What is the CarePath by Janssen Provider Portal A tool that will: Give you 24/7 online access to your XARELTO, INVOKANA and INVOKAMET patients activity within CarePath by Janssen in real time Allow you to submit requests for benefit investigations for new and existing patients with a few simple clicks Check status of existing requests, whether placed online or by fax Assist in creating customized support letters for your patients Manage your patients prescription needs with ease! 3
Getting Started - Landing Page Physicians and office contacts can register by clicking this button. Registered users log in here. 4
Getting Started New User Registration New users can register by completing the registration form. Required fields are marked with an asterisk. The email address entered here will be used to log in once registered. 5
Getting Started - New User Registration Users will input the registration information into the required fields. 6
Getting Started New User Registration Prescriber information is needed for doctor or office staff registration. Office staff Physician 7
Email Alert Notification: Opt-In Users can opt-in to email notification alerts by clicking the Yes radio button. Select Yes to receive email notifications or No to register without enrolling in email notifications. 8
Registration Results Page User will receive a message stating the registration will be completed within 1 business day 9
Getting Started Home Page The user s home page will be customized to their office. 10

My Alerts Tab My Alerts tab lists patients with status changes. Open and closed cases with a status change up to 30 days or prior to today s date. 12
My Open Cases Tab My Open Cases tab lists all open cases. All open patient cases. 13
Search Cases Tab Search Cases tab allows users to search cases by various criteria. 14
Account Information Clicking on the My Account tab brings up the user s profile information. They can also subscribe to email alert notifications. Update practice information with this tab. Subscribe to patient status notifications via email with this tab. View and manage office staff users with this tab. 15
Invite Users Invite additional users to register on the portal by using the Invite Additional Users button and complete the required fields. 16
Instant Benefit Investigation Users can request an Instant Benefit Investigation (IBI) by clicking the Submit Patient tab. The IBI only shows the patient out-of-pocket amount if successful. An unsuccessful IBI will display a rejection message. 17
Full Benefit Investigation Users can request a full benefit investigation or a reverification of benefits by clicking Full Benefit Investigation (BI) option under the Submit Patient tab. Request a benefits investigation by following this link. 18
Prior Authorization Services When requesting a Full Benefit Investigation, the user can also request prior authorization assistance by checking the Form Assistance and/or Status Monitoring checkbox(es). Select a Prior Authorization Service. 19
Electronic Prior Authorization (epa) Users can request a prior authorization form by clicking the Initiate Electronic Prior Authorization (epa) option under the Submit Patient tab. Select an existing patient or enter a new patient. 20
Benefit Investigation Status Users can view the status of submitted Instant Benefit Investigation or Full Benefit Investigation requests by selecting the Benefit Investigation Status option under the Submit Patient tab. Open cases, closed cases, or all cases can be selected using the drop-down menu. Search all patients or locate a case by patient name or ID number. Limit search by date range. 21
Prior Authorization Status Users can view the status of submitted Electronic Prior Authorization requires by selecting the View Electronic Prior Authorization Status (epa) option under the Submit Patient tab. Search all patients or locate a case by patient name. Limit search by start date. 22
Support Letters Users can download a sample letter or create a customized letter by selecting Support Letters under the Support Materials tab. 23
Customize Support Letters Users customize the letter by inputting requested patient information into the wizard. Information already in the system will be pre-populated. 24
Support Materials Each product s Prescribing Information is also available. The Benefit Investigation Form for XARELTO, INVOKANA, and INVOKAMET will open in a new window. 25
Links Users can access additional web sites directly from the portal. They will open in a new window. 26
Search Users can easily search for patients using a drop-down menu listing their patients by name. Select patient, then select Go. 27
Search Results Click on the Case Number hyperlink to see additional case details. Click on the Case Number hyperlink to see additional case details. 28
Case Detail The Case Detail screen shows case dates, status notes, patient contact, and pharmacy payer information. Case Detail shows case dates, progress notes, new task, and resolution. 29
