TELEPHONE BANKING Beginning Monday, May 6, 2013 at 8:00 am (CT), you will be able to use Providence Bank s Telephone Banking, a 24-hour phone banking service, by dialing 888-407-BANK (888-407-2265). e first time you call Providence Bank s telephone banking, you will need to choose option 5 to establish a new four-digit Personal Identification Number (PIN). You will need to enter your NEW Providence Bank account number and then you will be prompted to enter a four-digit PIN. At this time, your temporary four-digit PIN number will be the last four digits of your Social Security Number. e system will prompt you to enter a new four-digit PIN. Once you have completed this process, you will have full access to Providence Bank's Telephone Banking. HOW DOES ONLINE BANKING AND BILL PAY BANKING ONLINE WORK? All users of State Bank of Countryside s Online Banking service will be able to use Providence Bank s Online Banking and Bill Pay services beginning Monday, May 6, 2013. As of Friday, May 3, 2013 at 4:00 pm (CT), you will no longer be able to access your account information or download history via State Bank of Countryside s online service. To make this transition easier, we strongly recommend you make a record of your alerts, transfers and bill payment history, prior to May 1, 2013, as you will need all this information to re-establish your alerts, transfers and ongoing bill payment transactions. If you have any bill payments scheduled, please ensure they are scheduled to be paid on or before ursday, May 2, 2013. Please be certain to download your account history to a program like Quicken or Microsoft Money or print account history prior to 4:00 (CT) pm on Friday, May 3, 2013. NOTE: If you have any reoccurring bill payments scheduled, please delete those reoccurring payments before ursday, May 2, 2013. You will need to re-establish those reoccurring payments on Providence Bank s bill payment system. On or after May 6, 2013, you will be able to login to Providence Bank s Online Banking. Visit providencebank.com and enter your temporary User ID. Your temporary User ID will be your first initial, last name and birth year in all lowercase characters. Example: John Doe was born May 2, 1962 - his temporary User ID would be jdoe1962. Your temporary password will be your date of birth in MM/DD/YYYY format. Example: John Doe's temporary password will be 05/02/1962. Note, you must use the forward slashes in with your date of birth for the password. Once you login, you will be asked to create a new password, enter your primary email address and answer security questions that will allow you to use our forgotten password system. Once logged in, you may change your temporary User ID by accessing User Services and clicking on Change Access ID in the Security Settings. You will receive an email from Providence Bank that you have a statement available to view the day the statement becomes available. You will be required to register your email address and a password with the first notification. If you wish to receive your statements through Internet Banking, simply click on view statement and you will be taken through the registration process for Internet Banking Statements. With Internet Banking Statements, you will have up to 18 months worth of statement history. HOW WILL MY PERSONAL DEBIT/BANKING CARDS WORK? Each cardholder will receive a new Providence Bank Visa Debit Card. If you have any bills that are being paid automatically via your debit card, you must notify the company of your new debit card number and expiration date. DIRECT DEPOSITS AND ELECTRONIC FUND TRANSFERS FOR PERSONAL AND BUSINESS ACCOUNTS All of your electronic fund transfers will remain in place and will be routed to your account at Providence Bank beginning on May 6, by way of special arrangement with State Bank of Countryside. In most cases, everything will be automatically changed, and you will have no interruption in your direct deposit or transfer. If you currently have any direct deposits, Automated Clearing House (ACH) or other electronic funds transfers, established on your accounts, we will notify all third parties of your new Providence Bank routing number and modified account number. Subsequently, those parties may notify you directly that a change has been requested. However, each company with which you have authorized ACH transactions may have different notification policies. From our experience, here are some examples of what you may encounter: 1) You might receive a check instead of a direct deposit for a two to four week period, until your payroll department has your new account information in place; 2) You might be contacted by the third party asking you to verify the change request; 3) You may not hear from them and experience a seamless routing of your transactions; 4) Generally, you should experience no interruption of service with Social Security deposits. As of May 6, 2013, all future Automated Clearing House or other electronic transfer relationships that you establish should use Providence Bank s routing (ABA) number and account number in place of State Bank of Countryside s numbers. Providence Bank's Routing (ABA) number is 071926375. 18
