1 Chinatown Night Market Yè Shì Vendor Application Thursday, October 13 th 10th and Race Streets 7:00pm-11:00 pm* *Time and location subject to city approval The Philadelphia Chinatown Development Corporation are proud to bring you Chinatown Night Market Yè Shì, a roving street festival spotlighting great neighborhood restaurants and food trucks. Inspired by vibrant outdoor markets popular in Asia, Night Markets are social outdoor food courts where vendors sell a variety of street foods made with fresh, local ingredients. Night Markets are a lively gathering place with delicious, multi-ethnic foods, locally-brewed beverages and live music. Night Markets are proving that festivals can have high quality, diverse, amazing food and showcases the best of what the city has to offer. Past Night Markets have generated crowds of over 25,000 people! PLEASE NOTE: The entry fee has been established to offset event costs and to support the sustainability of Night Market in Philadelphia Chinatown Application Deadline for Chinatown Night Market Yè Shì: Thursday, September 8th, 2016 Instructions on submitting your application and payment are enclosed. Both application and payment must be submitted by the deadline to avoid the late fee. A late fee of $50 will be added if your application and payment are not received by the deadline date, or for any returned checks. LATE APPLICATIONS MAY NOT BE CONSIDERED.

Menu items should be: high-quality, easy to eat, smaller portioned and reflect regional foods in the US and around the world. Menus should be limited and include items that cost $6 or less. List menu items and ideas (food and/or beverage) that you would like to serve at the Night Market. Chinatown Night Market Yè Shì emphasizes the use of traditional recipes and local ingredients with an Asian flare preferred. Please feel free to share any relevant information about the food you are preparing (e.g. emphasis on sourcing local, organic ingredients, made from scratch, etc.). You are welcome to use more space for your description and/or include any marketing materials or other pertinent information about your business to this application. If applicable, where do you currently sell your product?

3 Licenses, Permits & Insurance Proper permits are required to vend at all Night Markets. If you currently do not hold proper permits, they need to be secured prior to the event. Please contact Betsy Lee if you have any questions. In order to participate in Night Market, you must have or obtain the following: A Philadelphia Food Safety Certificate (if you hold a permanent food license) If you re a mobile vendor/food truck: A Health Department Vendor ID # and a mobile vending license A Permanent Special Event license or Temporary Special Event Permit (health department applications must be completed and submitted separately) Certificate of Insurance for General Liability Insurance coverage with Philadelphia Chinatown Development Corporation ( N 9 th Street, Philadelphia PA 19107) listed as additionally insured with a $1,000,000 limit. Electricity Important: Electricity at Night Market is not guaranteed. We highly recommend that you come prepared with your own electricity source. Please let us know if you need electrical support. There is a 1500 watts maximum limit per vendor. There will be an additional $75 fee to secure an electricity source for you at the event. If you are requesting electrical support and pay the surcharge, make sure that we know what your electrical needs are (For example: 1 portable hand-washing sink and 1 fluorescent light: both use 120v plugs). Please come prepared with at least (2) 50 extension cords as well. My electrical needs are for: Set Up/Equipment The health department requires that all vendors have proper tents, tables and hand washing stations. See attached rental worksheet if you need to rent equipment (you may provide your own). For additional rental needs, contact Betsy. Participants are responsible for adhering to all Department of Health regulations. Participation & Costs: Thursday, October 13 th, 7-11pm* Mobile Trucks** or tents (upto 26 in length).....$275 Mobile Trucks** or tents (26 50 in length)...$350 **Absolutely no tents are allowed to be in front of your truck Cancellation Policy If you back out of Night Market after September 22 nd we will not be able to refund your entry fee. Payment should be made out to Philadelphia Chinatown Development Corporation and mailed to the address below, attn: Betsy Lee. Applications can be returned via scan/ , fax or mail. Payments and applications may also be dropped off in person.

This signature page Rental & cost worksheet Night Market vendor fee ($275, or $350, depending on length of truck/tents): made payable to: Philadelphia Chinatown Development Corporation PLEASE NOTE: LATE APPLICATIONS MAY NOT BE CONSIDERED A late fee of $50 will be added if your application and payment are not received by the deadline date. The health department requires that all vendors have proper tents, tables and hand washing stations as listed below. If you already have the specific equipment listed below and you do not need to rent, put a 0 in the Quantity and Total columns. See below for example. 3. Input your total cost at the bottom. All rental equipment will be delivered to the Night Market location on the day of the event and picked up after.
