Primary Care CVD Risk Screening Utility for WDHB Release Notes From mid-october North Shore and Waitakere Hospitals will be doing cardiovascular risk assessments on some inpatients. New Zealand has an ambitious target of ensuring that 90% of patients who are eligible for screening have had a cardiovascular risk assessment, within the last five years, by June 2014. Whilst general practice has made good progress in screening we are still about 30% away from the target. Screening in hospital will help ensure that all Waitemata residents are able to benefit from cardiovascular risk assessment and management, if they are at increased risk, as soon as feasible. Management of increased risk will remain with the patient s general practice. Patients will only be screened in hospital if there is no record of risk assessment within the last five years in primary care. Patient s NHIs will be matched against primary care records in PREDICT or The Edge. Only the screening status, not clinical information, will be provided to Waitemata DHB. Once a risk assessment is done in hospital it will be sent to the patient s general practice. It will be seen as a message in the Inbox, and will also be mapped into your Medtech32 system. This will mean that it will be recognised as being completed by the PHO Performance Management Programme. The results of the risk assessment will be discussed with the patient by the risk assessor. If the person is a high risk the patient will be asked to discuss the result with their practice nurse or general practitioner. The assessment will also be provided to the patient s inpatient team but in the large majority of patients management will be most appropriately done by the general practice team. More information on the hospital cardiovascular risk assessment programme will be provided by ProCare and Waitemata PHO prior to launch.

Inbox Screening Setup In/OutBox Inbox Screening This utility will insert into the Inbox Screening Mapping module the new Waitemata DHB Test Subject and Result Code for Calculated 5yr CVD Risk. Once the Inbox Screening Mapping Utility for Waitemata DHB has been run by the Practice, you will be able to see the new Subject and Result Code in the Medtech32 Inbox Screening module. Whilst inserting the new WDHB Code, it will automatically map the Calculated 5 Yr CVD Risk Subject Description to the CVR Screening Term and Screening Value that exists in your Medtech32 Setup. This is to ensure that the Practice will not have to manually map the new Inbox Screening Term when you receive the new WDHB Calculated 5 Yr CVD Risk Inbox Records. It will also ensure consistency in the Screening Term entry in Medtech32 when Providers file Inbox Results for patients. Should you have any questions with regard to the information contained in these Release Notes please don t hesitate to contact the Medtech Helpdesk on 0800 2 MEDTECH (633 832) (Option 1 and follow prompts).
