Please read all the following information completely and submit all documentation as specified.*** The College of the Redwoods' Licensed Vocational Nursing Program is a three-semester program admission by application approved by the California Board of Vocational Nursing and Psychiatric Technician (BVNPT). Upon successful completion of the program, the student is eligible to take the National Council Licensing Examination (NCLEX-PN) for licensure as an LVN. The student is also eligible to apply for a Certificate of Completion in Licensed Vocational Nursing and/or an Associate Science Degree for LVN. Note: Courses meet requirements of California Board of Vocational Nurse and Psychiatric Technician (BVNPT). 2 General Information 1. The LVN program is offered every year on the Eureka campus starting in the fall semester and concluding at the end of the summer session. On the Del Norte campus, the LVN program starts every other year in the odd years and completes the fall semester of the even years 8/ /2004; 8/ / Evaluation of the student nurse is based upon theoretical knowledge, manipulative skill proficiency, and the application of both of these in the clinical laboratory. To progress and graduate from the nursing program, students must achieve at least C grades in all nursing courses which require a satisfactory clinical grade. In addition, a cumulative 2.0 GPA must be maintained for the Associate Degree. 3. Students transferring from other LVN nursing programs who apply to the program are offered educational mobility opportunities that take into account their previous education. Where applicable, academic credits shall be evaluated and applied to nursing course requirements. Individual counseling and/or testing for advanced placement is available to assist students in making appropriate academic decisions. 4. Applicants who have previously enrolled in the College of the Redwoods LVN program must apply for readmission by consulting with the Director of Nursing and Health Occupations. Only one readmission to the LVN program is possible. 5. All nursing students must possess sufficient computer literacy skills to complete Internet research, communicate by , use CD-ROM, and other computer instruction and word processing. Those without these skills are encouraged to take CIS 1/lL prior to enrollment in the nursing program. 6. At the completion of program when applying for licensure in the State of California to the Board of Vocational Nursing and Psychiatric Technician (BVNPT), applicants are required to report any disciplinary action against a professional vocational certificate and/or misdemeanor or felony convictions; certain convictions may result in denial of licensure. Each case is considered individually by the BVNPT. 7. Availability of clinical facilities necessitates admitting a limited number of students to each class. Therefore, if qualified applications exceed the number of seats available, a random ranking by lottery at the end of the Enrollment Period will determine the awards of seats. 8. The demands of the nursing program are many. Most students find it difficult to carry the load of the nursing program and work more than part time. Clinical scheduling, assignments, etc. cannot be altered due to an individual s outside employment. Financial aid and several scholarships have been available each year. Contact the Financial Aid Office. We encourage students who are waiting for admission to take additional required general education courses for the nursing degree, above and beyond the prerequisite courses. Personal life issues need to be in balance to be successful. One must have reliable transportation, reliable childcare (if applicable), and an ability to be punctual to assigned classes and clinical locations. Tardiness is not an acceptable performance standard in this professional career. 9. Clinical labs may be scheduled for day, evening, and/or weekend shifts. Students rotate to various clinical facilities throughout the college district and are required to provide their own transportation, childcare, etc., to attend clinical labs. Many students car pool; limited childcare services are available at the Child Development Center. 10. Students planning to continue on to the LVN to RN Career Mobility Program may want to take BIOL 6 (Anatomy), BIOL 2 (Microbiology) and BIOL 7 (Physiology) rather than the LVN 114 course. Note that the College of the Redwoods LVN to RN Program considers LVN 114 equivalent to BIO 6 (Anatomy) only. BIO 2 (Microbiology) and BIO 7 (Physiology) are still required for the LVN to RN Program. 2
3 11. Cost of the Program (approximately $2,000 total): a. Enrollment fees for the program are $18.00 per unit per semester. Books, supplies and materials fees cost approximately $680 the first two semesters and $200 the third semester. b. Students are required to have uniforms, white shoes, and a watch with second hand. For specifics, see dress code in student handbook. Cost is approximately $100-$150. c. Immunizations for students who do not have the required immunity to several communicable diseases may cost over $220. Most are available through the Student Health Clinic, Eureka Campus, for reduced fees or free. d. CPR certification costs about $50. e. The school pin (optional) for graduation costs approximately $60-$130. f. Licensure application costs about $300. Note: Above costs subject to change. Entrance to the Program Entrance to the Program is subject to an application process (Eureka annually; Del Norte biennially). In filing an application for entrance to the LVN program, the applicant must verify: Enrollment at College of the Redwoods, High school graduation or equivalent, and Successful completion of prerequisite courses to the nursing program with a grade of C or better (See #4 below): for applicants who have completed the prerequisite courses at accredited colleges/universities other than College of the Redwoods, these courses must be comparable in course content and unit value. The Application Procedure requires the candidate to fulfill all of the following: 1. File a completed Student Enrollment Form to the College of the Redwoods with the Enrollment Services Office. Student must be enrolled at the College to participate in the LVN program. To participate in the class selection lottery, an applicant must have a College of the Redwoods student identification number that has been or will be assigned when you submit your CR Student Enrollment Form. If you have not applied to College of the Redwoods, download an application from the College web page at and submit it with the LVN application. Please indicate Nursing (NURS) as your major. 2. Submit an Application to the LVN Program (with all documents of verification attached) to the Enrollment Services Office during the Enrollment Period between 8:30 a.m. Monday, February 2, 2004 and 3:30 p.m. on Friday, February 13, Applications may be either personally delivered or mailed to the Enrollment Services Office with a postmarked on or before February 13. LVN Application Form is available from Health Occupations, Counseling, or online High school graduation must be verified by submitting an official high school transcript or an official letter from the high school or student may show equivalency by passing the GED (General Educational Development Examination) or an evaluation by a high school, unified district (proficiency examination), junior college, college, university or Board of Education showing that an education equivalent to a high school education in California was obtained. 4. Qualification to be considered for the random lottery selection is based on official verification of all of the following: Prerequisite courses must be completed with a grade of C or better. a. Evidence of high school graduation or equivalent. 3