ACCOUNT NUMBERS Your current account numbers with State Bank of Countryside will be modified when converted into Providence Bank. You can expect to receive a correspondence the week of April 29, 2013. PERSONAL AND BUSINESS ACCOUNT STATEMENTS e statement you receive dated May 3, 2013 will contain activity from State Bank of Countryside. Providence Bank's statement cycles are end of month for all business checking and money market accounts, the second Tuesday of each month for all personal checking, money market, and Health Savings accounts, and quarterly for all personal and business savings accounts. IRA statements will be generated annually at the end of the calendar year. Any service fees incurred will be assessed on a monthly basis. WHEN WILL FUNDS DEPOSITED IN MY TRANSACTION ACCOUNT BE AVAILABLE FOR WITHDRAWAL? Our policy is to make funds for your cash and check deposits available to you on the first business day after the day we receive your deposit. For determining the availability of your deposits, every day is a business day except Saturdays, Sundays and federal holidays. If you make a deposit before 4:00 pm on a business day that we are open, we will consider that day to be the day of your deposit. However, if you make a deposit after 4:00 pm or a day we are not open, we will consider the deposit made on the next business day we are open. Please remember that even after we have made funds available to you, and you have withdrawn the funds, you are still responsible for checks you deposit that are returned to us unpaid and for any other problems involving your deposits. Please refer to the Funds Availability information in the enclosed Providence Bank Account Agreement and Disclosure booklet. WHAT WILL HAPPEN WITH MY DEBIT CARD REWARDS PROGRAM? Your Debit Card Rewards Program will cease at conversion. Providence Bank offers a Perfect Cents Savings account linked to your debit card. If you sign up for Providence Perfect Cents Savings, the change from any transaction done with your debit card will automatically be swept into your savings. When you sign up for Providence Perfect Cents Savings, Providence Bank will match 100% of the round up amount for 90 consecutive days, starting the day of enrollment, and up to a maximum of $50.00. e amount matched will accumulate for one transfer to your Savings account at the end of the 90 day period. HOW ARE CHECKS PAID? Providence Bank pays your checks in check number order, paying the lowest number first. WILL MY CERTIFICATES OF DEPOSIT (CD) ACCOUNT CHANGE? Not at this time. All terms and conditions of your Certificate of Deposit will remain the same through the conversion. is includes crediting of interest through maturity. However, fourteen days before the renewal date of your account, you will receive a notice explaining your options. WILL MY IRA DOCUMENTS CHANGE? e name of your custodian will change to Providence Bank. WHAT WILL THE PROVIDENCE BANK EARLY WITHDRAWALS PENALTIES BE? Early withdrawal penalties for existing CD accounts, renewing CDs and new CDs will be subject to the following: 1% of opening balance or last renewal balance. Minimum $100.00. HOW WILL THIS MERGER AFFECT FDIC INSURANCE OF MY DEPOSITS? FDIC insurance will remain the same, if you currently have deposits at one, but not both banks. For customers with deposits at State Bank of Countryside and Providence Bank, the following insurance coverage will apply: Insurance coverage for your deposits will be separate for six months from the date of the merger, May 3, 2013. After six months, deposit insurance will be based on the combined deposits at the institutions according to standard FDIC regulations. Time deposits (CDs and IRAs) that have a maturity date after the six-month period of FDIC coverage expires are covered until the maturity date of the time deposit account. Time deposits that mature during the six-month period and are renewed for the same amount (with or without accrued interest added to the account) and the same term as the original deposit will continue to have separate insurance until the first maturity date after the expiration of the six-month period. Time deposits that mature during the six-month period and are renewed on any other basis, or that are not renewed and become demand deposits will be separately insured only until the end of the six-month period. Effective May 6, 2013, deposits opened at Providence Bank or the former State Bank of Countryside, will be aggregated when calculating FDIC insurance. 20
HOW WILL THE TAX REPORTING WORK FOR 2013? Form 1099, 1099R, and 5498: State Bank of Countryside will prepare and mail these statements to you for any reportable activity, including interest paid to you, from January 1, 2013, through May 3, 2013. Providence Bank will assume responsibility for reporting all activity that occurs after May 3, 2013. WHO DO I CALL REGARDING QUESTIONS ABOUT MY ACCOUNTS? WHAT WILL HAPPEN TO MY SAFE DEPOSIT BOX? Your Safe Deposit Box at the Orland Park Office will convert to Providence Bank. You will still have access to your boxes during normal business hours as you have in the past. Providence Bank will be working with you in the coming weeks on converting your box into a self storage box which gives you the flexibility to access your box, during normal business hours, without the need for an attendant.
