1.0 INTRODUCTION Colorado Springs Utilities (UTILITIES) is seeking qualified firms to provide verification welding inspection services, nondestructive testing services, and technical reporting to Colorado Springs Utilities (UTILITIES) on an as needed basis for work to be identified on an on-call basis. The range and extent of services will vary depending on the project and UTILITIES needs. No specific amount of work is guaranteed. UTILITIES reserves the right to utilize its own staff or other contracts to provide the services. The successful Respondent(s) will form a pool of pre-qualified firms to perform on a Task Order basis the required services, and on a rotation basis may be asked to respond to Request-For- Proposals and/or address project requirements in an expedited fashion to meet individual projects schedule during this period. Respondent s will be required to provide services on-call, and will have the ability to respond to short notice requests. The services required will cover any facility or construction project occurring on Colorado Springs Utilities infrastructure. This includes the areas surrounding El Paso County, Buena Vista, CO to the west and Pueblo, CO to the south. In addition, Respondents will travel to manufacturer s facilities to perform inspections for UTILITIES. Respondents must be able to perform all scope items in this Statement of Work. Subcontracting of scope items that are not a primary function of the Respondent is acceptable. Respondent will perform 3rd party services to Utilities, so no conflict of interest shall exist between the Respondents, and other members of the Project Team. 2.0 SCOPE & BACKGROUND 2.1 Scope 2.1.1 CONTRACTOR shall furnish Certified Welding Inspector (CWI) personnel certified in accordance with the American Welding Society (AWS) QC 1 and in accordance with project construction documents on an as needed basis. CWI shall have a minimum of 5 years of professional experience related to welding inspection similar to the work. CONTRACTOR shall furnish Nondestructive Testing (NDT) Quality Control personnel certified in accordance with the American Society of Nondestructive Testing (ASNT), requirements of Recommended Practice No. SNT-TC-1A. After receiving the NDT qualification, NDT Quality Control personnel shall have a minimum of at least 5 years of professional experience related to NDT inspection. 2.2 Background UTILITIES has identified a need for on-call welding services and nondestructive testing, due in part to the large amount of large diameter pipe to be installed over the next several years. However, this scope includes services for structural steel as well. For each project the CONTRACTOR shall be provided the schedule, drawings, plans and project specifications to be used as a basis for a quote. Work shall be completed in accordance with project plans and specifications. The services under this contract shall include, but is not limited to, the tasks as outlined below: Inspection of Shop Welds for Steel Pipe and Fittings Inspection and Testing of Steel Pipe Field Welded Joints Inspection of structural steel welds, including floor and roof deck welds Page 1 of 6

Inspection of stainless steel welds Nondestructive Testing, i.e. Ultrasonic, Magnetic Particle Testing and Radiographic Review of manufacturer s welding procedure specifications (WPS) Review of manufacturer s QA/QC plan Review of manufacturer s welder certificates of compliance Review of manufacturer s and field contractor s welder qualifications Review of compliance with construction documents 3.0 TASKS, SCHEDULE & DELIVERABLES 3.1 Tasks CONTRACTOR shall provide a certified welding inspector (CWI) and nondestructive testing (NDT) personnel to complete the following Services, including, but not limited to: 3.1.1 Perform nondestructive joint weld testing and CWI examinations as required. Welding inspections shall be performed in accordance with the latest construction documents and revisions as provided for each project. Scope of projects may include field welding inspection and nondestructive testing work. 3.1.2 Field Inspection and Testing of Steel Pipe Fabricated Specials and Fittings 3.1.2.1 Inspection and testing of field welded butt joints: Work may include, but not be limited to, inspection of the weld by visual observation and radiographic or ultrasonic testing methods in accordance with the requirements of the project construction documents. Defective welds will be re-inspected after repair by repeating the original test procedures. Equipment required for testing shall be provided by the CONTRACTOR. 3.1.2.2 Inspection and testing of interior and exterior field fillet-welded lap joints: Work may include, but not be limited to, inspection of the weld by visual observation and magnetic particle inspection or liquid dye penetrate testing methods in accordance with the requirements of the project construction documents. Defective welds will be re-inspected after repair by repeating the original test procedures. Testing may also include air-soap leakage testing. Equipment required for testing shall be provided by the CONTRACTOR. 3.1.2.3 Complete field reports shall be provided that include dates of inspection, descriptions and results of all welds inspected and reinspected until passing results are achieved. 3.1.3 Shop Inspection of Steel Pipe Fabricated Specials and Fittings 3.1.3.1 Visually examine and confirm shop fabrication, shop welding, and shop nondestructive testing of cylinders, specials, and fittings to confirm compliance with pipe manufacturer s welding procedure specification (WPS) and contract documents. Test shop welds by magnetic particle, liquid penetrant, ultrasonic, or radiographic methods as appropriate and as required. Page 2 of 6

