3 INTRODUCTION Patients, their families and friends commonly require support and assistance in order to cope with the broad range of needs that arise when approaching the final months of life. Likewise, health care providers require reference information to enable them to assist patients and their families. This publication seeks to address these needs by providing a compilation of community resources, including hospices and other organizations and agencies that are especially attuned to end-of-life concerns and the commitment to improve the care of the dying. In this document, we strive to share resources that enable and empower us to counter the pain, isolation, and loneliness, and to help with other aspects of advance planning in life s final months. This directory is a beginning step in bringing together in one publication a listing of support services available in Colorado. Inclusion in the directory does not reflect endorsement of a program, or of any specific resource or practitioner over another. No doubt, many available resources have been inadvertently omitted. We encourage users of this directory to supplement and update our listings by use of the form appearing at the back of this publication. 4 COMFORT CARE Comfort care or palliative care interventions alleviate distressing physical symptoms and address psychological, social, emotional and spiritual needs associated with the disease process. The goal is a more comfortable existence without prolonging the dying process or aggressively sustaining a quality of life that would be unacceptable to the patient. Comfort care uses less invasive methods to support the patient s physical and emotional well-being, such as medication, physical therapy and relaxation techniques, counseling and spiritual guidance. The Comfort Care Resources section of this guide includes an alphabetic listing, by county, of names and telephone numbers for the following: Adult Day Programs: Health and social services, individual therapeutic and psychological activities furnished on a regularly scheduled basis in an adult day services center, as an alternative to long term nursing facility care. Bereavement Support Resources: Bereavement support services, including education, counseling, and grief support groups, are offered to families and friends following the death of an individual and to communities following traumatic events. Bereavement services can be obtained through local communities or hospice programs. Community Mental Health Clinics: Mental health programs offering assistance to determine decision making capability, suggested medications for agitation and mental health assessment and treatment planning. s: Agencies available through local county government offices that may provide direction to additional resources and services in a particular area. : A location that provides funeral directing, embalming and/or cremation services to the public. Only those funeral homes and mortuaries belonging to the Colorado Funeral Directors Association are included in this directory.. : Part-time or intermittent skilled nursing services, other therapeutic services (physical therapy, occupational therapy and speech ii
5 therapy), and home health aide services made available on a visiting basis in a place of residence used as the patient s home. Only those home health agencies that are certified for Medicare reimbursement are included in this directory. : Coordinated programs of intermittent home and inpatient care, available 24 hours a day, using an interdisciplinary team to provide palliative and supportive services to a patient-family unit experiencing a life threatening disease with a limited prognosis. Hospice addresses the medical, psychosocial, spiritual, and practical needs of the individual and the related needs of the family and loved ones throughout the periods of illness and bereavement. Hospice usually includes medications and biologicals related to the terminal illness. Although normally provided in an individual s private residence, hospice care may also be available in long term care facilities or in hospice in-patient units. Hospitals (General): A health facility that offers and provides twenty-four hours per day, seven days per week, inpatient services, emergency medical and surgical care, continuous nursing services, and necessary ancillary services to individuals for the diagnosis or treatment of injury, illness, pregnancy, or disability. Medical Society: The county medical societies that are listed are staffed and try to assist patients with questions regarding medical issues within their communities. : County supported agencies that provide a range of services for senior and disabled persons. iii
