Project Brief Titwala is known for Shri Mahaganapati Temple, which is visited by thousands of devotees every day. Situated at about 65 km. from Mumbai; it is accessible by Central Railway suburban trains passing through the ever busy Kalyan railway junction and also by road through well organized network of State Transport (ST) buses. Titwala and surrounding 68 villages from Kasara to Shahad including taluka places like Murbad & Shahapur have a population of nearly 2,50,000 to 3,00,000 people. Creative Group, a Registered Charitable Trust, under the able leadership of its Founder & President Mr. Vikrant Bapat, a Neuro Technologist by profession; is an association of service minded & dynamic youth from Titwala town. They have been in the field of community service for the last 11 years. It all started by donating school uniforms to 21 poor & needy children in Vidya Mandir School in Manda-Titwala village in June 1999. The inadequate infrastructure, road conditions, electrical & water supply services, medical facilities in the neighboring areas and its effect on the community; prompted these young leaders to start working on many more community service projects. Soon, they realised that the facilities offered by Kalyan Taluka Village Hospital at Goveli and Primary Healthcare Center (PHC) at Khadavli did not serve the purpose in emergencies. There were many cases of patients dying due to either snake bites, heart attack and other serious cases in this area because of nonavailability of emergency medical treatment. This led to establishing of a Medical Polyclinic in October 2000 to offer consultation of specialist from different faculties and diagnostic services (e.g. ECG, X-ray, Pathology Lab, Physiotherapy Lab etc.) to the community at affordable rates. Since then, Creative Group has been successfully managing this facility on the No Profit No Loss principle. In June 2007, to further the motive of serving community more effectively, Creative Group started working on a bigger project. The vision is to build & operate a secondary care Hospital, with 50 beds including Operation Theatre, 2 ICU beds, 6 beds in Maternity Ward and 4 beds in Casualty. The hospital will provide secondary level of medical care including stabilizing the patients before they are referred to a tertiary level hospital for any advanced care. The hospital is being constructed near Shri Mahaganapati Temple in Titwala on a land measuring 7,100 sq.ft. Part of this land is donated to Creative Group by Smt. Pramila Vasant Dalal, a resident of Dombivili. With proper permissions from Municipal Corporation & other authorities, hospital building construction started in April 2010. Shri Siddhivinayak Temple Trust, Titwala is donating INR 1,05,00,000/- (USD 2,33,333/-) towards construction of the hospital building. The total project valuation is estimated to be INR 2,80,28,000/- (USD 6,22,844/-) In early 2010, Creative Group approached our club, Rotary Club of Mumbai Parleshwar for seeking donation of various medical equipments valuing INR 42,82,102/- (USD 95,158/-). Our Club member Rtn. Pramod Lele, CEO, P. D. Hinduja National Hospital, a renowned medical complex in Mumbai is an advisor to this upcoming project of Creative Group. After reviewing the project in detail, the club has decided to participate by donating the required medical equipments and further the motto of Rotary to serve the community. The club has reserved funds valuing INR 10,00,000/- (USD 22,222/-) as its contribution towards the project. The details of the equipments required by the upcoming hospital are as mentioned in Requirements.pdf 1.004 2

Creative Group, in its financial projections; has budgeted for the maintenance of these medical equipments & shall maintain these equipments after the free warranty period from the suppliers. We intend to do this project under Global Grants Scheme of The Rotary Foundation. Under this scheme, The Rotary Foundation matches the contributions by Rotary clubs; which enables the clubs to multiply their budgets and carry out bigger projects. A similar project, being carried out by our club under Matching Grant No. 72686 of The Rotary Foundation at Dr. V. N. Shirodkar Municipal Maternity Hospital, Mumbai, India is in the completion stage. Our club has set up a Project Committee to administer the project activities. The committee comprises of Past Club Presidents and other enthusiastic Rotarians. The time span estimated to carry out the project is about one year. Creative Group has assured us to exchange due credit in favour of The Rotary Foundation and participating clubs. The Rotary Club of Vile Parle Charitable Trust; which is our income & expense management arm for projects, will manage all financial transactions of this project. All donations to this Trust in Indian Rupees. can avail Income Tax exemption under 80G section. Our Trustees & the office bearers stand guarantee for rightful utilization of funds & proper execution of the project.