3.1.3.2 Review of pipe manufacturer s source quality control reports. 3.1.3.3 This description of Work, while not intended to cover all details, outlines the major items and Work to be accomplished. The CONTRACTOR shall bring nonconforming items to the immediate attention of the UTILITIES Representative, and note all such items in the daily report. If any item is not resolved in a timely manner or is about to be incorporated into the work, The CONTRACTOR shall immediately notify the UTILITIES Representative by telephone or in person, and post as a discrepancy in the inspection report. 3.1.4 Testing and Inspection of Structural Steel, Joists, Decking, Metal Wall Panel 3.1.4.1 Structural Steel Framing a. Attend pre-installation conference for steel erection. b. Respondent is responsible for the testing and inspections of Structural Steel Framing as specified in the specifications. The Respondent will not be responsible for enforcing requirements relating to aesthetic field. c. Implement Shop and Field testing program for UTILITIES as required by the specifications. d. Material & Welder Qualification i. Review each contractor s mill reports covering chemical and physical properties of steel conformance with the applicable ASTM specification ii. Review each contractor s field welder qualifications for conformance with AWS qualification requirement; per AWS D1.1. e. Bolted Connections: Bolted connections must be inspected according to RCSC s specification for structural joints using ASTM A325 or A490 bolts. i. Visually inspect bolted connections ii. iii. Perform calibrated wrench tests on all bolted slip critical connections. For connections not designated as slip critical inspect connections to insure that the plies of the connected elements have been brought into snug contact. f. Welded Connections: Welded Connections must be tested and inspected according to AWS D1.1. i. Perform inspection VT of all welded connections ii. iii. Perform testing UT of all complete penetration welds. Complete penetration CJP welds of HSS members must meet AWS class R acceptance Criteria g. Shear Connectors: connectors welded studs shall be tested and inspected according to AWS D1.1.., Clause 7 Stud Welding i. Perform visual inspections of all welded shear connectors. Page 3 of 6

4.0 SCHEDULE ii. Perform bend tests if visual inspection reveals a less than continuous 360 degree weld and at intervals required in the contract documents. 3.1.5 Steel Floor and Roof Deck 3.1.5.1 Perform inspections and tests as follows: a. The Respondent will inspect deck, deck fastening and side lap fastening and welding as per requirements set forth in the specifications. b. Welders Certification Qualification: Verify that qualification of welders is not more than one year prior to time welding work is to be performed. c. Shop and Field Welds: Visually inspect all shop and field welds. Conform to America Welding Society, Structural Welding Code -Steel, AWS D1.1 & Structural Welding Code-Sheet Steel, AWS D1.3, current edition. 3.1.6 Cold Formed Metal Framing The Respondent will be responsible for performing field test, inspection and prepare test reports to make sure that the work meets the requirements set forth in the specifications. Schedules shall be unique for each project. 5.0 DELIVERABLES CONTRACTOR shall provide the qualified CWI necessary for a reliable, high quality project as defined in the project construction documents. 5.1 Furnish Final Report. The CONTRACTOR shall submit written field reports periodically to Utilities. The CONTRACTOR shall submit a final signed report to UTILITIES stating that all items requiring special inspection and testing were fulfilled and reported and, to the best of his/her knowledge, in conformance with the approved design drawings and specifications. Items not in conformance, unresolved items or any discrepancies in inspection coverage (i.e. missed inspections, periodic inspections when continuous was required, etc.) shall be specifically itemized in this report. Provide project photographs as necessary to document the findings. A CD containing the final report and all periodic inspection reports will be provided within one week after the conclusion of a project. 5.1.1 Prior to the beginning of any work, contractor shall provide current certifications of all welding inspection personnel. No welding tester is to perform any Work without the UTILITIES Representative having the welding inspector s current certification(s). The welding personnel shall have all certifications complete prior to arriving on the job site. 5.2 Acceptance Page 4 of 6

5.2.1 Respondents shall demonstrate compliance with all standards, certifications, and training as required for the services under this contract. 5.2.2 The code of acceptance for the personnel qualifications shall be the "American Society of Non-Destructive Testing or American Welding Society, current revision or as incorporated by reference. 5.2.3 Respondents shall provide the firms written safety plan addressing jobsite safety, worker protection, and training. 6.0 CONTRACTOR S PERSONNEL 6.1 Key Personnel 6.1.1 CONTRACTOR shall provide a list of Key Personnel and subcontractors as required by UTILITIES. UTILITIES reserves the right to view and approve/disapprove any Key Personnel and subcontractors prior to start of project. Contractor shall demonstrate that they can support a sustained operation while providing ASNT Level II or higher personnel. Contractor shall have a Level III technician on staff to supervise and consult with Level II personnel. CWI s shall be current on visual (eye) testing as required by the code being inspected to. 6.1.2 The CONTRACTOR shall provide qualified personnel, within a 24 hour notification, at the project site and/or the off-site fabrication yards, necessary to undertake the specified services. The number of such staff shall be sufficient to perform such service in accordance with the construction schedule and sequences of construction utilized by the general Contractor and its subcontractors. 6.1.3 CONTRACTOR S personnel shall have current confined space training. The pipeline installing contractor will provide ventilation and lighting. The CONTRACTOR is responsible for providing all other protective equipment for CONTRACTOR S personnel. 6.1.4 Certain CONTRACTOR personnel are considered essential to the work being performed and as such shall be referred to as Key Personnel hereunder, and to such Key Personnel the following provisions will apply: 6.1.5 CONTRACTOR shall, at all times, provide Key Personnel of the appropriate experience, qualifications, and knowledge to perform the Work as specified in this Statement of Work. 6.1.6 Prior to diverting any of the specified Key Personnel to other programs or positions, or substituting any key personnel, CONTRACTOR shall notify the identified UTILITIES in writing in a timely manner with proposed substitutions including their experience, background, and certifications as related to this type of work. Replacements or substitutions of key personnel shall not be permitted until written approval of the replacement or substitution has been received from UTILITIES. Page 5 of 6

6.2 CONTRACTOR Personnel If CONTRACTOR s personnel lack the appropriate qualifications for this Work, CONTRACTOR shall immediately replace the non-qualified employee with another employee with qualifications appropriate to the Work being performed. UTILITIES may also require that CONTRACTOR s remove from the job, at no additional cost to UTILITIES, employees who endanger persons or property, are disruptive to the workforce, or whose continued employment under this contract is inconsistent with the requirements of the contract and/or interests of safety or security at the project site. Replacement of personnel shall be at CONTRACTOR S expense. Page 6 of 6
