1 Creating a new model of care for a healthier Ohio A Collaborative Vision of

2 Introducing Health Innovations Ohio In the true spirit of collaboration, four of Ohio s most highly regarded health systems are turning our considerable expertise to creating new ways of providing health services that will result in higher quality, better health and greater value for Ohioans. Catholic Health Partners (Cincinnati), Mount Carmel Health System (Columbus), Summa Health System (Akron), and University Hospitals (Cleveland) jointly founded Health Innovations Ohio (HIO) in 2012 as an independent organization designed to realize our shared vision for a new model of care. Inspired by our shared commitment to accountable, high quality, cost-effective care, we are reaching across our organizational boundaries to collaborate on initiatives that benefit residents in our communities. Through HIO, we are learning from each other and sharing best practices for managing the health of individuals and groups. Through HIO, we are working to improve care outcomes and reduce costs by aligning services and incentives across workplace health programs, doctors offices and hospitals. As we carry these innovative practices across Ohio within and between the communities we serve we have the potential to transform care throughout our state. Although we will continue to operate independently, HIO represents four like-minded, future-focused organizations which have come together in a statewide collaboration to create more effective and efficiently coordinated care in the midst of a fragmented healthcare delivery system. Together to Make a Healthy Difference We share a passion for building a better model of care for a healthier Ohio. What if we focused on continued, coordinated care instead of episodes of illness? What if doctors, nurses, hospitals and other care providers shared responsibility for keeping patients healthy? What if we engaged people in being active partners on their own healthcare team? What if we influenced behavior changes that prevent illness? What if we created a measurable difference with this new approach to care in lower costs, higher quality and better outcomes? The members of HIO have come together with the ambitious goal of turning this vision into reality.

4 For Seniors Effective January 2013, HIO arranged broader access to our affiliated Medicare Advantage plans, MediGold and SummaCare. According to the Centers for Medicare and Medicaid Services (CMS)*, each program is ranked among Ohio's highest-rated plans for service and other performance measures. Both programs have favorable reputations for consistently delivering affordable health insurance and quality, coordinated care to Ohio Medicare beneficiaries. Each is sponsored by not-for-profit Ohio hospital systems, differentiating both from commercial insurance options. As with any Medicare Advantage program, HIO-affiliated Medicare plans maintain a contract with the federal government to operate as private Medicare plans. Consumers who choose to enroll with these programs do not leave Medicare when they join. They simply receive their Medicare benefits, and more, from our affiliated and high-quality Medicare plans. For Medicaid Enrollees The more we can integrate the delivery of healthcare, the more efficiently providers can serve patients covered by managed healthcare arrangements. Through HIO, our four member organizations envision bringing Medicaid managed care members the healthy benefits of aligned, coordinated care by working collaboratively with Medicaid managed care contractors and aligning incentives through pay-forperformance and shared-savings contracts. For Employees and Dependents One of our primary goals for HIO is to research and develop best practices in population health management for our own employees. By applying these practices to engage them in achieving good health, we can have a tremendous impact. As HIO, our four member organizations provide healthcare coverage for more workers and their family members than any employer in the state. By introducing these proven workplace programs and incentives across our collaborative, we can help our tens of thousands of employees attain better health while we also lower healthcare costs. What s more, we envision sharing our approaches and results with other employers so that they, too, can reap the benefits of a healthier workforce. *CMS ratings for Medicare Advantage plan performance are assessed each year and may change from one year to the next.

5 Who We Are HIO s four founding organizations are all long-standing leaders in caring for Ohio. Catholic Health Partners, Cincinnati Catholic Health Partners (CHP) is the largest health system in Ohio and one of the largest nonprofit health systems in the United States, employing more than 32,000 associates in Ohio and Kentucky. CHP is one of the top healthcare systems in the nation for quality and efficiency as rated by Truven Health Analytics (formerly the healthcare business of Thomson Reuters). With $5.6 billion in assets, CHP operates more than 100 health facilities, including 24 hospitals, senior living/post-acute care facilities, hospice programs, home health agencies, primary and specialty care physician practices and a preferred provider organization, HealthSpan. You know CHP through its highly regarded markets named Mercy, Mercy Health, Mercy Health Partners, Humility of Mary Health Partners, Community Mercy Health Partners and St. Rita s Health Partners. Mount Carmel Health System, Columbus A member of the Catholic Health Association and CHE Trinity Health, Mount Carmel has been a leader in patient care for more than 125 years. Its team of more than 8,000 employees, 1,500 physicians and 1,000 volunteers provides an array of cutting-edge, patient-focused primary and specialty healthcare services at four central Ohio hospitals Mount Carmel East, Mount Carmel West, Mount Carmel St. Ann s and Mount Carmel New Albany. Mount Carmel also provides high-quality healthcare services at its surgery centers, multi-specialty physician group practices, ambulatory and urgent care centers, hospice, palliative care, home care services and through MediGold, a Medicare Advantage plan. Mount Carmel is dedicated to medical and nursing education and training as well, with six physician medical residency programs and the Mount Carmel College of Nursing Ohio's fourth-largest baccalaureate nursing degree program.

6 Summa Health System, Akron Summa Health System, headquartered in Akron, OH, is a non-profit Integrated Healthcare Delivery System that provides coordinated, value-based care across the continuum for the people and populations it serves. Summa holds itself clinically and financially accountable for health outcomes in its communities and is committed to providing the right care at the right time in the right place for its patients. Formed in 1989 with the merger of Akron City and St. Thomas Hospitals, the organization now encompasses eight hospitals, multiple community-based health centers, a health plan, a multi-specialty group practice, an entrepreneurial entity, research/medical education and multiple foundations.
