1 Position Profile Medical Director for Population Health and Clinical Integration Iowa City, IA April, 2014 This Position Specification is intended to provide information about the University of Iowa Health Care and the position of Medical Director for Population Health and Clinical Integration. It is designed to assist qualified individuals in assessing their interest.

2 The Opportunity The University of Iowa Health Care invites inquiries, nominations and expressions of interest for the role of Medical Director for Population Health and Clinical Integration. The Medical Director for Population Health and Clinical Integration will have responsibility to help develop and implement the University of Iowa Health Care clinical programs for population health within University of Iowa Health Care and function as the clinical liaison and coarchitect with University of Iowa Health Alliance partners for clinical integration. This is a full time position and the successful candidate will have a primary faculty appointment in one of the clinical departments in Carver College of Medicine and the recruitment will be conducted cooperatively with them. The physician recruited to this role will dedicate 50 percent effort to the role of Medical Director and 50 percent effort dedicated to maintaining an active medical practice, providing patient care in the appropriate clinical setting, and/or conducting research. The Medical Director will report equally to the Associate Vice President, University of Iowa Health Alliance and the Associate Dean of Clinical Affairs for UI Carver College of Medicine and will work closely with the Assistant Vice President for Strategic Planning. Qualified candidates must be a senior level physician with 5-7 years of leadership experience; primary care specialty with experience in population health management; experience in implementing medical home model and EMR implementation and optimization; demonstrated success working collaboratively with physicians, administrators, nurses and other providers, to include experience with interdisciplinary care teams; excellent interpersonal skills and the demonstrated ability to work effectively with diverse personalities; must be approachable, show respect for others, and be able to present data with effective communication and presentation skills; must be an effective consensus builder; experience in academic medicine preferred. 3 UI Health Alliance UI Health Alliance (UIHA), comprised of four of the state s premier health care organizations, was formed in the summer of 2012 and includes over 50 hospitals and more than 160 clinics within the state of Iowa. The members of the Alliance work together to advance the high quality of health care services and improve the health status of patients and communities. The four entities involved include Mercy Health Network (MHN), Genesis Health System, Mercy Cedar Rapids (MercyCare Service Corporation), and University of Iowa Health Care. UIHA created a platform for sharing expertise, selected support services, and information technologies needed to succeed in the emerging accountable care systems and payment programs, thereby reducing costs. Working together in UIHA, members strive to increase the value of services provided, improve clinical integration between the members, provide more streamlined and coordinated care to patients, and ultimately improve the health of people in Iowa and other regions served. The structure is designed to sustain and honor the members local missions and governance authorities, while participating in a larger effort to lead needed changes in the health care system. The Alliance also is designed to encourage additional health care providers in Iowa and contiguous states to join and participate in these important initiatives. Summary of Position University of Iowa Health Care seeks a highly collaborative physician leader with significant experience and an outstanding record of accomplishment to serve as the Medical Director for Population Health and Clinical Integration. The Medical Director for Population Health and Clinical Integration will have responsibility to help develop and implement the University of Iowa Health Care clinical programs for population health within University of Iowa Health Care and function as the clinical liaison and coarchitect with University of Iowa Health Alliance partners for clinical integration. The Medical Director will help lead the transition from a fee-for-service reimbursement environment to an environment that rewards exceptional clinical care, service and cost effectiveness. The Medical Director will focus on models of care that improve population health and enhance reimbursement through value-based purchasing and shared risk opportunities. The Medical Director will also assist with assessing, developing, and implementing quality data systems for population health management. Expectations for Leadership Outlined below, in no particular order, are some of the major areas that will require clear leadership, vision and action by the new Medical Director: Establish credibility with community physicians The Medical Director will travel the state meeting with community physicians serving as a spokesperson for the UIHA and educating physicians on making the transition from episodic interactions with individual patients to population health management. S/he will champion 2

