The On Track Program provides a structured environment where students relearn old skills, develop new coping skills, increase independence, socialize and support one another, and learn about community resources. Emphasis is placed on self-esteem building, understanding one's own injury, cognitive skill building and personal goal setting. Our goal is to assist individuals with acquired brain injury to transition from rehabilitation to once again becoming active and participating members of the community. The Program is conducted Tuesday, Thursday, and Friday from 10:00 a.m. to 3:00 pm at the Loma Vista Adult Center located at 1266 San Carlos Avenue in Concord, California.
