4 Financial Wellness Your pay is an important part of the overall total compensation package Lilly offers for the work you do and the contributions you make. Lilly also provides Financial many benefits to help you Career with personal and/or family needs as well as plan for your future and support your financial. Cash Compensation At the foundation of Lilly s compensation programs is our pay-for-performance philosophy. We ve developed our pay programs so your individual performance and our company s overall performance have a direct impact on the amount of money you earn. Each year, we benchmark our jobs to compare our compensation package with what other companies offer to their employees. This comparison is based, in part, on the type of work you perform and your location, and it s designed to ensure that at Lilly you ll earn top-tier pay for top-tier performance. Life Insurance Providing family security in the event of your death is another aspect of benefits. Eligible employees currently receive free coverage for a death benefit equal to two times annual base salary. Additional life insurance, both for the employee and for his or her dependents, is available for employees to purchase. You also can choose to reduce your coverage by half in exchange for credit to purchase other optional benefits. Flexible Spending Accounts Out-of-pocket health care expenses, as well as dependent day care expenses, can add up quickly. Flexible spending accounts can help you defray some of those expenses by allowing you to set aside pre-tax dollars to pay for them. By using pre-tax payroll deductions for eligible health care and dependent day care expenses, you increase your disposable income by reducing the amount you pay in taxes. Retirement Benefits: 401 (k) Plan Lilly currently provides a dollar-for-dollar match on the first 6 percent of base salary you contribute to your account. The company s matching contribution is made on a pre-tax basis and invested according to the investment selections you make for your own contributions. Participants become vested in employer contributions after two years of credited service. You can contribute on a pre-tax or after-tax (Roth) basis or a combination of both. You also can increase your contribution rate automatically each year. It s a benefit you can start soon after you arrive, in an effort to realize your retirement goals. Financial Planning and Investment Advice benefits Comprehensive financial planning information and investment advice is available to help you determine how much you need to save to meet your goals. You can talk with a licensed, professional financial planner any time you have a financial planning question or opt to have your Lilly 401(k) investments professionally managed. Online tools, articles, and calculators can be accessed at any time to assist you with your financial needs, including development of your own financial plan. 5 Your Health At Lilly, people s health is at the heart of what we do. That means the health of others is our primary concern even more so for our employees. We give you Physical the choice of health Community insurance options as well Social as other health services designed to keep you and your family healthy while managing the costs of medical care. Health Insurance Eligible employees can choose among two consumer-driven [high-deductible health plan] options, which are used in conjunction with a health reimbursement arrangement [HRA] or health savings account [HSA] through the Lilly Health Plan. If your eligible, out-ofpocket medical expenses in any year exceed approximately 6 percent of your base salary, the remainder of that year s eligible expenses is currently paid at 100 percent for the rest of the plan year. Your health insurance also includes vision coverage. Domestic Partner Benefits Lilly provides domestic partner health, prescription drug, and dental benefits to eligible active and retired employees. Partners may be of the same or opposite sex. Active employees may choose any of the health insurance options described above to provide health care coverage for themselves and their eligible domestic partner; however, a domestic partner must meet dependent status according to IRS guidelines to use the HSA account dollars. Prescription Drug Benefit In the HRA or HSA options, all drugs, including Lilly prescription medicines, are treated the same as other medical expenses meaning you pay the full negotiated cost for both Lilly and non-lilly non-preventive medicines until you satisfy the total annual deductible. Preventive medicines are covered at your respective co-insurance percentage for HRA or HSA plans and are not subject to the deductible. Once you satisfy your total deductible, prescriptions are covered at the same co-insurance percentages as other medical services except you will receive certain Lilly medications at no cost. Dental Insurance Dental coverage is available to eligible participants at a cost separate from medical coverage and with no deductible. In-network, it covers 100% of diagnostic and preventive care, 80% of minor restorative and 50% of major restorative. There is a $2,000 per person annual maximum and a $2,000 per person lifetime orthodontia limit. Financial Career Long-Term Care Insurance Employees can purchase plans for themselves and their spouse or domestic partner that provide up to $50,000 coverage for long-term custodial care as a rider to a permanent whole life policy. Employee Health Services From on-site care of minor illnesses, to preventive health exams and programs, to on-site physical therapy and emergency care, you ll be able to get help for many health-related concerns while at work. Eligible employees also can receive free screening mammographies, screening gynecological exams, screening colonoscopies, flu shots, and immunizations for business travel. Employee Assistance Program When a personal problem becomes too difficult to handle alone, employees can use the employee assistance program at no cost for a confidential assessment and referral to local, licensed, professional counselors. By calling 1 [800] 90-LILLY, you and your eligible dependents are each entitled to three visits per issue for problem assessment and appropriate referrals per calendar year. If you exhaust your three visits, you may still be eligible to receive certain mental health and substance abuse benefits through your mental health coverage. Tobacco-Free Workplace Lilly is committed to providing a safe and healthy workplace and promoting the health and of our employees. To encourage non-smoking, to minimize the exposure of non-smokers to tobacco smoke, to minimize the potential for fire, and to protect the quality of Lilly products, the smoking of tobacco and the use of smokeless tobacco is prohibited at all Lilly facilities, including entryways to buildings and parking lots. This applies to all employees, contractors, and visitors. For all Indiana facilities, Indiana law also prohibits smoking within eight feet outside any exterior public entrances. 