4 b. Completion of ENGL 350 (Reading and Writing Skills), 150 (College Reading and Writing), or ENGL 1A (College Composition) or English assessment testing eligible for English 150 or 1A. c. Completion of MATH 375 (Pre-Algebra) or Math assessment testing eligible for Math 105/106. d. One of the following: a. Grade of C or better in HO 110, Basic Patient Care. b. Current California Nurse Assistant (CNA) or Home Health Aide (HHA) certification. c. Passing score on HO 110 challenge exam for those meeting challenge application requirements (Contact Health Occupations Office for guidelines and application.) d. Corp person with documentation of military specialty. 5. Students must have completed or be currently enrolled in all prerequisite courses to be considered as having a qualified application. For courses that are in progress during the spring semester at a school other than College of the Redwoods, students must submit verification of current enrollment with the application to the LVN program. Upon course completion students must submit an official transcript verifying grade of C or better. 6. Applicants, who have taken coursework from accredited colleges/universities that is to be applied toward College of the Redwoods/ LVN program, must attach official sealed transcripts from each respective institution. Even if official transcripts have been filed with Enrollment Services in the past, a new transcript must accompany each nursing program application. For equivalency verification, please attach course description for courses taken at other school as specified on the application. DO NOT ATTACH COPIES OF TRANSCRIPTS FROM COLLEGE OF THE REDWOODS. 7. Submit completed application to: Enrollment Services College of the Redwoods 7351 Tompkins Hill Road Eureka, CA Immediately following the Enrollment Period, the Selection Process will randomize qualified applications that have submitted a complete application to the LVN Program with all specified verification documents attached as requested. Unverified and/or invalidated entries on the application form will cause disqualification of the applicant, and applications will be returned. Qualified applications to the LVN program will be selected by lottery until all available seats are filled. If more qualified applications are received than there are seats available, qualified applicants who apply during the Enrollment Period who are not awarded seats will be maintained in order of the random lottery on a waiting list. The Health Occupations Office will notify students by letter of their acceptance into the LVN program or their waitlist number. Acceptance will be subject to completion of all prerequisite courses in-progress with a grade of C or better during the spring semester. From the end of the Enrollment Period until the first day of class in the fall, seats that become available will be filled in order from the waiting list. Once the program has begun in the fall semester, any remaining qualified applicants who applied during the Enrollment Period will be carried forward for the award of a seat in the next Enrollment Period. Only if seats remain unfilled will applications be received after the Enrollment Period for consideration of the award of seats in the LVN program. Applications received after the Enrollment Period will not be maintained on a waiting list for the award of a seat in the next Enrollment Period. Reapplication will be required for a seat in the next year s class for applications received after the Enrollment Period. Therefore, it is in the best interest of the applicant to qualify for enrollment by submitting a complete application with all specified documentation attached during the Enrollment Period. 4
5 Students accepted into the program must return an acceptance letter to the Health Occupations Office to retain their seat. Any student who is unable to maintain his/her seat in the LVN program should notify the Health Occupations Office immediately to permit waiting list mobility. Please keep the Health Occupations Office notified of changes in address and/or phone number/ address. Waiting list applicants will be notified of vacancies. Program Requirements 1. In order to protect the public s safety and health, applicants must be in good health and be able to perform the physical and mental work required in the program. To participate in clinical learning experiences all students must submit the following documents prior to enrolling in the first semester of the LVN Program. a. Proof of tetanus, measles, rubella and Varicella immunity must be documented on the Health Occupations Student Health Form. Hepatitis B is highly recommended but a signed waiver is possible for this immunization only. The vaccines are available from the Student Health Center fall and spring semesters. b. A tuberculosis test. c. A College of the Redwoods Department of Health Occupations Physical Examination and Health History Form completed by a MD, PA, or FNP. Students must meet established physical and emotional requirements for safe clinical practice. Any concerns regarding health status issues should be forwarded to the Director of Nursing and Health Occupations. Without the submitted documents, students will have unexcused absences from clinical learning, which may result in the inability to meet program requirements. d. Freedom from alcoholism, drug abuse or emotional illness that could jeopardize the student s ability to provide safe client care. Students are advised to seek voluntary aid for conditions that could, if left unattended, lead to disciplinary action and may prevent licensure to practice nursing in California. 2. Current, active CPR (Cardiopulmonary Resuscitation) certification, including both adult and child CPR, and both one-rescuer and two-rescuer CPR prior to entering clinical nursing courses. This can be satisfied as follows: a. American Heart Association Basic Life Support (BLS) Level C. b. American Red Cross CPR for the Professional Rescuer course.
