About the LPN-AAS Nursing Program The Colorado Mesa University LPN-AAS (RN) program is fully accredited by NLNAC (National League for Nursing Accrediting Commission, Inc.) This program is designed for licensed practical nurses to achieve an associate of applied science in nursing degree, opening up greater employment opportunities, increased compensation, and more job security. Students will develop nursing skills in caring for a diverse population in today's health care settings. They will utilize data related to the client's biological, psychological, socio-spiritual, cultural and developmental needs to promote, maintain, and restore health. The associate degree nurse takes the NCLEX-RN and becomes a registered nurse. The associate of applied science in nursing graduate is a technical nurse who has achieved a balance between general college and nursing education. The associate of applied science in nursing RN is prepared to be a direct care giver in hospitals, long-term facilities, and ambulatory care-clinic settings. In order to be eligible to apply to this program the applicant must: Completed all general education and prerequisite courses or will complete them by the end of the semester in which application is submitted Be a graduate of a state approved LPN program Have current LPN license in good standing at time of application to LPN-AAS program and submit proof to that effect Be IV certified at time of application to LPN-AAS program and submit proof to that effect Admission requirements may change from year to year. Application Process 1. Submit a completed Colorado Mesa University application, including application fee, through the Admissions Office or reactivate application file, declaring Associate of Applied Science/Associate Degree in Nursing as area of emphasis. 2. Submit copy(ies) of all transcripts from other colleges/universities and from your LPN school to CMU Office of the Registrar, (contact previously-attended schools and have them send transcripts to CMU Registrar - who will then do a transcript evaluation and make the decision on which courses taken at a previous college will transfer to Colorado Mesa University.) Transcript evaluation by Registrar is required for all courses taken at other colleges. Applicants must have at least a 2.0 ("C") on a 4.0 scale for all courses required for completion of the Associate of Applied Science. This policy applies regardless of when the course was taken. 3. Submit Transcript Request Form (at the end of the application) to the CMU Office of the Registrar. 4. Successfully complete (or be about to complete) general education requirements and prerequisites (applicant must complete all general education and prerequisite courses with a C or better. If currently enrolled in a prerequisite, include an instructor-signed "request for grade" form (available on website or in nursing department) verifying the grade you are expected to receive. 5. Submit a separate LPN to AAS application prior to October 1 for priority consideration for spring entrance. Application is included in this packet and is also available on the Colorado Mesa university website www.coloradomesa.edu/healthsciences/ladder.html, or in the Department of Health Sciences, or at the Montrose CMU Campus. Please be aware that students are admitted contingent upon their passing the criminal background investigation. 6. Submit copy of LPN License with application to LPN-AAS program. (Additional testing may be required if applicant graduated from LPN school more than 10 years ago.) 7. Submit proof of IV Certification with application to LPN-AAS program. How do I apply to the Department of Health Sciences Step-by-Step Tree Begin here! Are you currently a CMU Student? Yes No Apply to CMU one month before Health Sciences application deadline.. Have you taken course at other colleges or universities? Yes Request that transcripts from other colleges be sent directly to CMU Registrar and request that a transcript evaluation is done at least one month before Health Sciences application deadline. No Submit application to Department of Health Sciences 2 weeks before the application deadline. Submit Transcript Request Form to Office of Registrar 2 weeks before application deadline. Are you enrolled in one or more prerequisites at application time? Yes Submit a Request for Grade form to your instructor(s) one week before application deadline. It is student s responsibility to make sure that the form(s) is submitted to Health Sciences before application deadline. No Follow the steps above to make sure ALL steps are complete and ALL paperwork is submitted to Department of Health Sciences by application deadline! 6

After Admission to the LPN-AAS Program 1. Background checks are a requirement to be enrolled in the nursing program and at clinical sites. Approximate cost is $60.00. 2. Proof of a Drug screen is required. Information on completing the drug screen will be given to student after admission to the program. 3. Physical and Immunizations: students admitted to the LPN to AAS program are required to have a physical examination along with proof of several immunizations; proof of two Measles/ Mumps/ Rubella immunizations, Varicella, Tetanus (current within 10 years), Hepatitis B (this is a series of three immunizations), an Influenza vaccination (yearly), as well as a current TB skin test. (Either a two-step TST within the past 12 months or proof of negative TST documented yearly for 2 years. If the student has proof of a one-step TST within the past 12 months, a one-step TST should be done.) 4. Required Professional liability insurance: Students who wish to may apply for insurance through the National Professional Group as a student nurse. The website is http://www.nso.com. Should student choose to go through a different company, the policy must be comprehensive general liability insurance covering bodily injury and property damage liability with minimum coverage limits of $1,000,000 per occurrence/$2,000,000 general total limit, and medical professional liability insurance with minimum coverage limits of $1,000,000 per claim/$3,000,000 annual aggregate. Insurance MUST be for both a licensed practical nurse AND an RN student and this must be stated on the insurance certificate. 5. Submit a copy of appropriate CPR certification card (CPR/AED for Professional Caregiver (ARC) or CPR for the Healthcare Provider (AHA). 6. Grand Junction area students - activate CMU email account by going to the Tomlinson Library computer room (if not already activated). Montrose area students - call the Colorado Mesa University help desk at 248-2111. This account is free for CMU students, and all nursing students are required to activate - and regularly check CMU e-mail. 7. If interested in participating in the AmeriCorps UCAN Scholarship Program, students must attend orientation at a time and location to be announced. Failure to attend an orientation program will eliminate students from consideration for the program for this semester. For information on what the AmeriCorps UCAN scholarship program is, visit the website www.americorps.gov, or call 1-888-507-5962 or e-mail edawardvoucher@cns.gov. 8. Unemployed Grand Junction area students - call the Mesa County Workforce Center at 248-0871 at least one month before classes start regarding possible scholarship dollars.
