All students and instructors must sign in on the Connecticut State Firemen s Association Attendance form and the roster (attendance form) shall be included with this signed permit. Date of Evolution: Contact Number: Organization Using Facility: Organization Supervisor: Instructor-in charge: Safety Officer: Permission Form I, Chief/Supervisor hereby authorize the attached roster from Fire Department / Organization to participate in Live Fire Training at the Fairfield Regional Fire School. I also understand that the firefighters will be covered by my Department s/organization s Worker Compensation Insurance coverage while participating in such training and that the Town of Fairfield, Fairfield Fire Department, its officers, directors, agents or instructors shall not be liable for any injuries or death sustained during such training. These applicants are considered by my Department s/organization s standards to be physically and emotionally fit to perform the fire fighting evolutions without special considerations and where applicable, meet the requirements of OSHA CFR for the use of self-contained breathing apparatus and OSHA CFR for protective clothing. 2 This Live Burn Policy sets forth a "MINIMUM" requirements and shall not limit the Instructor-in-charge or the Safety Officer from exceeding it if he/she feels it is necessary to ensure the safety of the students or instructors. In the event of an emergency or injury during training. Call 911. A phone is located at the main entrance to the building and inside all classrooms. Classroom phones require that you dial 9 for an outside line and then 911. Separate water sources shall be utilized for the supply of attack lines and backup lines. Prior to conducting live fire training evolutions a pre-burn briefing and walk-through of the building shall be conducted. During the briefing the Instructor-in-charge and or the Safety officer shall ensure that participants have knowledge and familiarity with the building to facilitate evacuation. If rescue mannequins will be used in the evolution this information will be relayed to participants. At no time shall a live person(s) be allowed to play the role of victim inside of the burn building. Burn ONLY Class A (wood) combustibles - [NO treated wood]. Absolutely NO flammable liquids exception: limited quantity of diesel to start fire. NO tires. NO upholstered/stuffed/foam type furniture that includes mattresses. Burn ONLY in the container(s) provided; do not move them. NO fires on the concrete floor. Fuel must be contained within the burn container(s); fuel may not extend more than 1 foot above the side of the burn container or extend over the sides. Absolutely NO fire of any kind in the Tower portion of the building; when burning, keep the door between the burn room and the tower CLOSED. There will be no live fires set on the second floor of the smoke house, the stairway leading to the second floor, or the tower itself. The Fairfield Regional Fire School attempts to appoint a Safety Officer for all live fire evolutions. If the school is unable to provide a safety officer, the organization conducting the live-fire training MUST appoint a Safety Officer for the duration of the exercises. If you cancel a scheduled training session, please contact the school ASAP. All those who are to participate in interior operations SHALL familiarize themselves with the interior layout of the first and second floors of the burn building paying particular attention to means of escape. A charged hose line min. 1 ¾ in. size must be available during the burn. When training is completed, clean-up area inside and out. It is best to allow any fire to burn out naturally as long as no nuisance is posed to any surrounding residential areas; the balcony door on the 2 nd floor may be left open for ventilation. VEHICLE/OUTDOOR FIRES NOT permitted if wind is blowing towards homes on Fairfield Beach Road or, if smoke would be a nuisance to any surrounding area. Outdoor activities to be concluded NO LATER than 10 PM. Live Fire Instructor Training Permit Page 2 of 6

Coordinate lighting of fires with instructor-in-charge 4. Ensure compliance of participants personal protective equipment 5. Ensure that all participants are accounted for, both before and after each evolution 6. In the event of an emergency or injury during training. Call 911. A phone is located at the main entrance to the building and inside all classrooms. All hose lines checked: Sufficient size hose for the area of involvement Supervised by qualified instructors/officers Adequate number of personnel 4. Necessary tools and equipment positioned 5. Participants checked: Approved full protective clothing Compliant SCBA with Personal alarm devices All equipment properly donned 6.