4 strategies for providing more coordinated experiences that will produce improved outcomes as well as lower costs. Achieve clinical integration with primary and specialty care To improve patient care and clinical outcomes, the medical director will introduce mechanisms for primary care physicians, specialists and health systems in UIHA to work together efficiently. By establishing a culture that emphasizes complementary delivery of care and using clear communication, the Medical Director can effectively establish an environment that coordinates care in the best interest of the patient. Produce care guidelines and achieve measurable quality The medical director will develop policies to establish standards of care using published care guidelines or evidence-based medical studies. These guidelines will help alliance members implement clinical best practices for their patients and achieve genuine, measurable quality improvement. Principal Duties and Responsibilities The Medical Director will have the following principal duties and responsibilities: Patient Centered Medical Care: Lead the process to achieve NCQA Medical Home designation for UI Health Care in all primary care locations. Population Health Management: Lead physicians through the change process from providing care for individuals to providing care for populations, to include assisting with the development and achievement of population health and clinical integration metrics. Help develop improved coordination of care delivery across the UI Health Care patient population in all settings to improve clinical and financial outcomes. Patient Engagement: Will lead initiatives around direct patient engagement. University of Iowa Health Alliance: Will work at the direction of the Associate Vice- President for UI Health Alliance and with the Associate Dean of Clinical Affairs for the UI Carver College and the Assistant Vice-President for Strategic Planning to develop and implement clinically integrated population health management within UI Health Care and with UI Health Alliance partners. The Medical Director will be the University of Iowa Physician lead for clinical integration activities with other Alliance medical directors regarding population health. The following are specific core responsibilities and duties: Accountable Care: Provide physician leadership for the ongoing development, implementation and medical management of the UI Health Care ACOs (Medicare, Medicaid, Commercial). Help assess health care cost and quality data; develops and implements specific recommendations to control costs and enhance quality. 3

5 Provide physician leadership for engaging and leading new ACO participants, such as community primary care physicians. Risk Sharing: Work with care management, strategic planning, finance, and operations leaders to develop appropriate risk sharing and physician compensation models for population health. Data Systems: Assist in assessing, developing, and implementing quality data systems for population health management, engaging physicians through data reporting, scorecards/dashboards and metrics. Network Development: Work with physician leaders in the Carver College of Medicine, Strategic Planning, and the Medical Director of Off-site Medical Clinics to implement strategies related to the development of primary and specialty care networks. Physician Relations: Work with Physician Relations to engage independent, community-based primary care physician groups, hospitals and their affiliated medical practices, including critical access hospitals on issues related to medical home, population health, clinical guidelines and metrics, clinical care follow up and integrated care management. Qualifications and Personal Characteristics Education & Certifications An M.D. or equivalent medical degree and able to obtain licensure in Iowa. Experience & Qualifications Senior level physician with 5-7 years of leadership experience. Primary care specialty with experience in population health management. Experience in implementing medical home model and EMR implementation and optimization. Experience in academic medicine preferred. Reporting Relationships The Medical Director for Population Health and Clinical Integration will report equally to the Associate Vice President, University of Iowa Health Alliance (Ronald Weigel, MD, PhD) and the Associate Dean of Clinical Affairs for UI Carver College of Medicine (Doug Van Daele, MD) and will work closely with the Assistant Vice President for Strategic Planning (Christine Miller). These include: information technology, finance, human resources, marketing and communications, and strategic planning and business development. A unique aspect of UIHC is its integration with CCOM. UIHC/CCOM has one overarching structure, mission, and strategy within University of Iowa Health Care under the leadership of the Vice President for Medical Affairs (VPMA), Jean Robillard, MD. His direct reports include the following: Assoc VP & UIHC CEO (Ken Kates), CCOM Dean (Debra Schwinn, MD), CNO (Ann Williamson, PhD, RN), CMO (Theresa Brennan, MD), Assoc VP for UI HealthAlliance (Ronald Weigel, MD, PhD), and the corporate services group including Assoc VP Finance & CFO UIHC (Ken Fisher), Assoc VP for Legal Affairs (Bill Hesson, JD), Assoc VP Human Resources (HR; Jana Wessels, JD), Assoc VP for Information Technology Services (Lee Carmen), Asst VP for Compliance & Privacy Officer (Debbie Thoman), Asst VP for Integrated Strategic Planning & Business Development (Christine Miller), and Assoc VP for Marketing & Communications (Ellen Barron). All key operating functions have been integrated across both the hospital and college. Therefore there is one office of finance (including revenue cycle for both UIP and UIHC), one main HR group, one office of compliance, one overarching IT group, common legal expertise, and integrated strategic planning. This structure provides efficiency, lowers cost, and allows financial support for key missions. Oversight, planning, and strategic decisions are accomplished by a small group of senior leaders called the Cabinet, that includes the following individuals: VPMA, UIHC CEO, CCOM Dean, Assoc VP Legal Affairs, Assoc VP Finance & UIHC CFO, CMO, CNO, and two rotating CCOM Department Chairs (currently internal medicine and neurosurgery). This integrated structure facilitates a common goal of supporting a high quality clinical practice including a nationally ranked hospital and a strong, externally-funded academic mission. Recent successes include 3rd time designation as a magnet institution for nursing, NIH funding #28 in the U.S. (remarkable for a faculty of only 1000 versus 3000 faculty in top 10 institutions), top 10 public academic medical center, and UIHC is repeatedly rated the #1 hospital in Iowa as well as highly ranked in the region and country. 6

