3 HEALTH PROGRAMS ADMISSIONS REQUIREMENTS Below are the general admissions requirements for all health programs followed by the program specific requirements. Please keep in mind that admission to all health programs is a two-step process. Step 1: Complete your SPCC Admissions File 1. Complete the SPCC Application for Admission Submit an application for admission online at If you already have a CFNC account, use your login information to fill out SPCC s application. If you do not have a CFNC account, you will create one as the first step of the application. Keep track of your account information. You may apply to only one health program. 2. Submit official high school, High School Equivalent, or adult high school transcript It is your responsibility to request transcripts and pay any applicable fees. Official transcripts should be mailed directly from the issuing institution/agency to SPCC. Unofficial (student-issued/faxed/ ed) transcripts will not be accepted. 3. Submit official transcripts from all colleges listed on your application A Request for Transcript Evaluation Form must be completed and sent to the Registrar for evaluation of prior course transfer credit. General Education credits over 10 years old can be appealed through the Transfer Credit Appeal process. All foreign transcripts (secondary and post-secondary) must be evaluated by an authorized translation company at the student s expense. To evaluate foreign transcripts, the transcript, course descriptions, and the grade system must be translated. 4. Prepare for and take the College Placement Test (CPT) You have one opportunity to take the CPT (Reading, Math Diagnostic, and Computer Competency). This initial test is free. Students are strongly encouraged to thoroughly review test study material and use our test preparatory services. Visit the Testing Center webpage for study resources: Student Resources, Testing Center. Reading, Math, and Computer diagnostic scores are good for five years. Accuplacer Reading, Sentence Skills, Computer Competency, Arithmetic, and Algebra scores are good for five years. All students are required to take the CPT unless exempt by a waiver. High school graduates within five years of enrolling that meet certain college ready requirements (Multiple Measures). SAT scores of at least 500 on Critical Reading and Writing and 500 on Mathematics will exempt student from the CPT. ACT scores of at least 22 on Reading and 18 on English and at least 22 on Mathematics will exempt student from CPT. Completion of college-level English and math (with a grade of C or higher) from a regionally accredited institution within 10 years of enrolling. 3

4 Completion of Developmental courses in English and math (with a grade of C or higher) within three years of application packet deadline. Completion of a Bachelor s degree or higher. Appropriate scores on Advanced Placement (AP) or College Level Examination Program (CLEP) exams. SPCC must receive an official score report from ACT, the College Board, or from the college where the Diagnostic was taken. SPCC must receive official high school and/or college transcripts for any waivers related to course credit or degree completion. Students submitting a Transfer Credit Appeal will have to take the CPT even if a waiver is awarded. 5. Complete the New Student Orientation online View the online orientation video at Flash Player is required. At the end, you will be asked to answer a few questions correctly and then enter your personal information. Step 2: Medical Laboratory Assistant Admission Requirements 1. Have valid required College Placement Test scores or appropriate developmental coursework completed within three years of program application deadline. 2. Attend mandatory How to Apply to Health Program Workshop offered at various times on both campuses. To reserve your seat, contact Enrollment Services or scan code below or go to https://spccadmissions.youcanbook.me to make appointment. The workshop is valid for that academic year (July 1 to June 30). After your admission application is submitted, attend this workshop as soon as possible. 3. Obtain CPR certification or certification document no later than March 1, 2015, for Spring 2015 entry, and no later than October 1, 2015 for Fall 2015 entry, to include first aid, child, infant, and adult CPR. This course is available through SPCC s Career Development & Continuing Education Division, at Career Development & Continuing Education, Medical Professions, CPR & First Aid Certification. Submit CPR certification by appropriate deadline to the Administrative Assistant, Allied Health and Nursing. NOTE: On-line CPR courses are not accepted. You will be unable to attend clinical sites if you do not complete your CPR certification. 4. Take the TEAS V test and submit a copy of your TEAS V score with your packet. TEAS V scores are valid for two years prior to the program application deadline. Go to to make your testing appointment online. (See page 11 for Testing Center Permission Form TEAS.) TEAS scores not taken at SPCC must be officially transferred from ATI Testing and must be the same version administered at SPCC (TEAS V). 5. All course work must be at least a C ; and your program GPA must be at least a 2.5 to be considered for admission. 4

