Empowering Early Years Practitioners in Nottingham to Promote Healthy Nutrition Vicki Watson, Specialist Dietitian Nottingham CityCare Partnership
Early Years Nutrition in Nottingham 42% of children are from BME groups 39% of children are classed as living in poverty Fewer than 50% of mothers are breastfeeding at 6 weeks Childhood obesity levels are above the national average The city has a universal vitamin D programme Slide 5
Why a focus on Nutrition? Concerns around infant and toddler diets high intake of processed snack foods, low vegetable intake, poor weaning practices, lack of cooking skills, iron deficiency Workforce development needed to ensure all children s workers able to promote key nutrition messages and contribute to obesity prevention (Recommendation 1: Improving the nutrition of pregnant and breastfeeding mothers and children in low income households NICE PH Guidance 11, 2008) Slide 6
Nottingham s approach 1. Early Years workforce development - from 2009/10 specialist nutrition training made available for health visiting, children s centres workers and voluntary sector 2. Partnership working between Health and Children s Centres to facilitate delivery of groups for parents e.g. weaning groups 3. Supporting resources developed - parent friendly, interactive nutrition resources, evidence based food and nutrition guidelines Slide 7
An Initial Focus on Weaning To promote good weaning practices across the city To increase the number of babies being given homemade, nutritious weaning foods To support obesity prevention using a consistent approach by practitioners Slide 8
First Foods Education Programme Groups for new parents - delivered by trained workers, in baby friendly venues Content interactive quiz, baby food tasting, demonstration of how to manage choking, an activity around responsive feeding Supporting resources facilitator kits with lots of visual resources, evidence based information Slide 9
Feedback from health visiting A nutrition training needs analysis survey among health visiting staff (n=61) in 2012 highlighted that training and resources provided around weaning were very effective Slide 10
What do parents say? I feel more confident about weaning and trying new foods I enjoyed the class and it put several myths right Very informative, I learnt new things that I didn t know before Was nice to get some ideas from other parents Slide 11
Evaluation from 11 centres Knowledge of cultural weaning practices is essential Further work needs to be done to engage with parents who intend to use ready made baby foods Many parents wanted a follow up session when baby was 6 months and many were interested in cooking sessions Slide 12
Learning points Style of facilitating is important a relaxed, informal session works well (group skills training is essential) New staff need to be able to access training Updating of practitioners and the resource kits is required Students need practical training around baby feeding e.g. making up bottles and first foods Slide 13
What changes have we made? A new First Foods booklet for parents Thinking about the language we use Promoting vegetables and fruit as best first foods More of a focus on responsive feeding Slide 14
Evidence base Interesting research around early exposure to vegetables: The TASTE intervention study Appetite 2014 Vol 76 p210 Habeat European project ( ) Obesity prevention guidelines: Development of an evidence-based practice guideline for UK public health nurses (health visitors) to use with parents of infants at risk of obesity. Redsell S.A. et al 2013 ( Slide 15
Other Resources Oral health posters Eatwell Early Years and Cook and Move Facilitator Packs Packed lunch leaflet Sugar, salt and fat kits Eatwell and multicultural foods kits Healthy weight training Slide 16
Supporting activities Practical nutrition sessions and cook and eat courses Children s Centre groups e.g. Cook and Move Infant Feeding Forum dissemination of new evidence, sharing of good practice Nutrition guidelines and resource directory Slide 17
The Challenges Identifying the most effective way to engage and influence parents who are most likely to bottle feed, use manufactured baby foods, have poor cooking skills etc (informal sources of advice are most influential in younger mothers and those of lower educational attainment) Working alongside financial constraints and re-structuring Quality assurance Slide 18
Early Years High Impact Area 4 Healthy weight, Healthy nutrition We need to ensure our health visitors are equipped to translate the evidence base into practical real-life advice for families High quality interactive nutrition resources can help to facilitate change We must evaluate and adapt practice if necessary Slide 19
In Summary Access to specialist nutrition training empowers early years practitioners Motivational interviewing training underpins effective practice Partnership working makes most effective use of resources Support with cooking skills and how to adapt meals for baby/young children increases parental confidence and improves eating habits Slide 20
