1 Document and Fee Verification at Reporting Centre (NIT DURGAPUR) for B. TECH Admission in the Academic Year (JoSAA CSAB 2015) The candidate at the reporting centre has to produce original documents for verification. The verification official will sign on the filled reporting form submitted by the candidate after checking the personal details, allocation details and documents of the candidate and finding the details to be correct in all respect. Following are the list of documents to be checked with production of original documents as mentioned below: 1. Printout of provisional allotment letter for the particular round for which the provisional admission is going on. 2. Printout of locked choices and terms and conditions taken from the CSAB website and duly signed by the candidate. 3. Original documents for verification i. Photo ID proof as per Govt. India norms (In case of Photo on JEE Admit card is not clear) ii. iii. iv. Admit Card of JEE (Main) Score Card of JEE (Main) issued by CBSE ( downloaded score card is acceptable). Class X (High School) Board Certificate as proof of date of birth and Name of Candidate. v. Certificate and Marks sheet of qualifying examination. (see Section 6.4 of information brochure for the list of qualifying exams of information brochure) vi. vii. viii. Certificate of category (SC/ ST), if applicable, issued by the competent authority in the prescribed format (Annexure - IV) Certificate of category of OBC-NLC (central list), if applicable, is to be issued by the competent authority in the prescribed format (Annexure - V) and should clearly mention that the candidate belongs to Non Creamy Layer. The certificate should have been issued based on the parental income in the financial year viz. 1 st April 2013 to 31 st March 2014 Non-Creamy Layer OBC Certificate issued based on the parental income in the financial year viz. 1 st April 2013 to 31 st March 2014 be accepted in any case. Certificate for Persons with Disabilities (PwD), if applicable (Annexure - VI). The certificate should be issued from a duly constituted Medical Board. The certificate would be validfor a period of 5 years for those whose disability is temporary. For those who acquired permanent disability, the validity can be shown as permanent. Certificate not issued by a Medical Board or invalid / expired on the date of reporting shall not be accepted in any case. 1

2 ix. If the candidate has passed the qualifying examination from outside India then passport is required for determining the 'State code of eligibility'. For Indian nationals passing the Class XII (or equivalent) examination from Nepal/Bhutan, the state code of eligibility will be determined on the basis of permanent address in India as given in the passport of the candidate. x. Proof of place of examination centre of the final examination from the National Open School for candidates having Senior Secondary Certificate; this is required for determining the 'State code of eligibility'. xi. Copy of e-challan of online payment of initial fee of Rs. 45,000/ for candidates with the category tag GEN, OBC-NCL or Foreign and Rs. 20,000/- for candidates with the category tag SC, ST, GEN-PwD, OBC-NCL PwD, SC-PwD or ST-PwD (The fee should be paid through e-challan only. Any other form of payment will not be accepted) 4. One set of self-attested copies of all the documents listed above are to be retained in the counselee file (to be issued from NIT DURGAPUR) of the corresponding candidate at the reporting centre (NIT Durgapur). The originals will be returned after verification and self-attested copies will be retained by the Reporting Centre. 5. One passport size photograph identical to the one pasted on the JEE (Main)-2015 application form. 6. Submission of Annexure I, II, III are mandatory in nature for all candidates, however other Annexure IV (For SC & ST Candidates), Annexure V (For OBC-NCL Candidates), Annexure VI (For PwD Candidates) are to be submitted. Annexure IV, V, VI are to be issued by the competent authority before the candidate reports at the reporting centre. Note:If the original certificates are not in Hindi/English, duly certified Hindi/English version translation of such certificates will be required. In case of any discrepancy, the verifying officer will accordingly mark corrections in the Reporting form at Annexure - I. The verifying officer may specifically update the State code of Eligibility, Category, sub-category (as per change in category rule from OBC -NLC / SC / ST to OP), Gender (M to F or F to M), status of qualifying examination. Updates in category from OP to OBC-NLC/SC/ST or PwD are not permissible under any circumstances. Finally, the verifying officer will sign on the Reporting form submitted by the candidate after checking the personal details, allotment details and documents. The candidates have to enclose one set of self-attested Xerox copies of the documents verified and one passport size photograph in the given folder. The team at this counter will ensure that the candidate has signed all the relevant and appropriate declarations / undertakings. All Annexure (ANNEXURE I to ANNEXURE VI) are given below 2