6 Social Financial Career Time Off You need some time to call your own. We understand that everyone needs a break. We all do. Keeping this in mind, we offer you a range of vacation and personal leave options to give you the time you want, when you want it. Vacation and Holidays During your first year at Lilly, you ll earn 10 hours of vacation for each month of service. The amount of vacation time will start with 120 hours [15 days] per year and will increase in year 10, and every five years thereafter up to the 25th year of service, culminating with 216 hours [27 days] per year. Employees who complete 10 years of service, and for every five years of service thereafter, also receive 40 additional hours of recognition vacation in those anniversary years. Eligible employees also can purchase up to 40 hours of additional vacation time each year, and up to 80 hours of additional vacation time in anniversary years. Employees who don t want this much vacation can trade up to 40 hours of vacation time, in four-hour increments, for credits they can use to obtain other benefits. Lilly also provides an average of nine paid holidays and an additional annual year-end break for four of the working days between the days the Company designates for the observation of Christmas Day and New Year s Day. Most calendar years, this totals 28 paid days off your first full year at Lilly. Time off without pay may also be available for certain personal reasons, including use of the Family and Medical Leave Act as well as an educational leave. Maternity Leave Lilly s expectant mothers are able to leave work with pay up to one month before their baby s due date. This leave continues with pay according to Lilly s Illness Pay Plan until release from care, generally six weeks following the birth. An additional six weeks of unpaid leave is also available, providing employees with a 16-week maternity leave. Personal Absences and Leaves You may take time off with pay with your supervisor s approval for certain personal reasons, including: Birth of child [up to 40 hours for father] Adoption or foster placement of child [up to 40 hours] Illness in the immediate family [up to 64 hours] Death in the family [up to 40 hours] Illness pay program - based on Lilly service [e.g., up to four months of illness paid time off for employees with less than one year, up to 15 months illness paid time off for employees with service of ten years or more Other personal business, with approval [up to 24 hours] Your Total Compensation & Benefits Packages Fixed-duration Employee & Postdoctoral 6

Used both on occasional basis or as a customized arrangement, employees and supervisors have the opportunity to identify a work schedule that helps employees be their most productive. 8 Family Employees with children or elder dependents need to manage special challenges. To alleviate some of these pressing issues, we offer our employees who are caring for others some outstanding Social options. Community LifeCare Resource and Referral Service LifeCare, a Lilly-paid benefit, offers employees a wide variety of work and life services. Available 24 hours a day, employees can find information on anything from child and adult care to health and wellness, to relocation and associated topics. Nursing Mothers Program New mothers who choose to continue breastfeeding when they return to work may utilize one of the Nursing Mother Rooms available at specific locations. A hospitalquality electric breast pump is provided in private spaces. New mothers may also talk with a lactation consultant. Career Adoption Assistance Lilly reimburses eligible employees up to $10,000 of qualified expenses for the adoption of a child. Adoption assistance includes not only financial reimbursement, but also information about the adoption process and time away from work. Support Groups Employees with similar personal needs meet for information sharing and support. Existing support groups include those for new mothers, employees caring for elders, and parenting. On-Site Child Development Centers Two Indianapolis centers offer high-quality care on a full-time or part-time basis for dependent children age 6 weeks to 5 years. The center also provides backup care when regular childcare arrangements are unavailable. National Back-Up Care Back-Up Care Advantage is a national service offering center based back-up care for well children, in-home back-up care for healthy and mildly ill children and in-home back-up elder and adult care. Lilly s Global Day of Service Our annual Global Day of Service ranks among the largest single-day volunteer initiatives of any company in the United States. In 2013, about 20,000 employees in nearly 60 global locations volunteered. Employee Volunteerism Program We hope you ll find the time to give back to your community through volunteer support. To encourage this volunteer spirit, the Lilly Foundation will provide a $250 grant to eligible community organizations in which an employee has continuing, active involvement. The company also allows, under certain circumstances and with management approval, time away from work for volunteering. Lilly helps ease a few of those responsibilities by providing a range of employee conveniences. No-Stress Dress Code Forget about struggling into formal business attire every day, unless your job or your tastes call for it. Dress at Lilly is business appropriate/business casual, which means you can dress in a way that best fits your work environment and schedule for the day. In Some Lilly Locations Food Services On-site cafeterias offer a variety of meals and snacks, while gourmet coffee shops provide a tasty perk and plenty of vending areas help to keep you energized. Banking and Postal Services Who has time for banking? You will. The Eli Lilly Federal Credit Union offers banking services on-site and online, including checking, savings, financial planning, loans, auto leasing, home mortgages, insurance, and the option of buying a new car in one stop with home delivery service. Nearby, you can purchase stamps and send your mail. Convenience Store It won t replace your grocery, but the on-site store at the Lilly Corporate Center allows you to purchase items such as gifts, cards, birthday cakes, flowers, and toiletries. Dry Cleaning and More Save yourself from after-work errands with on-site dry cleaning, shoe repair, and alteration services. Summer Science Camp/School Vacation Programs School-aged children of Lilly employees can enjoy fun day camp activities enhanced with science activities during the summer break at the Lilly Recreational Park located in Indianapolis. In order to help you stay active, we offer an assortment of Employee Activities, from exercise classes and sports to hobby clubs and organized events all throughout the year. Free Gym Membership Employees and their dependents 18 or older are eligible for free access to participating facilities in the nationwide Prime Fitness network. LIFE Building and Colonel s Field Located in Indianapolis, the LIFE Building is a wellness destination supporting physical fitness, stress relief, personal development and team events. Included is a bike hub for those cycling to work and a number of CityBikes that can be used to ride around town.
