4 Introduction and welcome Welcome to the practice-based component of your qualification. This is your Portfolio, which is an integral part of your learning process. It is not simply a catalogue of learning activities; rather, the Portfolio should provide evidence of learning from academic activities as well as from practice experience or in the workplace. It is particularly important for nursing students to demonstrate achievement of the Nursing and Midwifery Council (NMC) Standards for Pre-Registration Nursing Education (NMC, 2010). You are responsible for the safekeeping of your Portfolio. You must at all times maintain the confidentiality of service users, carers, clients and colleagues, and comply with the current Data Protection legislation. (More information about this can be found on the United Kingdom government s Data Protection website.) It is important that you ensure your Portfolio is kept secure at all times, whether at home, in practice, in tutorials, or in any other place. You must observe the standard of confidentiality set by you practice learning organisation(s) and any code of conduct associated with your field of practice. Your Portfolio should not be shared with anyone who is not connected with the module or the qualification you are following. You must ensure that your Portfolio is made available to your mentor and practice tutor when requested. The material in your Portfolio, once submitted to the University, may be shared with relevant personnel associated with quality assurance, examination board processes and relevant regulatory/professional bodies as appropriate. This information will be kept for the period of time prescribed by The Open University (OU) for purposes. You must sign the declaration below indicating that you agree to share this information. 6 Completing your Portfolio Guidance on how to complete your Portfolio successfully is provided throughout, either before or at the beginning of each new form. It is important that you become familiar with the layout of the Portfolio and ensure you understand the requirements for its completion. Please read the instructions about how to complete each section of the part relating to your qualification very carefully so the records are completed accurately and fully. Completing your Portfolio legibly, accurately and fully will help you to enhance your skills in effective record keeping, an essential skill to develop in healthcare and nursing practice. If you need a fresh copy of any of the pages please download them from your module website. This Portfolio is used by students taking the module KYN237 Principles and skills for nursing practice as part of the Pre-Registration Nursing Degree. Students taking this module as part of the degree programme will have a practice-based mentor. Submitting your Portfolio on completion of the module You must submit only the pages identified as ESSENTIAL INFORMATION to the Open University on completion of the module. Supernumerary practice for nursing students During practice learning nursing students are required by the Nursing and Midwifery Council to be recognised as supernumerary. You learn in a supernumerary capacity under the supervision of a named practitioner (your mentor) and with the support of your employer and practice tutor. Supernumerary status is defined as you: not working as a paid member of staff or care team and not rostered as part of the workforce having a legal right to practise in the designated setting as a student rather than as an employee being overseen in your practice learning by your practice tutor (PT) who has responsibility for facilitating access to practice learning opportunities to enable you to fulfil the NMC s experience requirements and achieve its specified competencies receiving supervision and support from a mentor who is employed by your employing organisation and whose name appears on the live register of mentors. 8 Signature verification record This signature verification record should be completed by your practice tutor, mentor and all other qualified practitioners who observe and assess your practice. For a registered nurse to act as your mentor they must meet the following criteria: be qualified in the same field of practice as the one in which you intend to practise, i.e. either adult or mental health nursing have completed a minimum of one year s post-qualifying experience in the same field of practice as the one in which you intend to practise have completed mentor preparation as specified by the NMC s standards to support learning and in practice (NMC, 2008) comply with NMC standards related to continuing to act as a mentor have completed an induction to the OU BSc (Hons) Pre-registration Nursing Programme. Mentors must commit to: facilitating your practice-based learning providing supervision commensurate with your competence and confidence liaising with colleagues to access feedback on your practice performance liaising with your practice tutor as required through agreed methods of communication participating in the three review meetings with you completing practice records being accountable for the of your practice performance alerting your practice tutor to any concerns about your capacity/ability to meet the standards of conduct and health required of a student of nursing. A registered nurse who does not meet the above criteria cannot take on responsibilities for you. It is therefore important that prior to summative you check that all the above criteria are met and if not, you must discuss the situation with your PT so alternative arrangements can be made. Mentors who summatively assess your practice must be registered on the same part at the same level of the register as you are seeking to enter. Nurses registered at level 2 can contribute to supporting your learning and providing supervision; however, they cannot undertake summative. KYN237 Portfolio 8

