, and costs (when applicable), as well as appointment information for your wellness screening. Please join us in assuring A HEALTHIER for the school year!
2 Please read the following flyer for details regarding the District s Wellness Screening and Flu Shot Clinic In March of this year, the District opened the employee Clinic to provide quality, affordable, convenient health care for its employees and their covered dependents. To continue that initiative to provide wellness opportunities, the District is proud to announce a Wellness Screening and Flu Shot Clinic from November 19 th through November 21 st. This comprehensive and voluntary Wellness Screening and Flu Shot Clinic is hosted by The Clinic, through the University of Oklahoma College of Community Medicine and OU Physicians Work, as well as the Tulsa Health Department. This wellness intiative has two voluntary components, the Wellness Screening (information starting on this page) and the Flu Shot (information starting on page 3). Please read both sections to learn details regarding each component. WELLNESS SCREENING OPPORTUNITY (The acronym PWP below refers to personal wellness profile.) Wellness/PWP Biometrics The biometrics portion of the Wellness/PWP Screening will consist of blood testing, body mass index (BMI), waist-to-hip ratio (WHR), and blood pressure screening. Blood Testing: Your blood will be tested for lipids (total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides) and glucose (blood sugar). As you may know, having high cholesterol or triglyceride levels in your blood may lead to increased risk for heart disease and stroke. Additionally, your glucose level will be tested to screen for diabetes. It is best to draw blood after fasting for at least 8 hours, so the test results will be more accurate. However, the blood test can still be a useful screening tool even if you are not able to fast. Body Mass Index (BMI): Your BMI will be determined during your wellness screening. BMI is a practical measure for approximating total body fat and is a measure of weight in relation to height. A higher BMI is associated with increased risk for heart disease, diabetes and other ailments. Waist-to-Hip Ratio (WHR): Your waist-to-hip ratio (WHR) will also be taken during the screening, to compare waist circumference to hip circumference. Persons with extra weight located around the middle can be at higher risk for diseases such as heart disease and diabetes than those who carry weight around their hips and thighs. Blood Pressure: Your blood pressure will also be taken during the screening. High blood pressure is associated with increased risk for heart disease, stroke and other ailments. Wellness/PWP Eligibility and Cost You are eligible for the comprehensive and confidential Wellness/PWP as follows: Available free for employees/participants and covered dependents (18 years or older) who participate in the District s PPO, High Deductible PPO, or HMO. Available free for part-time, non-provisional employees, who have an employment date of August 31, 2008 or earlier. 1
3 $40 fee for any employee that currently waives the District s PPO, High Deductible PPO, or HMO. Wellness/PWP Screening Location, Time, and Dates PPO, High Deductible PPO, or Waived: Appointment required; see below. The following locations, times and dates are for those covered employees/participants and dependents, age 18 or older, of the District s PPO, High Deductible PPO, and employees currently waive the District s insurance: UMAC (Classroom III) 7:00 a.m. until 5:00 p.m. and November 20 th Clinic 7:00 a.m. until 5:00 p.m., November 20 th, and November 21 st ESC Service Center 8:00 a.m. until 5:00 p.m. 8 th Grade Center 7 a.m. until 5 p.m. 6 th /7 th Grade Center 7 a.m. until 5 p.m. I-High 7 a.m. until 5 p.m. o November 20 th (If more appointments are necessary, an additional day may be announced. Each location will experience a short lunch break and no appointments will be made during those times.) HMO: Appointment required; see below. The following locations, times and dates are for those covered participants and dependents, age 18 or older, of the District s HMO: UMAC (Classroom III) 7:00 a.m. until 10:30 a.m. Clinic 7:00 a.m. until 10:30 a.m. (If more appointments are necessary, an additional day will be announced.) Appointments for the Wellness/PWP Screening In order to ensure that all those interested in obtaining a Wellness/PWP Screening are provided ample time to have the biometrics taken; anyone interested in the wellness screening must make an appointment for a specific time slot. The appointment is expected to take no more than ten (10) minutes. All participants are asked to complete an online Wellness/PWP questionnaire prior to his/her appointment for 2
4 taking the biometrics and instructions will be provided when the appointment is made. If you have recently seen your personal physician and, as part of a doctor s visit, had blood work done and would still like to participate in the Wellness Screening, you may also complete the online Wellness/PWP questionnaire; however, you would just need to bring a print out of your test results with you to your scheduled Wellness Screening appointment. Please contact one of the following staff members in Human Resources to schedule an appointment for your Wellness/PWP Screening or send an to set an appointment via and to receive instructions in completing the online Wellness/PWP questionnaire. Please make sure that you make your Wellness/PWP Screening appointment by October 31, Carlene Foust foust.carlene@unionps.org Erin Wyatt wyatt.erin@unionps.org Carol Levesque levesque.carol@unionps.org Celia Armstrong armstrong.celia@unionps.org After the initial Wellness/PWP Screening, you will have an opportunity to make an individual appointment with the Clinic nurse to receive your health screening results, where you will receive any applicable follow-up recommendations. This follow-up appointment at the Clinic is included as part of the screening (no additional cost). FLU SHOT OPPORTUNITY Eligibility You are eligible for the free flu shot if you are: A District full-time employee, a part-time, or a provisional employee. Flu Shot Cost Free ($0) Flu Shot Location, Time, and Dates The following locations, times and dates are for those interested in receiving a free flu shot (or flu mist as quantities last). Please note that no appointment is necessary for the flu shot. However, to ensure that we have enough flu shot vaccine and/or mists, please sign up for your flu shot with your school nurse (or the Benefits Department if you are not at a school site) by October 31, UMAC (Classroom III) 7:00 a.m. until 5:00 p.m. and November 20 th Clinic 7:00 a.m. until 5:00 p.m., November 20 th, and November 21 st ESC Service Center 8:00 a.m. until 12 Noon 8 th Grade Center 7 a.m. until 5 p.m. 3

Confidentiality of Your Health Information The Clinic, through the University of Oklahoma College of Community Medicine and OU Physicians Work, as well as the Tulsa Health Department, are committed to maintaining the privacy of your personal health information. Your personal health information will only be accessible to authorized members of OU Physicians and will NOT be available to others outside of OU Physicians without prior authorization from you. Aggregate/summary data on the entire employee group will be reported to the District, to be utilized in improving wellness, but the data will be de-identified so that no employee s individual data will be known by the District.
