Service Information October 2013 Setup and Installation Your local Authorized LiftMaster Dealer sells, installs and services your new dwellinglive software and hardware. The purchase price includes all necessary hardware and your 1st month s software service. In order for continued service, the community administrator must submit dwellinglive s Activation Form available at http://liftmaster.dwellinglive.com/ activationform.pdf. You will get an email confirmation from dwellinglive when activation is complete. Monthly Software Service LiftMaster will bill your community directly for each month s software service after the 1st month. Terms of Service Monthly Software Services Fees are due on the 1st of each month s service and considered delinquent the 10th. A late payment charge of 6% will be applied to the total unpaid balance on your account if full payment is not received the 10th. Service will be suspended if no payment is received for accounts 60 days past due. A reactivation fee of $150.00 will apply to reinstate service. Monthly Service will increase 2.5% per year. Cancel anytime with 30-day written notice.