NUTRITION AND FLUID MANAGEMENT 4.1 Assists people to choose a diet that provides adequate nutritional and fluid intake. Skills i. Under supervision helps people to choose healthy food and fluid in keeping with their personal preferences and cultural needs. ii. Accurately monitors dietary and fluid intake and completes relevant documentation. iii. Supports people who need to adhere to specific dietary and fluid regimens and informs them of the reasons. iv. Maintains independence and dignity wherever possible and provides assistance as required. v. Adheres to an agreed plan of care that provides for individual difference, for example cultural considerations, psychosocial aspects, and provides adequate nutrition and hydration when eating or swallowing is difficult. MEDICINES MANAGEMENT (continued) 5.3 Works as part of a team to offer holistic care and a range of treatment options of which medicines may form a part. Skills i. Demonstrates awareness of a range of commonly recognised approaches to managing symptoms, for example relaxation, distraction and lifestyle advice. ii. Discusses referral options. 5.4 Ensures safe and effective practice in medicines management through comprehensive knowledge of medicines, their actions, risks and benefits. Skills i. Uses knowledge of commonly administered medicines in order to act promptly in cases where side effects and adverse reactions occur. 5.5 Safely orders, receives, stores and disposes of medicines (including controlled drugs) in any setting. Utilises and safely disposes of equipment needed to draw up and administer medication, for example needles, syringes, gloves. iii. Administers and, where necessary, prepares medication safely under direct supervision, including orally and by injection. 5.7 Keeps and maintains accurate records using information technology where appropriate, within a multidisciplinary framework as a leader and as part of a team and in a variety of care settings. Demonstrates awareness of roles and responsibilities within the multidisciplinary team for medicines management, including how and in what ways information is shared within a variety of settings including at home. 59 Gaining service user and carer consent for your involvement in their care Service users have a fundamental legal and ethical right to determine what happens to their own bodies. Therefore, valid consent to any intervention is absolutely essential in all forms of healthcare. The Department of Health has issued a range of guidance documents on consent for examination and treatment, which can be accessed on its website. OU healthcare students are required to observe the following protocol to ensure that service users have freely agreed to have care delivered by a student under supervision. Service users may withdraw their consent at any time. If a person indicates that they do not want a student to be involved in their care, you must respect their views and inform your mentor. Protocol for gaining service users consent As a student, you must always ensure that service users and their carers have agreed to your involvement in their care and, in addition, know who is responsible for supervising your practice. You must follow this protocol when you first meet a service user or their carer: 1. Introduce yourself as a student and state who is supervising your practice. 2. Ask the service user or carer/advocate whether they agree to receive care from you as a student under supervision. 3. If the service user or carer asks questions, or seeks more information about the care you want to offer, explain this care and discuss it with them. 4. Ask the service user or carer whether they would like to speak to your mentor about the care that you want to offer. If they would like to discuss any aspect of this care with your mentor, tell them you will ask your mentor to speak to them before you offer any care. 5. If the service user or carer does not offer complete and freely given agreement and/or seems confused or worried about your role, refer this to your supervisor before taking any further action. 6. If the service user or carer offers complete and freely given agreement, you may progress to offering the care agreed. 7. If the service user or carer would prefer you not to contribute to their care, their preference must be respected. 