SYMPHONY POST ACUTE NETWORK Happy 2015! I hope you had an enjoyable holiday and happy new year. Our member facilities ended 2014 with some exceptional events, achievements, work in the community and solid initiatives for 2015. I m very proud of The Imperial of Lincoln Park in Chicago and McKinley Court in Decatur for receiving a 5-Star Rating from CMS. This is a tremendous recognition for the care and services that both facilities provide to their guests and residents. Additionally, The Renaissance at 87th was #1 in outcomes in the Advocate Health Care Post Acute Network (PAN) for the 2nd year in a row. Among Advocate s goals are 30-day readmissions, length of stay as well as patient satisfaction. This reflects the care and service that The Renaissance at 87th delivers. Symphony member facilities were active in the community as the year came to a close. Many south Chicago member facilities sponsored a Senior Breakfast together with WVON radio. Additionally, The Imperial of Lincoln Park hosted a small get together in support of Advocate Illinois Masonic s Virtual Gala raising money for the President s Fund. Four Member Facilities - McKinley Court, Symphony of Decatur, Avanti Wellness & Rehab and Symphony of Joliet had Grand Re-Openings showcasing their upgraded décor and amenities, as well as enhancements in clinical programming to local medical communities and partners. 2015 will be a great year in Evanston and neighboring North Shore communities, as Symphony of Evanston now has over 100 Medicare and Medicaid beds and a large scale remodeling project to update its décor and amenities will start soon. Symphony of Crown Point and Symphony of Dyer in Northwest Indiana are now accepting reservations for assisted living with plans to open summer of this year. Wishing you a great 2015, DAVID J. All four facilities underwent major renovations and were proud to host their local mayor, chamber of commerce, physicians, hospital partners and members of their respective communities. QUARTERLY REPORT WINTER 2015 THE 23 RD ANNUAL WVON SENIOR BREAKFAST sponsored by symphony post acute network december 6, 2014 For the past 23 years, the WVON Senior Breakfast has brought together hundreds of seniors for a joy-filled morning. This year s breakfast included free flu shots, blood pressure screenings, health tips as well as live entertainment. Symphony was proud to be in attendance to support our incredible senior community. THE IMPERIAL OF LINCOLN PARK HOSTS ADVOCATE S #NOBALLATALL This year, instead of doing the fancy gala, Advocate Illinois Masonic chose to do a #NoBallAtAll. The premise of this event was to continue to raise awareness and funds for the Advocate Illinois Masonic President s Fund, but without the official gala. The #NoBallAtAll ran from November 1-30 and encouraged donors to participate in a virtual setting. The Imperial of Lincoln Park was the premier title sponsor for this years event hosting a fun cocktail hour. 5
