2 Kaiser Permanente is committed to providing its employees with a competitive total compensation package that includes more than just a paycheck. This comprehensive package offers you and your family an array of valuable benefits, including medical and dental coverage. You may also receive paid time off to use in the event of an illness, for vacation, or to spend time with your loved ones. Its aim is to support a healthy lifestyle mind, body, and spirit. In addition, you may have the option to elect disability and life benefits to provide you and your family with financial assistance during hard times. Kaiser Permanente also offers generous retirement benefits, including a traditional pension plan and a tax-deferred retirement savings plan. Additional benefits include flexible spending accounts, professional development opportunities, employee discounts, and much more. This brochure gives you a summary of the benefits portion of the total compensation package you may be eligible for as a Kaiser Permanente employee. Eligibility You will find eligibility requirements for you and your eligible dependents at the beginning of each section. These requirements vary based on the benefit. Eligible Dependents You may enroll your eligible dependents in several of the benefits offered to you. An eligible dependent includes your spouse or domestic partner, civil union partner, or reciprocal beneficiary (see page 2), as well as your or your spouse, domestic partner, civil union partner, or reciprocal beneficiary s children. In order to cover a dependent child, he or she must meet the following requirements: Benefit Medical and Dental Dependent Life Insurance Dependent Accidental Death and Dismemberment Insurance Children Must Be Under the age of 26, regardless of student, marital, or financial dependent status (coverage will continue through the end of the month in which your child turns 26) Under the age of 21, or 23 if enrolled as a full-time student at an accredited school or university, and unmarried Under the age of 25; and unmarried Note: You may cover a dependent child past the regular age limits if he or she is incapable of self-support due to a mental or physical disability that begins before the child reaches the age limit, as long as the child was also covered under your benefit plan prior to reaching the age limit. 3 Domestic Partner/Civil Union Partner/Reciprocal Beneficiary Benefits You may extend certain benefits, such as medical and dental coverage, to your domestic partner, civil union partner, or reciprocal beneficiary and his or her dependents, provided you submit the necessary documentation. Benefits extended to your domestic partner, civil union partner, or reciprocal beneficiary and his or her dependents may be taxable, in accordance with the law. Benefits by Design You are eligible if you are in a position with an employment status of 20 or more hours per week. Coverage begins on the first day of the month following three months of employment (based on your benefit service date). Benefits by Design is a flexible benefits program that allows you to customize your benefits to meet your individual and/or family needs. The program includes your choice of: Medical plans Accidental death and dismemberment coverage Dental plans Disability income coverage Life insurance coverage Flexible spending accounts Certain benefits, such as retirement and time off benefits, are offered separately from the Benefits by Design program. 90-Day Plan Until you become eligible for Benefits by Design, you are provided with the 90-Day Plan (effective on the first of the month following your date of hire or on your date of hire if you are hired on the first of the month), provided you are in a benefit-eligible status. This plan includes the Basic medical plan for yourself and your eligible dependents, and Employee Life insurance for yourself only equal to approximately one times your annual salary (not to exceed $50,000). Kaiser Permanente pays the full premium costs of the 90-Day Plan for you and your eligible dependents. Your 90-Day Plan coverage will end when you become eligible for Benefits by Design. Benefits by Design Credits Under Benefits by Design, you receive a specific amount of employer-provided credits to purchase benefits. Credits are applied to the first two pay dates of each month. You will receive approximately enough credits to purchase: Basic medical plan (including Supplemental Medical) for yourself only Basic dental plan for yourself and your eligible dependents Employee Life insurance coverage equal to approximately one times your annual salary Long-Term Disability for yourself at 50 percent of your base salary If you are an employee with an employment status of 26 to 31 hours per week, your credits are funded at 80 percent. If you are an employee with an employment status of 20 to 25 hours per week, your credits are funded at 60 percent. 2 Hawaii Salaried June 2013

