2 Summary of Benefits This brochure is a brief summary of current benefits. It contains information about who is eligible, when benefits are effective and the amount of s contribution. These benefits represent a significant portion of the overall compensation plan extended to employees. For assistance in understanding this brochure, refer to the following definitions of employment (your employment status will be explained during your interview): Exempt Non-Exempt s paid on an annual salaried basis. s who are covered by the overtime provisions set forth in the Personnel Policy. Overtime Hourly Call Pay Additional one half times regular hourly rate for work performed on holidays Additional dollar amount based on job title for work performed on 2nd and 3rd shifts or weekends Additional one half times hourly rate over 40 hrs. worked in a week Upon completion of 6 weeks of employment for shift differential; upon employment for weekend differential Department Specific Additional compensation for hours of on call duty Insurance Group Term Life Insurance Dependent Life Insurance Additional Life Insurance Life insurance equal to your annual base salary. Includes AD&D coverage. Up to a maximum of $150,000 Life insurance equal to $4,000. Includes AD&D coverage Optional coverage for spouse and children or no coverage. $5,000 coverage for spouse and $2,500 coverage for each child over 14 days old Long Term Disability Insurance Short Term Disability Insurance Critical Illness Supplemental options to Group Term Life insurance include: ½, 1, 1½, 2, 3, 4 times or no coverage. Up to a maximum of $500,000 Income protection pays 50% of base salary in event of disability Pays 50% of base salary up to $1,000 per week in the event of a disability Pays a tax-free lump sum benefit directly to the employee. Guaranteed issue benefit amounts of $15,000 and $30,000 for employee. Spouse and/or child(ren) are covered at 50% of the employee s benefit amount., benefits begin after 3 month elimination period, benefits begin after 15 or 30 day elimination period.. (Must enroll by calling enrollment center within 30 days of hire for guaranteed issue.) 4
5 Insurance continued Critical Illness continued Pays a tax-free lump sum benefit directly to the employee. Guaranteed issue benefit amounts of $15,000 and $30,000 for employee. Spouse and/or child(ren) are covered at 50% of the employee s benefit amount.. (Must enroll by calling enrollment center within 30 days of hire for guaranteed issue.) Whole Life (with long term care rider) Provides a death benefit for your beneficiaries if you pass away but can also build a cash value that you can use while you are still alive. A long-term care rider is included which provides benefits for home care, home health, or adult day care.. (Must enroll by calling enrollment center within 30 days of hire for guaranteed issue.) You will receive a health payment debit card to pay for eligible health care expenses directly from your flexible spending account. Vidant Clinic employees plus spouses and dependents age 16 years and up on the Vidant Health Medical Plan Clinic located at Vidant Medical Center that provides minor sick care, preventive care, physicals, chronic disease management, medication management and counseling services. pays, Vidant Health Medical Plan copay applies. Vidant Wellness Center employees and families Wellness Centers located in Greenville, Washington and Ahoskie with state-of-the-art exercise equipment, fitness classes, pools & aquatics programs, adult and child wellness programs. Vidant Health subsidizes membership; employee pays discounted rate Vidant Assistance Program Pharmacy Assessment, referral and/or short-term service for personal problems that may impair work attendance or performance Discount on prescription and over the counter products Vidant Health pays for first six sessions; employee pays for outside referrals Early Return to Work Program ows sick/injured employees to return to work in alternative positions until able to return to their normal jobs or new positions 6