76 Completing the record of student hours As a student and employee you have an obligation to account for your time. Over the course of the module you must complete 855 hours (114 days) of practice learning, of which 342 hours (46 days) must be completed with the supervision of your mentor. On commencement of the module, supervision from your mentor will be direct; however, by completion of the module you should be practising under the indirect supervision of your mentor. You need a copy of the Record of student hours form for each month of the module. You can either photocopy the form or print copies from the Portfolio forms document on your module website. Every month this form should be completed and signed for each day you are in the workplace. At the end of each day, complete the form and ensure a staff member signs it to confirm those hours. Completion must not be retrospective. Once completed you need to take a copy of each form to send to your employer and practice tutor (see comments below on submitting the form to your practice tutor). Remember to keep the original copy for your own records. Actual hours to document At the end of every day when you are in your workplace, enter the start and end times of the shift and number of hours completed, for example You were present for 8 hours, but 30 minutes were your break time, so you completed 7.5 hours. Sign the entry. If your mentor has been available to you during the day then ensure your mentor signs the entry. This enables your practice tutor to see that you have had access to your mentor. If your mentor has not been available to you, another registered practitioner/manager must sign the entry. Across the module you need to add together the monthly totals of practice hours to give you the total number of practice hours to be entered on the Record of student hours form. Annual leave/sickness/absence If you are on leave, enter A/L against the relevant date(s). If you are on sick leave, enter sick against the relevant date(s). If you are absent, enter absent against the relevant date(s). Monthly totals At the end of the month add up the hours spent in practice, on leave, sick/absent and enter the totals into the monthly totals boxes on the final page of the form. Submitting your forms to your PT Once your forms are completed at the end of each month, either scan and them to your practice tutor or photocopy and post them. A copy is also required by your employer. You must do this within 5 days of the month end. Your mentor will report to your practice tutor any concerns she or he has about your attendance or practice experiences achieved and performance. 81 Collecting Portfolio evidence and using the evidence index The following table provides guidance on the variety of evidence you can gather and use to demonstrate you achievement of skills and competencies or standards. Using the Evidence Index, list the types of evidence from the table you have collected to demonstrate your achievement of the skills and competencies or standards. Please ensure that you mark each piece of evidence with a number, so that it is easy to find, and give a description so that the nature of the evidence is clear. You should clearly indicate which competency or standard each piece of evidence supports as a piece of evidence may well be able to demonstrate achievement of several skills and competencies or standards. It is important that a range of evidence is provided to inform you mentor s decisions; however, it should be remembered that your mentor will be seeking quality evidence rather than large quantities. File each piece of evidence behind the Evidence index form in the order in which it is listed. This will help you to locate the relevant supporting evidence you have selected to demonstrate your progress to your mentor in preparation for each review meeting. 82 Service user involvement Your mentor must seek the views of at least two service users to inform their of your performance and achievements using the feedback proforma in Appendix 1. Please note that in order to protect the confidentiality of service users, they have the right to ask that you do not see their comments, but if they choose to share it with you, it may be used as evidence in your Portfolio. Appendix 1 contains mentor guidance for gaining service user/carer or relative feedback and the proforma to provide feedback on student performance. Confidentiality Your Portfolio evidence can draw on both personal and practice experiences. This may mean including information about, and passing judgement on, the organisation you work for as well as your colleagues, service users and/or carers. When using such material in your Portfolio you should make sure that all information is made anonymous, for example by changing or removing the names of people or places. If you refer to a person by name then you should change their name and explicitly acknowledge that you have changed it. It is good practice to cite the requirements of any professional code of conduct associated with your field of practice. You should also check that you have the permission of the person(s) concerned to use any material. Make sure you read through your supporting evidence and remove every mention of settings and people s details. If you want to use your employer s headed paper for any items of supporting evidence, you must gain your employer s permission and ensure that their policy on the use of headed paper is observed. Using such paper in your evidence does not in itself compromise service user confidentiality provided that the anonymity of the person and the setting is preserved. KYN237 Portfolio 82