4 The cost of your benefits will depend on the options you choose. The difference between the credits you receive and the cost of your benefits will determine whether or not you will have out-of-pocket expenses, break even, or receive additional after-tax dollars in your paycheck. Default Coverage If you do not enroll in Benefits by Design during your initial enrollment period, you will receive default coverage for the remainder of the year. Your next opportunity to make changes to your coverage is during the annual open enrollment period, unless you have a qualifying family or employment status event. When you are in default coverage, you do not receive any Benefits by Design credits. Default coverage includes: Basic medical plan coverage for yourself only Employee Life insurance coverage equal to approximately one times your annual salary (not to exceed $50,000) Health Care Options Your comprehensive health care benefit helps keep you healthy, and provides you with valuable protection in the event you become ill or injured. The Benefits by Design flexible benefits program offers you a choice of three Kaiser Foundation Health Plan (KFHP) medical plans. You may also choose to waive your coverage if you provide proof of coverage in another medical plan. Kaiser Foundation Health Plan You can cover yourself and your eligible dependents (see page 1). You may use your Benefits by Design credits to purchase this coverage. KFHP coverage includes basic and major medical care, such as routine and pediatric checkups, immunizations, mammograms, hospitalization, lab tests, medications, and emergency care. Benefits by Design offers a choice between three plan options with varying copayments and other applicable charges. 5 Supplemental Medical You are automatically enrolled if you elect any of the three medical options offered under KFHP. You can cover yourself and the same eligible dependents (see page 1) you cover under KFHP. Coverage begins at the same time as your KFHP coverage. This coverage is provided at no cost to you. Supplemental Medical reimburses you for certain medical services not covered, or that exceed the limits, under your Kaiser Permanente-sponsored medical plan. In most cases, the plan pays 80 percent of reasonable and customary charges and covers such medical services as acupuncture and chiropractic care. You must meet an annual deductible, and some services are subject to annual or lifetime maximum limits. You may be responsible for filing your own claims. Dental Care Options Kaiser Permanente s benefit program includes comprehensive dental coverage to help provide for your and your family s dental care needs. Hawaii Dental Service You can cover yourself and your eligible dependents (see page 1). You may use your Benefits by Design credits to purchase this coverage. Benefits by Design offers a choice of two Hawaii Dental Service plans. The plans cover many of the same services, however, the costs and coverage levels vary. You may also choose to waive dental coverage. 6 Life Insurance Benefits by Design offers life insurance options that may provide financial assistance for you and your dependents in the event of a serious injury or death. Employee Life Insurance You must be actively at work for benefits to be effective. You may use your Benefits by Design credits to purchase this coverage. You may purchase Employee Life insurance coverage at the following levels: $10,000 $50,000 (in $10,000 increments) $100,000 $750,000 (in $50,000 increments) No coverage Newly eligible employees may elect up to $100,000 of Employee Life insurance without providing Evidence of Insurability (EOI), which is proof of your good health. If you do not elect Employee Life insurance when you are first eligible, you will be required to provide EOI if you wish to purchase coverage at a later date. Dependent Life Insurance You may cover your eligible dependents (see page 1). However, reciprocal beneficiaries are not eligible for Dependent Life insurance coverage. You must be actively at work for benefits to be effective, and your dependents must not be confined at home, in a hospital, or other medical institution, for benefits to be effective. You pay the premiums for this coverage through after-tax payroll deductions. You have the option to purchase Dependent Life insurance coverage for your eligible dependents at the following levels: Spouse or Domestic Partner/Civil Union Partner 1 Each Child 1 $10,000 $2,000 $20,000 $4,000 $30,000 $6,000 $40,000 $8,000 $50,000 $10,000 No coverage No coverage 1 Spouse or domestic partner/civil union partner coverage is bundled with child coverage. Note: You must elect Employee Life insurance coverage of at least twice the amount of the spouse or domestic partner/civil union partner Dependent Life coverage you wish to purchase, and enroll each eligible dependent you wish to cover under Dependent Life insurance. If you do not elect Dependent Life insurance when you are first eligible, you will be required to provide EOI, which is proof of your eligible dependents good health, if you wish to purchase coverage at a later date. 7 Accidental Death and Dismemberment You may cover yourself only, or yourself and your eligible dependents (see page 1). However, reciprocal beneficiaries are not eligible for Accidental Death and Dismemberment insurance coverage. You must be actively at work, and your dependents must not be confined at home, in a hospital, or other medical institution, for benefits to be effective. You may use your Benefits by Design credits to purchase this coverage for yourself. You pay the premiums for your eligible dependents coverage through after-tax payroll deductions. Accidental Death and Dismemberment (AD&D) provides additional income protection in case of an injury or death as a result of an accident. You have the option to purchase AD&D coverage for yourself at the following levels: $10,000 $50,000 (in $10,000 increments) $100,000 $350,000 (in $50,000 increments) If you choose to cover your dependents, your enrolled spouse or domestic partner or civil union partner is covered at 50 percent of your coverage and each child you enroll is covered at 10 percent of your coverage. You must enroll each eligible dependent you wish to cover under Dependent AD&D. If you do not elect AD&D when you are first eligible, you will be required to provide Evidence of