CPE Eligibility CPE eligibility is limited to physician executives of the highest caliber. The credential is awarded by the CCMM based on four criteria: Professional Status Educational achievement Medical management experience Successful completion of the CPE Certification Program Professional Status CPE candidates are evaluated on their status as a physician based on: Medical Training Must be one of the following: Graduate of a United States or Canadian medical school accredited by the Liaison Committee on Medical Education (LCME) Graduate of an osteopathic school of medicine accredited by the American Osteopathic Association (AOA) Certified by the Education Commission for Foreign Medical Graduates (ECFMG) Licensure Candidates must have a current license to practice medicine in the United States, Puerto Rico, Guam or the Virgin Islands. Candidates from other countries must be licensed in the country in which they reside and practice. Clinical Practice Experience Candidates are required to have three years experience in clinical practice beyond residency and fellowship training. Board Certification Candidates must be (or have been) board certified in a clinical specialty recognized by the American Board of Medical Specialties or the American Osteopathic Association. Must have been in good standing if board certification was not renewed. 4

Education CPE candidates must have one of the following: 150 hours of management education This includes 116 hours of tested required courses provided by ACPE (see CPE Course Completion Chart or visit ccmm.org/cpechart), plus 34 hours of tested elective management courses. A limited number of tested courses from other organizations may be considered for equivalency. A graduate management degree (MBA, MMM, MS-HQSM, MHA, MPH or MS in Management) Transcripts must be provided to the CCMM Credentials Committee for review (except for MMM, MBA or MS-HQSM degrees through ACPE). Diplomate status in the American Board of Medical Management Medical Management Experience Candidates must have at least one year of medical management experience. Successful Completion of the CPE Certification Program CPE candidates must attend the 4 1/2-day skill-building CPE Certification Program offered twice a year. Before enrolling for the program, you must complete the online CPE application and submit the required information outlined on page 6. Certification Programs are held two times each year. Designation The American College of Physician Executives (ACPE) designates this live educational activity for a maximum of 32 AMA PRA Category 1 Credits. Physicians should only claim credit commensurate with the extent of their participation in the activity. ccmm.org 5

Application Process and Required Documents Applicants who meet all of the eligibility requirements will become CPE candidates and be scheduled to attend the CPE Certification Program (see page 7). Participation in the full CPE Certification Program is required and the final step to achieving the CPE designation. To apply, complete the application at ccmm.org/cpeapply. In addition, the items listed below are required. Upon receipt of your application and required attachments, the CCMM Credentials Committee will evaluate your application. Medical Training: A copy of medical school diploma or ECFMG standard certificate. Licensure: A copy of current license renewal. Clinical Practice Experience: A CV or resume showing 3 years of clinical practice experience beyond residency and fellowship. Board Certification: A copy of board certification in a specialty recognized by ABMS or AOA. Education: A copy of graduate degree transcript (except for MMM, MBA or MS-HQSM degrees through ACPE). Medical Management Experience: A narrative (not to exceed one typed page) describing management experience. (Management experience means managing people, information and money. For example, managing 10-20 people, hiring, firing, determining salaries, and doing performance evaluations.) A letter of recommendation from candidate's immediate supervisor affirming management experience. In the absence of a supervisor, a letter from the CEO of another significant health care organization in the candidate's community is acceptable. Fees: The non-refundable CPE application fee is $125; the CPE Certification Program fee is $2,500. The program fee is due upon approval of your application. You will be notified by e-mail of the CCMM Certification Committee s decision within approximately four weeks after submitting all required and complete information. CPE Certification Program 4 1 / 2 days / 32 CME The CPE Certification Program goes beyond a written-examination-only approach and includes video practice sessions that focus on management techniques. Candidates must attend in person and are evaluated on their ability to: Deliver a convincing presentation describing their skills and accomplishments to a panel of health care leaders Give feedback within their cohort groups Complete case-study exercises developed around the nine core courses listed on the CPE Course Completion Chart (visit ccmm.org/cpechart). The Certification Program covers the following topics (all testing occurs during the program): Communication Communication Strategies: Effective Listening and Feedback Communication Styles and Techniques of Effective Presentations Management Organizational Politics Influencing Physician Behavior and Dealing with Disruptive Physicians Career The Medical Management Marketplace, Interviewing From Both Sides of the Desk and Networking Executive Image Presentation: Interview and Workplace Attire Salary and Benefits: Negotiation Techniques and Managing the Unexpected Unemployment Mentoring Resources Internet Research: Sources of Current Medical Management Information Certification Presentations Before Assessment Panel The final step in the program is a verbal video-recorded presentation to a panel of health care leaders. Visit ccmm.org for upcoming CPE Certification Program dates. Due to demand, applicants will be enrolled on a first-come, first-served basis. Please do not make travel or hotel arrangements until you receive an approval letter for your application. ccmm.org 7

CPE Recertification CPE certification is effective for 10 years, after which you will need to recertify for another 10 years to maintain your CPE status. ACPE Fellows maintain their CPE certification and do not need to recertify. To recertify your CPE credential, submit the following to the CCMM: Five-minute video describing your accomplishments during the past 10 years, with particular emphasis on the past 3 to 5 years. Also include how you have contributed to the improvement of your organization or its products in ways that have enhanced patient care, using statistics whenever possible. Letter attesting to the accomplishments described in your video and signed by your immediate supervisor. Two-page description of how you have counseled or mentored a younger physician about the future of health care and medical management. Recertification fee of $600. ACPE Fellowship Become an ACPE Fellow. Because of the significant achievements made and contributions to medical management, ACPE Fellows are not required to recertify to maintain their CPE credentials. A Fellow of ACPE is a Diplomate of ACPE who has been a member for the last 5 consecutive years (Diplomate status in ACPE is granted to all Certified Physician Executives), and has demonstrated that his or her knowledge and experience has attained regional or national stature. To be considered for fellowship, you must meet several other criteria and submit a complete application for review by the Council of Fellows. The deadline for Fellowship application is December 31.