5 MEDICAL LABORATORY ASSISTANT (MLA) PROGRAM Application Period for Spring Semester 2015 Ends November 30, 2014 Application Period for Fall 2015 Ends June 30, 2015 How do I apply for admission to the Medical Laboratory Assistant program? Complete Step 1 and 2 listed on pages 3, and 4. When you have completed both steps, collect only the required documents listed below and put them into a 9 x12 envelope with your full name, SPCC ID, and program printed clearly in the upper right corner. Your packet should only include: Medical Laboratory Assistant Application pages 9 and 10. Copy of TEAS V test results. Letter of recommendation on letterhead from high school health occupations teacher showing proof of completion of the Allied Health Science OR Medical Science classes, if applicable. When are students admitted into the Medical Laboratory Assistant program? Health programs at SPCC have limited enrollment and a competitive admissions process. Students are admitted into the Medical Laboratory Assistant program in the fall and spring semesters each year. Students who submit a complete application packet by the November 30 deadline will be given priority consideration for admission for the spring 2015 semester class. Students who submit a complete application packet by the June 30 deadline will be given priority consideration for admission for the fall 2015 semester class. NOTE: If the number of qualified applicants meeting these deadlines is not sufficient to fill all program openings, complete application packets will continue to be accepted until the maximum enrollment number is met. What do I need to do to be considered for entry into the Medical Laboratory Assistant program? To be considered for admission into the Medical Laboratory Assistant program, you must complete the requirements listed on pages 3, and 4 and submit a complete admission packet by the deadline. What are the required CPT scores for the Medical Laboratory Assistant program? Combined Reading and Sentence Skills score 151 or higher Computer Skills: 80 Mastery in DMA CPT test scores are valid for five years. If you do not meet these required CPT scores, you must enroll in and complete the appropriate developmental course(s). Recent SAT, ACT, ASSET or COMPASS scores may be used in place of some of these requirements. 5

6 What is a TEAS test? The TEAS V (Test of Essential Academic Skills) measures basic essential academic skills related to success in Allied Health programs in English, reading, mathematics, and science. For more information on the TEAS test, go to TEAS study guides are available at SPCC bookstores or at for purchase. How do I take the TEAS test? Go to Student Resources, Testing Center to make your testing appointment online (see page 11 for contact information). The TEAS test may be taken once every three months, for a cost of $50 (fee subject to change) per test (payable to SPCC s Business Office on the day of the test). Complete the Testing Center Permission Form TEAS (see page 11), attach the receipt for payment of the testing fee, and bring both to the Testing Center on the day of your appointment. Be sure to bring a photo ID and ATI login and password to your testing appointment! NOTE: Calculators and outside study aids are not allowed during the test. Use of these or refusal to follow all Testing Center rules will result in test disqualification. Is there a waiting list? There is no waiting list to enter into any SPCC health program. Students are chosen for the Medical Laboratory Assistant program based on ranking of points. Points are based on (1) TEAS V score; and/or (2) completion of high school Allied Health Science I & II or Medical Science I & II with a grade of B or higher within the last twelve months. There is no minimum point s requirement to apply for this program. If you are not accepted into the program for the upcoming fall or spring session, your packet will be shredded. Please make a copy of your packet documentation prior to submission. Enrollment Services is not able to copy your packet for you. How will I know if I m accepted into the Medical Laboratory Assistant program? The Admissions Office will notify you of your provisional acceptance into the program. ONCE PROVISIONALLY ACCEPTED, you must do the following: (1) Attend the Medical Laboratory Assistant orientation session. (2) Submit the completed Student Medical Form for North Carolina Community College System Institutions. Deadline will be included in your provisional acceptance letter. NOTE: Accepted students will be required to have a physical exam and complete listing of immunizations. Cost is the responsibility of the student. You will receive a follow-up with specific instructions for completing the form within one week of receiving your provisional acceptance letter. Applicants accepted for admission into the Medical Laboratory Assistant program may be required to complete a criminal background check and a drug screen after notification of acceptance and prior to participation in the onsite clinical portion of the program. If required, cost is the responsibility of the student. Based on the results of the criminal background check and the drug screen, hospitals or clinical facilities where you will participate in onsite training may deny you access to their facility, resulting in your inability to complete the clinical portion of the Medical Laboratory Assistant program. If you are unable to complete the clinical portion of your training, you will be unable to progress in the program. 6