Insurability, which is proof of your good health, if you wish to purchase coverage at a later date. Business Travel Accident Insurance You are automatically enrolled if you are in an employment status of 40 hours per week. Coverage begins on your date of hire. Kaiser Permanente provides this benefit at no cost to you outside of Benefits by Design. If you die as a result of an accident while on company business, your beneficiaries will receive a benefit equal to four times your annual salary, with a minimum of $100,000 and a maximum of $250,000. Disability Income Kaiser Permanente offers you disability benefits to provide you with financial assistance in the event you are unable to work for an extended period of time because of a serious illness or injury. Temporary Disability Insurance You are automatically enrolled if you are in a position with an employment status of 20 or more hours per week. Call-in or part-time employees who meet specific employment criteria may also be eligible for TDI. Coverage is effective on your date of hire or benefit eligibility date. You are responsible for a portion of the premium for this coverage. Temporary Disability Insurance provides you with 58 percent of your average weekly earnings if you become ill or disabled due to an off-the-job sickness or injury, provided you meet the eligibility requirements. 6 Hawaii Salaried June 2013

8 Supplemental Disability Insurance You are eligible if you are in a position with an employment status of 40 hours per week and have less than two years of service. Coverage if effective on your date of hire, provided you are actively at work. You pay the premiums for this coverage. Supplemental Disability insurance provides 50 percent of your base salary up to a maximum of $2,000 per month for up to two years. You must complete a qualification period of six months of total disability before benefits commence. Long-Term Disability You must be actively at work for benefits to be effective. You may use your Benefits by Design credits to purchase this coverage. Long-Term Disability (LTD) covers a portion of your salary if you are unable to work due to disability. You have the option to elect coverage at the following levels, or you may waive coverage: 50 percent of base pay (60 percent when integrated with other disability income) 60 percent of base pay (70 percent when integrated with other disability income) Benefits are payable after six months of continued disability. Certain restrictions apply to pre-existing conditions. If you do not elect LTD when you are first eligible, you will be required to provide EOI, which is proof of your good health, if you wish to purchase or increase your coverage at a later date. Flexible Spending Accounts You may enroll if you are in a position with an employment status of 20 or more hours per week. If you enroll, your participation will begin on the first day of the month following three months of employment (based on your benefit service date). You must re-enroll in these accounts each year during open enrollment to continue participation. Benefits by Design offers you a choice of flexible spending accounts that allow you to set aside pre-tax dollars to pay for certain expenses through automatic payroll deductions. Only expenses incurred after you are actively enrolled are eligible for reimbursement. Certain Internal Revenue Service (IRS) restrictions govern these accounts, so you should carefully estimate your needs each year. Health Care Spending Account You can contribute up to $2,500 annually on a pre-tax basis to pay for eligible health care expenses (e.g., copayments for medical and dental services, or expenses over your plan limits). Dependent Care Spending Account You can contribute up to $5,000 annually on a pre-tax basis to pay for eligible dependent care expenses (e.g., licensed day care center or practical nursing care for an adult). 9 Time Off Benefits Kaiser Permanente offers you a number of programs to help you keep your home and work life in balance. Your time off benefits include: Observed Holidays Vacation Sick Leave Observed Holidays You may be eligible for the following paid holidays if you are a part-time, regular part-time, or full-time employee. New Year s Day Independence Day President s Day Labor Day Kuhio Day Thanksgiving Day Memorial Day Christmas Eve Day Kamehameha Day Christmas Day Vacation You are eligible if you are a part-time, regular part-time, or full-time employee. You may use your vacation hours for leisure, hobbies, family matters, or other activities of your choice. Full-time employees with an employment status of 40 hours per week are eligible for vacation according to the following schedule: Length of Continuous Service Hours per Pay Period 1 Days per Year 1 Maximum Accrual (Hours) Upon eligibility to completion of 3rd year Start of 4th year to completion of 6th year Start of 7th year and beyond Based on a full-time schedule. Accruals are prorated for part-time employees. The maximum number of vacation hours you may accumulate is listed in the above chart. In-Service Cash-Out The In-Service Cash-Out (ISCO) program allows you to cash out vacation hours that you will accrue in the year following the cash out election at 100 percent of their value and receive the money for those hours in your paycheck as taxable income. You may elect to cash out a minimum of 24 hours, or in 40-hour increments up to 160 hours, as long as you do not exceed your expected annual accrual for the following year and you maintain a 40-hour vacation balance. ISCO elections can only be made during the annual open enrollment period. 8 Hawaii Salaried June 2013

