Parents: Please read specific information regarding: The school and its programs (See School Information) Health and physical exam requirements (See School Information) Financial obligation (See Fee Sheet) Extended Care (See S.P.E.C.K Information) Application for Enrollment Process: Print out or obtain from the school office an enrollment form for each student. Preschool and Kindergarten families also need to complete the appropriate Initial Information sheet for each student. Schedule an interview with the principal (new students only). If desired, visit a classroom or tour the school (by appointment, please). Bring completed enrollment form, non-refundable deposit, and birth certificate (birth certificates needed for new students only) to the school office. Enroll at the proper time (see schedule below). Enrollment Schedule: 1. Member families applying for enrollment and returning students applying for enrollment in grades pre-kindergarten through eight may enroll beginning Tuesday, January 3, All families are encouraged to enroll no later than March 1, The first period for accepting Application for Enrollment is reserved for member families applying for admission to pre-kindergarten and kindergarten and returning students applying for admission to grades preschool through eight. (Returning students are those students who are enrolled in classes during the current school year.) The date and time are noted on each application form as it is received in the office. Member families applying for admission to pre-kindergarten and kindergarten or returning students applying for admission to grades preschool through eight who do not apply during this period may apply at any other time, but will be given no preference over other families if classes are filled. 2. Open Enrollment Tuesday, January 31, 2012 Open Enrollment begins January 31, 2012 and Applications for Enrollment are accepted until classes are full. A Waiting List is maintained for filled classes. The date and time are noted on each application form as it is received in the office.

2 Waiting List: The Principal reviews Applications for Enrollment with the Parish School Ministry after March 1 in order to notify by March 15 those families whose Application for Enrollment has been accepted and those who have been placed on a Waiting List. When an opening occurs, students are moved from the Waiting List to enrollment status based on the date and time noted on the Application for Enrollment. Saint Paul Lutheran School is supported and maintained by Saint Paul Lutheran Church, a member of the Lutheran Church- Missouri Synod. In addition to formal religious instruction in every classroom, the Lutheran Christian faith and values are taught and modeled throughout all grade levels and curricular areas. One of the most important strengths of Saint Paul lies in its Christian teachers and a loving, family-friendly environment. EARLY CHILDHOOD (Pre-Kindergarten through Grade 2) offers Dedicated, highly qualified teachers, many with advanced degrees. An outstanding program in which children s learning styles are respected and nurtured. Age-appropriate developmental activities which gradually become more academic through the levels. A language-rich environment coupled with the basics of phonics, literature, mathematics, science, social studies, music, art, physical education, computers, and cooperative community building. Participation in regularly scheduled performance productions, in church and on the school stage. MIDDLE ELEMENTARY (Grades 3-5) offers Excellent, highly-trained teachers, some with Master s degrees, teaching children in a self-contained classroom. setting. Increased student access to the school library/media center and computer lab to enhance academic study. Opportunities for participation in co-curricular and extra-curricular activities including Outdoor Education, band, athletics, Lego League, Scouts, and other clubs, as appropriate to the age. JUNIOR HIGH (Grades 6-8) offers Outstanding teachers, many with advanced degrees and who are specialists in their fields, working with students in a departmentalized setting. An assigned homeroom teacher who monitors individual student progress through the course of the departmentalized curriculum. A progressive program which includes instruction in Spanish, Computer Education, Pre-algebra and Algebra studies, art in several media, instrumental and vocal music and music history study, in addition to the basic core of lab-based science, English and literature, World and U.S. history, mathematics, and P.E. Increased opportunities for participation in co- and extra-curricular activities, including Academic Fair, Geography and Spelling Bees, sports, cheerleading, and Lego League. Opportunities for leadership in school-wide programs: Student Council, Chapel assistants, Reading Buddies, tutoring younger children, and more. IN ALL GRADE LEVELS, our Resource Teacher serves as a consultant to classroom teachers, enabling them to better instruct students with special learning needs. Acting as liaison to area public schools, she assists parents who wish to access services through their district. Further, she plans enrichment activities for large and small groups. SAINT PAUL EXTENDED CARE FOR KIDS provides a warm, caring environment for our students before and after school. This program is available to students in Pre-K (on days of school attendance) through Grade 8.

