This progam allows older Georgians a greater choice in deciding where they live and what assistance they need. The CCSP provides a range of home and community-based services which include: Adult Day Health, Alternate Living Services, Care Coordination, Emergency Response Services, Home Delivered Meals, Home Delivered Services, and Personal Support Services. To be eligible, the client must be certified for intermediate nursing home care, be assessed by a CCSP nurse (RN), and be determined to have health and personal needs which can be adequately met in the community eith established cost limits; and be an eligible Medicaid recipient or potentially eligible for Medicaid. Geriatric Care Managers A Geriatric Care Manager (GCM) is a professional who specializes in assisting older people and their families in meeting their long-term care arrangements.
