1 Dear Junior Leader: THE FURNACE Where the Best Become Better National Youth Leadership Training NYLT June 9 to June 15, Camp McKinley, Lisbon, OH Congratulations on being nominated by your Troop to attend the National Youth Leadership Training Conference! During the week of the conference you will have a chance to try out many new Scoutcraft skills that you can use in the program in your own Troop or patrol. Since you are a Leader in your Troop you naturally want to know how to do your Leadership job better. The Skills of Leadership will be the main topic of the conference. During the week you and other youth Leaders from all over our council will be living, camping, working, and learning together as members of a patrol and Troop. A great staff has been recruited to assist in the training. Refer to the attached Suggested Personal Equipment Checklist for what to bring. If you have any problem with this list, check with your Scoutmaster for help. Please complete the attached Personal Resource Questionnaire, Knowledge of Outdoor Skills form, and Parent's/Guardian's Consent form. Please see that these forms are completed and brought with you to the Pre-Conference Orientation. Bring your filled out and signed Personal Health and Medical Record Form. Your physical form will be returned to you before you leave McKinley Scout Reservation on June 15, You will NOT be permitted to remain in camp without a current physical form. The mandatory, Pre-Conference Orientation is scheduled for May 19th, 2013 at Faith United Methodist Church, th Street, North Canton, OH (See attached map). The program will begin at 2:00 p.m. Be sure to bring all the forms filled in and signed to this meeting. This will be the only participant meeting before the conference. Your attendance at this meeting is necessary to coordinate all the necessary documents, answer any questions that you may have, and finalize any financial, health, and medicine control requirements. During the orientation you will also receive arrival and departure instructions including dates, times and map to McKinley Scout Reservation. The conference will be busy with many activities. For this reason, we must discourage visitors during the week. Mail will be delivered to us daily. 2 THE FURNACE 2013 Buckeye Council, BSA National Youth Leadership Training - Camp McKinley, Lisbon, OH PARENT'S / GUARDIAN'S CONSENT & OVER THE COUNTER MEDICATION RELEASE In consideration of the benefits to be derived, and in view of the fact that the Boy Scouts of America is an educational organization, membership in which is voluntary, and having full confidence that every precaution will be taken to ensure the safety and well-being of my son during this conference, I hereby agree to his participation and waive all unjustified claims against the leaders of the National Youth Leadership Training and officers, agents and representatives of the Boy Scouts of America. The adult leaders of the National Youth Leadership Training Conference are accountable for the safety of your scout, but they expect your son to take responsibility for his own safety and to obey the adult leader s safety rules and instructions. However, if your son's actions are not mature enough to show responsibility for his own safety, then the adult leaders will not let him risk personal injury, nor will they take any further liability risk. You will be called to come get your son and take him from the conference immediately. In the event I cannot be reached in an emergency, I hereby give permission to the physicians selected by a designated representative of the Boy Scouts of America to authorize emergency medical care, hospitalize, secure proper anesthesia, or to order injections for my son. I request that measures be instituted without delay as judgment of medical personnel dictates. My son is in good health, has all required immunizations and I assume the health responsibility for him. By my signature below as parent or guardian, I agree with and will adhere to the statements above. Additionally, I give permission for the camp Health Officer to administer the following over the counter medications to my son if needed during participation at NYLT. I understand that said Participant will only be allowed ½ the maximum recommended daily allowance (listed below) without contacting me or my representative for further direction in the care of said Participant. Participants may obtain and take at the time of obtaining the following over the counter medications: Acetaminophen, 500mg tablets (Tylenol ingredient) (4 tablets within 24 hrs; 12 yrs. old min. age) for pain relief. Acetaminophen, 160mg tablets (Tylenol ingredient) (3 doses within 24 hrs.) 12 yrs. old 640mg = 1 dose for pain relief Pepto-Bismol, 262mg (8 tablets or 4 doses within 24 hrs; 12 yrs. old min. age) for treatment of upset stomach, diarrhea, nausea, indigestion, and heartburn. Diphenhydramine, HCI 25mg (Benadryl ingredient) (3 doses within 24 Hrs.) (1 dose = 25mg to 50 mg >12 yrs. old) (1 dose = 25mg <12 yrs. old) for treatment of sneezing, nasal congestion, runny nose, and itchy, watery eyes. Loperamide Hydrochloride, 2mg (similar to Imodium A-D) Over 12 yrs. old - 3 tablets for treatment of diarrhea. Be prepared for an overnight hike as well as living in camp Required: (The following items are the minimum suggested items for the course. Also, before coming, see that all badges and insignia are properly placed on uniform.