4 SAINT PAUL PARENT TEACHER LEAGUE provides parents with opportunities for fellowship and schedules volunteers for many fundraising programs which support the school. Please ask the school office to learn how you can become involved! SAINT PAUL PARISH SCHOOL MINISTRY is the governing body of the school and holds monthly meetings which are open to parents. HEALTH INFORMATION: PHYSICALS & EMERGENCY FORMS The State of Illinois requires PHYSICALS upon entering Pre-Kindergarten, Kindergarten and Sixth grade. Saint Paul requires annual PHYSICALS for all students new to the school and for those participating in extra-curricular athletics. In addition, ALL students must have a school-supplied Emergency Form on file in the office. The State of Illinois requires DENTAL Examinations for children in kindergarten, second, and sixth grades. The State of Illinois requires VISION Examinations for children in kindergarten and those new to the school. STUDENTS WILL NOT BE ALLOWED TO ATTEND CLASSES OR PARTICIPATE IN PRACTICES OR GAMES UNTIL PHYSICALS AND EMERGENCY FORMS ARE ON FILE IN THE SCHOOL OFFICE. If the immunization record, listed on the physical form, is found not to be in compliance with state guidelines, students will be given two weeks in which to comply with the regulations before being excluded from school. Information about these guidelines and Cook County Department of Public Health assistance are available in the school office. LEAD POISONING PREVENTION ACT: This legislation requires that parents of children between the ages of six months through six years provide a statement from a physician or health care provider that the child has been screened for lead poisoning. This statement is to be provided prior to admission and subsequently in conjunction with physical examinations. The Illinois Department of Public Health has revised the Certificate of Child Health Examination (School physical form) listing lead screening as a physical examination requirement. However, neither the School Code nor the Lead Poisoning Prevention Act authorize the exclusion of children who have not been screened from educational programs in which the child is otherwise eligible to participate. STUDENT ACCIDENT INSURANCE: Parents are required to provide documentation of accident coverage before registering the student in August, listing the name and policy number of their insurance company on the school emergency form. Students who have no accident insurance coverage through a parent are required to purchase student insurance before attending classes or participating in extra-curricular athletics at Saint Paul. ADMISSION POLICY: Saint Paul Lutheran School admits students of any race, color, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, national and ethnic origin in administration of its educational policies, admissions policies, scholarship and loan programs, and athletic or other school-administered programs. Saint Paul Lutheran School gives preference in applying for admission to pre-kindergarten and kindergarten classes to members of Saint Paul Lutheran Church. Returning students (students enrolled in classes during the current school year) are given preference in applying for admission. Students must be three years of age by November 1 of the current school year in order to enter Pre- Kindergarten classes; five years of age by September 1 in order to enter Kindergarten; and six years of age in order to enter first grade. All students enrolling at Saint Paul Lutheran School for the first time (or re-enrolling after a lapse in attendance) are required to submit a birth certificate. Parents of transfer students will be required to fill out a Request for a Transfer of Records form which will enable Saint Paul to receive an official transcript from the student s previous school. Saint Paul will request special education and disciplinary records in addition to the usual health, academic and attendance records. Photos of any student enrolled at Saint Paul School may, from time to time, be used on the church s web page or included in the school s correspondence. No names or personal information are ever used in these publications. Children with special needs which cannot be addressed by programs offered at Saint Paul or Release Time programs may be denied enrollment or enrollment may be discontinued in order to encourage them to enroll at a school that can provide an appropriate program. 5 S.P.E.C.K. Saint Paul Extended Care for Kids Early Childhood: Preschool & Kindergarten S.P.E.C.K. is an extension of Saint Paul Lutheran School. All of the philosophies and beliefs of the school are also those of the Extended Care Program. S.P.E.C.K S GOAL: To provide a family sense of unity/love & forgiveness. PURPOSE: Saint Paul Lutheran Church views children as precious resources and gifts from God and as such, acknowledges its responsibility, along with the parents, for their care and nurture. Therefore, Saint Paul Lutheran School carries out its mission and ministry by establishing and maintaining an enrichment program of extended school care to provide a safe, secure and caring home-like atmosphere for children. This program is for, but not limited to, children whose parents, due to work-related responsibilities, find it difficult to provide adequate care for their child(ren) before or after school. S.P.E.C.K. is available to all students enrolled in Saint Paul Lutheran School, kindergarten through eighth grade. It is also available to pre-kindergarten students on the days that they are enrolled in school S.P.E.C.K. will be available on scheduled in-service / conference days if a substantial need is shown and adequate personnel is available to staff the facility. Hours for Full-Day Students: 7 a.m. 6 p.m. Before School Hours for Morning Half-Day Students: 7 a.m. 8:30 a.m. for Kindergarten students 7 a.m. - 8:45 a.m. for Preschool students Extended Care Hours for Afternoon Half-Day Preschoolers: 3 6 p.m. Fees: Before School hours, and hours after 3 p.m. are billed monthly at an hourly rate. Families are charged only for the time used. S.P.E.C.K. accepts children enrolled in the IDHS Subsidy Program.