When used as supporting evidence in your Portfolio, the activities provide potentially strong evidence and can reduce your study workload because this is work you have already completed. The unit activities often relate to skills development and, in many instances, include learning the theory underpinning the skill and then using the skill in practice. Tutor-marked assignments TMA If you use TMA content as the basis of Portfolio evidence you will need to write a short account to indicate how the TMA supports achievement of the skills or fieldspecific competencies and to highlight the relevant text within the TMA. Interactive computer-marked icma As part of the of your learning you will complete online icma questions. These can provide useful evidence of your knowledge and understanding which underpins your practice of the associated skills. Certificated learning C Participation in a certificated workshop or training, supported by a summary account of what you learned from it. A certificate alone does not constitute evidence; it should be supported by an account of what you have learned and how it will improve your practice. The certificate should be placed behind the evidence that refers to it. If you refer to it in more than one piece of evidence, you should cross-reference it. Summary accounts SA You could write a summary account following your participation in a forum discussion, about an episode of care you have observed or participated in (with supervision), your understanding of a journal article, or your study of another module. If feedback is offered verbally you should write it up in a way that the person who gave you the feedback can read and understand it. For example, if a service user who is visually impaired offers you feedback and you wish to use it as evidence in your Portfolio you may wish to audio record your summary of what was shared with you. If a young child offers you feedback you may wish to use a smiley face satisfaction scale. For some skills and field-specific competencies it is essential you seek feedback from service users and/or carers. A pro forma is provided in Appendix 3 for your mentor to obtain feedback from service users and carers. Please ask your mentor to seek feedback on your performance from at least two service users or carers. Learning in relation to an audio or video clip AV You may have chosen to learn more about, or seek different views on a topic by accessing websites, for example Patient Voices, watching a video or listening to an audio recording. Accessing such resources can add depth and breadth to your knowledge, understanding and practice. Presentations P You may present at a conference, during a seminar in your practice setting or at a meeting. You will need to provide verified evidence of your involvement. Project work PW You may decide that you want to follow up aspects of the module, perhaps through website and evidence searches. If you do this, discuss your work and its relevance to KYN237 practice with a qualified practitioner, and ask them to verify it. 86 Appendix 1 Service user/carer/relative feedback on student performance Introduction With the person s freely given and fully informed consent, your mentor is required to seek feedback from a minimum of two service users, carers or relatives about your performance whilst contributing to their care and/or support. This feedback will contribute to the evidence your mentor considers to inform their review of your performance and their decisions. Please ensure your mentor is reminded of this requirement in good time, prior to your final meeting. Information to mentors A key source of information about a student s performance in practice is service users, carers and relatives. To this end, to inform your of the student s performance, a requirement of the module is that mentors seek feedback from a minimum of two service users or carers or relatives. To assist you a statement is provided below that you can use to explain to people why you are seeking their feedback: I would like you to share with me your views about the way student [insert name] has contributed to the care you have received. This is to help [insert name] to learn and develop his/her skills in providing care in a compassionate way and communicating effectively with people using our services. The feedback you give will not adversely affect the care you receive. I will record your feedback anonymously on this form and will not include any information that could breach your confidentiality. If you would prefer not to give feedback this is perfectly acceptable. After the service user, carer or relative has completed the form, please provide an opportunity for them to read it. 88 Appendix 2 Raising and escalating concerns Raising and escalating concerns: a process for Open University students studying the Foundation Degree in Healthcare Practice, the Diploma in Higher Education in Healthcare Practice or individual work-based modules within these qualifications. Any student concerned about the safety or well-being of service users or carers or the safety of a practice environment must speak to their supervisor or the registered practitioner in charge/senior carer on duty and outline their concerns without delay. Students must also inform their line manager and follow their employer s procedures and policies for Whistleblowing. Students must also report their concerns to the OU Practice Tutor, again without delay, who will provide advice and support. The Practice Tutor should refer the issue to the OU Staff Tutor/Regional Education Manager. Raising and escalating concerns: a process for Open University Pre-registration nursing students Nursing students must follow the protocol outlined above.