7 How do I calculate my GPA? Only use your courses that are required for your program. If you ve repeated a course, use the highest grade for calculations. Transfer credit is calculated using the grade earned at your previous college. Grades and credit hours can be obtained from your GO! account. Please use the blank GPA calculation chart below to determine your GPA. These courses may also be taken prior to admission into the program. Disability Services SPCC provides an ongoing program of services that encourages students with disabilities to integrate themselves into all aspects of college life. The Disability Services provider is available to assist with registration, counseling, academic advisement and arrangements for appropriate accommodation. Reasonable accommodations are established on an individual basis. Students must provide documentation of a disability from a physician or other licensed professional in order to establish eligibility for services. All transcript evaluations, transfer credit appeals, and/or grade forgiveness evaluations have been completed prior to submitting my application packet. STEP 2: Medical Laboratory Assistant Program Requirements Met CPT scores or completed appropriate developmental coursework. I understand that I must obtain my CPR certification no later than March 1, 2015 for spring 2015 semester or October 1, 2015 for fall 2015 semester and submit documentation to the Administrative Assistant, Allied Health and Nursing. How to Earn Additional Points: To earn points in addition to your TEAS score, you may provide proof of completion of the courses listed below to earn points based on the course grade or proof of the work experience. Allied Health Science Points Completion of Allied Health Science I & II with a grade of B or higher within the last 12 months. Must provide a letter from the instructor in the admission packet. (10 points total) OR Medical Science Points Completion of Medical Science I & II with a grade of B or higher within the last 12 months. Must provide a letter from the instructor in the admission packet. (10 points total) Total Points I certify that all information submitted is both complete and accurate. Double check packet prior to submission. Letters will be mailed within a month of the program application deadline notifying you of your status. 10

11 SOUTH PIEDMONT COMMUNITY COLLEGE TESTING CENTER PERMISSION FORM TEAS (Test of Essential Academic Skills) Go to Student Resources, Testing, Center to make your TEAS testing appointment online. This form, along with your SPCC Business Office receipt showing payment of the TEAS testing fee** must be presented to the Testing Specialist before you will be admitted. **$50 for current SPCC students; $70 for non- SPCC students (fee subject to change).** In order to take the TEAS, you must have a valid ATI login (username and password). Setup your account prior to your test date and bring your account information with you to your testing appointment. Details on setting up an ATI account are on the Testing Center webpage at Student Resources, Testing Center, TEAS. Before taking the TEAS test, you must agree to the following: I have met all CPT scores and/or developmental requirements for my chosen program of study. I am prepared to take the TEAS (Test of Essential Academic Skills) for entrance into my chosen program. I understand that I may take this test once every three (3) months at SPCC, and that TEAS retests will not be accepted once I have submitted my program admission packet for consideration. I understand that there is a $50 for SPCC students or $70 for non-spcc students (fee subject to change) nonrefundable fee for each time I take the TEAS test. I understand that no calculators or other study aids are allowed in the Testing Center. I understand that I must follow all Testing Center rules. If not, my TEAS score will be disqualified.
