This Request for Proposal (RFP) consists of providing the Phone System and Phones for. Details of the proposed work are set forth in the attached Drawings, Bid Form, Scope-of-Work, and Project Schedule. The completed Request for Proposal must be submitted no later than December 11 th, 2015 COB (5pm EST). We anticipate awarding the contract on or before December 15 th, 2015. Phone System & Phones Invitation to Respond to Request for Proposal (RFP) I. Background: A. Client is interested in acquiring a hosted VOIP phone system for their real estate project at 4261 N 5 th St., Philadelphia PA ( the Project ). The selected bidder will enter into a contract with Client to provide a phone system and phones that align with the RFP attached. B. The selected bidder will be responsible for completing the project per the schedule and coordinating installation activities with the selected General Contractor. A project schedule is included as an exhibit to this RFP. C. Scope of Work: 4261 North 5 th Street Phone System: - A VOIP phone system should be priced (cloud managed) as well as phone numbers and phones. - Phone Specification should be the Polycom VVX410 and add alternate pricing should be included for a Polycom phone spec of the vendor s choice with supporting documentation. - 84 Total Phones will be needed at the 4261 N 5 th St. Location. 73 Premium accounts, 11 Standard accounts, and 1 Conference Bridge account for the Polycom IP6000. - 2 reception Clients at the 4261 N 5 th St. location. - 1 Conference Phones (Polycom IP6000 POE- Sound Stations) with two microphones. Faxing: - 2 cloud hosted fax accounts D. ALL labor and deployment labor surrounding the above scope of work must be included within this RFP. The selected bidder(s) should handle ALL aspects of implementation and connection of equipment. E. RFP Response 1. Client intends to make its decisions based solely on the analysis of the RFP response received by or before the response deadline. It is essential that each prospective contractor submit its best response to this RFP on time. a. The deadline for your electronic response to this RFP is December 11 th, 2015 (COB- 5pm).

All bidders are required to fill out the Bid Form exhibit A 2. Provided for each bidders information is the anticipated project schedule in exhibit B for this location. 3. The bidder s proposal shall include all labor, material, equipment and supervision necessary to complete the work as specified and referenced for the completion of the Client s Project. The following items clarify, but do not limit, the intent of this scope. 4. It is not intended that the drawing or scope of work cover every minor detail associated with the project. If a bidder determines the drawings are in error or incomplete, the bidder should contact Watchdog prior to submitting the bid. Additional costs for work required which is apparent will not be accepted. 5. By providing a proposal for this Project, the bidder verifies that it has familiarized itself with the building regarding access, existing conditions, etc., and any costs associated with same are included in this bid.