10 Sick Leave You are eligible if you are a regular part-time or full-time employee. Sick leave may be used for those times when you have an illness or injury and cannot come to work. Full-time employees with an employment status of 40 hours per week accrue 3.70 hours of sick leave each pay period (prorated for regular part-time employees). The maximum number of sick leave hours you may accumulate is 800. Sick leave benefits accrue from your date of hire or benefit eligibility date and may be used at the completion of the first pay period. Other Time Off In addition to paid time off benefits, you may be eligible for the following paid leaves: Bereavement Leave, in the event of the death of an immediate family member. Jury Duty, for when you need to serve on jury duty. Kaiser Permanente also provides certain types of paid and unpaid leaves, such as Family, Medical, Military, and Personal leaves. Retirement Preparing for a financially secure retirement is just as important as funding your lifestyle today. Kaiser Permanente offers retirement programs designed to help provide you with financial assistance down the road. Kaiser Permanente Salaried Retirement Plan You automatically become a participant in the employer paid Kaiser Permanente Salaried Retirement Plan (KPSRP) after you complete one year of service during which you are compensated for at least 1,000 hours. The KPSRP is a defined benefit pension plan that provides you with retirement income based on your compensation and years of credited service. You are vested in this plan after five years of service, or after one year of service if you are age 65 or older. In general, you may receive a benefit when you retire at age 65, or earlier if you meet the eligibility requirements for early retirement. Kaiser Permanente Supplemental Savings and Retirement Plan You are eligible regardless of your employment status and work schedule. You automatically become a participant after two years of employment. Kaiser Permanente will contribute an amount equal to 5 percent of your base salary to this plan. You may also elect to make after-tax contributions into the plan and invest them in a broad range of investment funds. You are immediately vested in both the employer and employee contributions to this account. Hawaii Salaried June

11 Kaiser Permanente Tax-Sheltered Annuity Plan You are eligible regardless of your employment status and work schedule. You are automatically enrolled in the plan on your date of hire with an employee contribution of 2 percent of your eligible pre-tax earnings. You have 45 days to opt out if you do not wish to participate, and you may increase or decrease your contribution rate at any time. The Kaiser Permanente Tax-Sheltered Annuity Plan is designed to help you save money for retirement and reduce your current taxable income. You may contribute up to 75 percent of your pre-tax earnings (or to the current IRS limit) into the plan and invest your contributions in a broad range of investment funds. You are immediately vested in your account. Each year after your first year of participation, your employee contribution rate will be increased by 1 percent, until you reach a 6 percent contribution rate. You may opt out of the automatic increase feature at any time. Retiree Medical Coverage You may be eligible for retiree medical if you are at least age 55 and have a service date of 15 or more years with Kaiser Permanente when you retire under the provisions of the pension plan. You can cover yourself and your eligible dependents. Coverage is effective when you turn age 65. This benefit is provided at no cost to you. Once you and your eligible dependents become eligible for Medicare, you and your eligible dependents must participate in Medicare Parts A and B and enroll in the Kaiser Permanente Senior Advantage plan to be eligible for retiree medical coverage. Other Benefits Employee Assistance Program You are eligible on your date of hire regardless of your employment status and work schedule. This program provides referrals to professional resources for child and elder care referrals, as well as confidential counseling for issues with alcohol or drugs, marital difficulties, family crises, financial or legal questions, and emotional concerns. All services are entirely confidential, and are not reported to your employer. Education Assistance Program You are eligible if you have at least one year of continuous employment as a full-time or regular part-time employee. Education Assistance is provided to assist you in continuing your education in subjects that will help improve your job performance and increase your potential for advancement and employability. You may be reimbursed for expenses (e.g., tuition and textbooks), up to the plan maximum. 10 Hawaii Salaried June 2013

Employee Discounts As a Kaiser Permanente employee, you may purchase discounted tickets for movie theatres and theme parks, health and fitness programs, travel, mobile phone service, computers, and more. You may also purchase over-the-counter medications and other products such as vitamins and sunblock at a discounted price from any Kaiser Permanente pharmacy when you show your employee identification. Health and Wellness Kaiser Permanente s Healthy Workforce program offers resources and tools to help you the people who power KP be as healthy as you can be by teaching and encouraging you to be active, eat well, and thrive. In addition, Kaiser Permanente-sponsored farmers markets make it a little easier to eat your five servings of fruits and vegetables for good nutrition. This is a brief summary of your benefits in effect when this booklet went to press.