Carver College of Medicine The University of Iowa Roy J. and Lucille A. Carver College of Medicine is a nationally ranked medical school with exceptional balance across its three missions of research, education, and clinical care. It was founded in 1870 and was the first public medical school in the country to admit women on an equal basis with men. The college s annual budget is more than $600 million, and it employs over 1,000 faculty and more than 1,700 staff. The faculty teaches 640 medical students, 972 resident and fellow physicians, 200 associated medical science students, and 5,000 undergraduate students. 7

UI Carver College of Medicine faculty is also some of the most productive when measured by NIH dollars. Researchers in the College's over 280 laboratories are served by state-of-the-art core research facilities and research support facilities. Much of the research conducted is interdisciplinary and collaborations are fostered and coordinated through major research centers, institutes and programs. Many faculty members are internationally renowned for their research. Four faculty members are Howard Hughes Institute Investigators, 17 are members of the Institute of Medicine and three are members of the National Academy of Sciences. Wellstone Muscular Dystrophy Cooperative Research Center, one of six such NIH-funded centers in the nation; The Carver Family Center for Macular Degeneration; and The Iowa Institute for Biomedical Imaging The buildings housing the Carver College of Medicine are the centerpiece of a recently redesigned health sciences campus that includes pedestrian-friendly commons and a functional and stimulating environment for students, faculty, and staff. University of Iowa Hospitals and Clinics As part of UI Health Care, University of Iowa Hospitals and Clinics offers care and hope to patients, discovers new ways to treat disease, and trains tomorrow s health professionals These missions are carried out in collaboration with the UI Roy J. and Lucille A. Carver College of Medicine and UI Physicians. As a leading teaching hospital with a full range of specialty care, UI Hospitals and Clinics serves as a regional referral center, attracting patients from across the state and the U.S., as well as many other nations. The comprehensive academic medical center houses the Hawkeye State s only National Cancer Institute-designated comprehensive cancer center, its only Burn Treatment Center, its highest rated neonatal intensive care unit, the most comprehensive transplantation program in Iowa, and its only Level I adult and pediatric trauma service. In the 2011 fiscal year, UI Health Care employed 1,432 physicians, residents, and fellows, and 7,482 non-physician employees, including 1,806 professional nurses. The organization is assisted each year by a dedicated corps of more than 1,300 volunteers. 9

