Instructions for Completing the CSUMentor Application for Alcohol and Drug Counseling The on-line admission application can be accessed at www.csumentor.edu. If you are new to the CSUMentor website, click on the CREATE AN ACCOUNT button to create an account. You will need to create a username and password to access the online application. In order to create your own unique account, you will need to supply information such as your name, date of birth, e-mail address, etc. Your personal e-mail address is as important to us as your street mailing address or your phone number since our campus will use your e-mail address to communicate with you. Once you completed the account creation process, you can then Log on. If you already created a CSUMentor account, click on LOG ON to proceed with the application. 1. Click on the Apply Online tab. 2. Click on the Undergraduate Admission Applications if you are graduating from high school and entering college as a first time freshman. This link is also for people who are transferring from community college or another college and have not earned their bachelor s degree. Page 2
3. Go to the CLICK THIS BOX IF YOU ARE APPLYING FOR: box for the term of your choice. 4. At the Personal Application Manager screen, select CSU Dominguez Hills and click on the [start new application] link. Page 3
5. Read campus information. 6. Scroll to the bottom of the page and click on Begin Undergraduate Application for CSU Dominguez Hills. 7. You will now be in the first screen of the application. On the left side of the screen, you will see a series of buttons to help you navigate through the application. Page 4
Screen 1 Enrollment Information 1. Term applying for <Select a term for which you are applying> 2. Select Intended major <Alcohol Drug Counseling Certificate> 3. What is your degree objective? <Select None> 4. Teacher or other credential program <Select Not interested in a credential program> 5. Subject to application and availability, do you plan to live in campus housing? <Select Yes or No> 6. Entry Status/Total college transferable units: <Select your entry status> 7. Previous Application/Attendance (if applicable) < Select Never Applied or if you have applied or attended before, please click on the term and enter the year.> 8. At the bottom of the screen, Click Save and Continue to go to the next screen. Page 5
Screen 2 Name and Address 1. Enter your legal name 2. Enter mailing address. IMPORTANT: If mailing address is the same as your permanent address, then click on Click here if your current mailing address is the same as your permanent address. 3. Enter your phone number. Enter your email address. Make sure to provide a current and valid E- mail address. 4. Click save and continue to go to the next screen. Page 6
Screen 3 Personal Information Use stated responses when provided and enter information to all required questions as prompted at the screen. 1. If you don t have a social security number, you can skip the field and proceed to answer the next fields. (Date of Birth, Place of birth and Citizenship). 2. Answer the questions pertaining to your residence. Page 7
3. Skip this section. You don t have to answer the questions in this section. 4. Miscellaneous Information. Select Yes or No. 5. Click save and continue to go to the next screen Page 8
Screen 4 CA Residency Information 1. Answer the following California residency questions. 2. Click save and continue to go to the next screen. Page 9
Screen 5 Demographic Information 1. The questions on this screen are to conform to the new guidelines from the Federal government. Please answer the questions as required. 2. Select Gender. 3. Answer the following Military questions. 3. Skip Parent/Guardian section. 4. Click save and continue to go to the next screen. Page 10
Screen 6 High School Information 1. Add your High School Attended, by clicking on Add A High School. Another window will open where you will select the state of your high school and the country of your school. Once you select a state and a country, you can search for a school in the Search box, or you can just click on the first letter of the name of your high school and a list of high schools will display. Example shown below: 2. Once you have located your school, click on it and then click on the next button. Another window will open. Select term type of the school and enter your dates of attendance and then click on add high school at the bottom of the window. Page 11
3. The system will now take you back to the High School Information screen where you will then enter your high school graduation date. If you received a GED, enter the date in the appropriate box. See below screenshot. Graduation date = (e.g., 06/2014; Note: the date has to be in the MM/YYYY format) OR click on the checkbox of the Check here if you will not have graduated from high school or will not have received a GED. 4. Test Information section. (Skip this section) 5. TOEFL/IELTS/PTE Academic (Skip this section) 6. Click save and continue to go to the next screen. Page 12
Screen 7 College Information 1. Entering a College/University on this screen is the same steps as adding a high school. If you have attended multiple colleges, you can add the colleges by clicking on Add a College/University button as many times as you need to to enter all of the colleges you ve attended. 2. In the Advanced Standing Credit, check all that applies to you. 3. In the Transfer Information section, enter the information requested. 4. In the following section of this screen, please select YES or NO to both questions. Page 13
Screen 8 - High School Preparation 1 1. You will need to complete this section if you are entering as a first time freshman or a transfer who has fewer than 60 transferrable semester units. REMINDER: Please have your transcripts in front of you while completing this section. This section requires detailed and accurate course information for Area A: Social Science/US History/Government Area B: English Area C: Mathematics. 7. Please follow instructions on this screen. 8. Click save and continue to go to the next screen. Page 14
Screen 9 High School Preparation 2 1. This screen is the continuation of the high school preparation which includes the following: Area D1: Biological Science Area D2: Physical Science Area E: Language Other Than English Area F: Visual/Performing Arts 2. Scroll down to the end of the screen, enter your high school gpa. 3. Click save and continue to go to the next screen. Page 15
Screen 10 College Courses 1. If you are taking any courses in progress, enter courses by clicking on Add A New In Progress College Course button. Enter the required information when the window opens. Repeat if there are more courses to enter. 2. Enter courses planned. 3. Click save and continue to go to the next screen. Page 16
Screen 11 Educational Opportunity Program (EOP) 1. Are you interested in applying to EOP? = No 2. Click save and continue to go to the next screen Screen 12 Application Fee Waiver 1. Select No. 2. Click save and continue to go to the next screen. Page 17
Screen 13 - Review Your Application 1. Begin to review your application. 2. When you have completed the review and make the necessary corrections to your application, click on the SUBMIT YOUR COMPLETED APPLICATION. 3. CSUMentor site will prompt you the option to pay the application fee. You are NOT required to submit application fee payment. 4. VERY IMPORTANT: You must select Check or Money Oder payment option in order for us to appropriately waive your application fee. 5. Enter your name in the Signature field. 6. Click on the button to submit your on-line application. 7. Do NOT log-out or close your browser until you receive a submission confirmation number. 8. Print out the confirmation number page for your record. Page 18