11 UI Health Care is a 711-bed hospital, including the 190-bed UI Children's Hospital. In total, UI Health Care admitted more than 32,000 patients during the 2012 fiscal year for in-patient, hospital care and had more than 59,000 emergency department visits. UI Health Care also represents more than 200 outpatient clinics and care areas, receiving 977,337 clinic-patient visits in FY2012. UI Hospitals and Clinics and UI Children's Hospital together deliver quality care in collaboration with University of Iowa Physicians, the state's largest multi-specialty medical and surgical group practice composed of faculty physicians of the UI Roy J. and Lucille A. Carver College of Medicine. These partners work together as University of Iowa Health Care, to provide patient care, conduct biomedical research, and train the next generation of health professionals. UI Physicians (UIP) University of Iowa Physicians is the largest multi-specialty medical and surgical group practice in Iowa and is operationally and financially part of the Roy J. & Lucille A. Carver College of Medicine (CCOM). More than 700 physicians in 19 clinical departments provide specialized comprehensive care for patients, and serve as the medical staff of University of Iowa Hospitals and Clinics. This multidisciplinary specialty and subspecialty group, comprised of faculty physicians with the UI Roy J. and Lucille A. Carver College of Medicine, provides a depth and breadth of services unmatched in Iowa. Created in 1949 and reorganized in 2006, UI Physicians provides care in every medical specialty at UI Hospitals and Clinics, in specialty outreach clinics with community and provider partners across the state, and in off-site specialty and primary care clinics, primarily in Johnson County and eastern Iowa. Outpatient clinic visits predominantly occur within UIHC and a new 5-story stand alone facility at Iowa River Landing (IRL); a total of 818,178 outpatient visits occurred last year, not including surrounding area outreach clinics. Overall, UIP generated $233 million in revenue in FY13. UIP also participates in a statewide alliance of hospitals and physicians called the UI Health Alliance. Together this group covers 2/3 of the population of Iowa (about 2 million lives), from rural farming areas to rural access hospitals, regional centers and UIHC tertiary/quaternary care. UI Physicians brings the best of academic medicine to patients: quality, innovative clinical care with exceptional outcomes enhanced by the benefits of knowledge gained through discovery and ongoing education of the next generation of health professionals. 10

The members of the Alliance are working together to advance the quality of health care services, improve the health status of patients and communities, and achieve efficiencies that will help member organizations reduce the rising cost of health care. The four entities involved include Mercy Health Network (MHN), Genesis Health System, Mercy Cedar Rapids (MercyCare Service Corporation), and University of Iowa Health Care. Purpose The partnership allows each organization to maintain its legacy and independence, while promoting clinical integration and care coordination. It does not involve a merger of assets or changes in ownership or control. The structure is designed to sustain and honor the members local missions and governance authorities, while participating in a larger effort to lead needed changes in the health care system. The alliance also is designed to encourage additional health care providers in Iowa and contiguous states to join and participate in these important initiatives. UIHA efforts Include: Strengthening primary care to ensure Iowans have access to a medical home, through which they can receive preventive services and improved coordination of care across settings and levels of clinical services and specialties. Assisting members in developing performance metrics and comparative reporting to support improvement efforts, i.e. using comparative data to identify and share best practices and reduce the variations in care, which will improve quality and lower costs. Developing programs to determine and address the health status of communities served. Developing educational programs for health care providers, patients and consumers. Developing techniques and systems for improving patients engagement in managing their health. Sharing the high costs of the information systems and experts needed to analyze clinical data and convert it into information that can be used by physicians and others to improve care and better manage populations of patients with chronic diseases. Sharing expertise and operational costs associated with development of accountable care initiatives, enabling the members to evaluate and participate in programs offered by governmental and commercial insurance. Collaborating in research initiatives. 11

13 Examples of current collaborations Cooperation in organ recovery and transplant services. Shared programs in education and graduate medical education for physicians. Cardiovascular collaboration between U of Iowa Health Care and Mercy Health Network (Mercy-Des Moines) for a sophisticated treatment for heart failure called LVAD. Collaboration between U of Iowa Health Care and Mercy Health Network (Mercy-Clinton) for radiation oncology services. CoOportunity Health This collaboration between CoOportunity Health and UI Health Alliance brings two health insurance product options to Iowans: CoOportunity Choice UI Health Alliance, an open-access, tiered benefit plan that provides consumer savings when going to tier one providers - including UIHA facilities and clinicians CoOportunity Preferred UI Health Alliance, a select-network plan that provides the most cost-effective choice for those interested in using an exclusive provider network featuring UIHA providers The UI Health Care/Mercy-CR Accountable Care Organization (ACO) This joint venture arrangement between University of Iowa Health Care and MercyCare Service Corporation provides Medicare fee-for-service beneficiaries with high-quality service and care, while reducing the growth in Medicare expenditures through enhanced care coordination. The service area covers Linn, Johnson, Benton, Cedar, Iowa, Jones, Keokuk, Louisa, Muscatine and Washington counties in Iowa. The program is designed for increasing quality for patients and collaboration with its partners.
