Hello and welcome to the Scorpion Family! In the following package you will find forms and information to help you register for a Scorpion program or team. If you cannot make one of these registration dates please contact us via email at supercheerniagara@gmail.com to arrange another time and date to visit our gym. The registration, competition, uniform and makeup fees are NON-REFUNDABLE. 2. Athletes will not receive a uniform, makeup or team apparel if they quit after payment. 3. Trip deposits and payments are non-refundable at any and all times, regardless if the athlete attends said trip. 4. A $50.00 administration fee applies to ALL REFUNDS requested. 5. There is a $25.00 NSF charge for cheques returned NSF and/or those put on stop payment. 6. ALL tumbling fees are non-refundable. 7. Fee refund requests must be made in writing to Melissa Cecchini @ acecchini@cogeco.ca 8. Refunds will be processed according to the date received in writing by e-mail. 9. No refunds of fees, competition fees or trip expenses and/or deposits will be allowed if athlete is terminated due to the athlete s or parents violation of the code of conduct. 10. If the athlete quits the team prior to the trip; full payment is still required as we do calculate the cost of the trip according to all athletes anticipated participation on said trip; 11. Refunds are processed as follows: a. No refunds of water scorpions or sand scorpion programs once registered. b. Fees for the month following when said athlete quits will be refunded. Ie. If the athlete quits September 1 st, fees for October 1 st till April 1 st will be refunded minus the $50.00 administration fee. 12. Crossover fees are non-refundable and non-transferrable. The Scorpions would like to remain as a positive learning environment and do not tolerate any forms of bullying, bashing or gossiping between clubs, teams, athletes and parents through ANY medium. No drinking or use of illegal substances within any of our programs. If found with any of these substances will mean immediate dismissal from The Scorpion program. We request parents not linger upstairs in the practice area as it is very distracting to the athletes. You are more than welcomed to stay in our parents room as your child practices. Occasionally parents will be invited upstairs to view a routine before an upcoming competition. The Scorpions are not responsible for any lost or stolen valuables in the gym. No Jewellery, FOOD or bobby pins are permitted in the gym. Water only please! If a staff member is talked to out of line or shown negative by an athlete or parent it will mean dismissal from the program.

You may not post any Scorpion routines or routine material online without permission from head office. The placement of an athlete is done by the discretion of the head coaches/owners. Members are responsible for being on time to practices, competitions and other team outings. This is part of the commitment you made to the Scorpion family. ALL PRACTICES ARE MANDATORY! COACHES ARE MORE LENIENT IN SUMMER MONTHS FOR VACATION BUT IF YOU ARE IN TOWN PLEASE BE AT PRACTICE!! Parents are to inform the coach of any late arrivals or miss of practice beforehand via email, text, or phone call. Head coaches numbers will be given out at practice OR ask your team manager for your HEAD coaches phone number. When at competition please behave respectfully because you are not only representing yourself but also the entire Scorpions club. Athletes are required to stay for award ceremonies IN FULL UNIFORM. You may only wear Scorpion athletic wear to practice or to any team outing Parents must pay their financial obligations. I understand and agree with the following rules and regulations enforced by The Scorpions. I have read and understand my financial obligations to the Super Cheer Scorpions. WAIVER FORM AND RELEASE I do hereby acknowledge that I intend to participate in one or more athletic endeavours while a member of Super Cheer Inc. All-Star Cheerleading Team(s). I also acknowledge that I will be doing so of my own free will. I, as parent of the above, or if 18 years or over, acknowledge my child s desire to participate in the cheerleading program of Super Cheer Inc. We understand that the coaches/advisors of Super Cheer Inc. are trained coaches. We further acknowledge that these coaches/advisors will behave prudently in the instruction of cheerleading skills in an effort to avoid accidents and/or injuries from occurring. We realize that participation in athletic endeavours entails the risk of injury to the participants. We accept this risk regardless of the nature of the injury and/or the athletic endeavour in which they/we will participate. We acknowledge that the term athletic endeavour as used herein includes any cheerleading related activity, drill, stunts, choreography, jumps and gymnastics of any kind. We also waive and absolve the Niagara Catholic District School Board; it s schools and any other practice facility, the coaches, advisors, sponsors, parent volunteers, or other volunteers, the staff and/or volunteers of any of the venues in which we practice or perform, or any one or more of them or their executors, administrators, heirs, next of kin, successors or assigns, of and from any and all liability and responsibility for injuries, sickness, accidents, and/or natural occurrences during participation by myself/my child in camps, clinics, private coaching, choreography, and any other cheerleading related activity. We understand that participants are responsible for their own personal health, medical, dental, chiropractic, and accident insurance coverage. We, intending to be legally bound, do hereby, my heirs, executors, and administration, waive, release, and forever discharge any and all rights and claims for damage which I may have or may hereafter accrue to me or my child against Super Cheer Inc., and any other practice facility, the coaches, advisors, sponsors, parent volunteers, or other volunteers, the staff and/or volunteers of any of the venues in which we practice or perform, for any damages which may be sustained or suffered by me or my child in connection with my association with or participation in, or arising out of travel to and/or return from any cheerleading related site or activity affiliated with Super Cheer Inc.

CODE OF CONDUCT ATHLETES I agree to treat the coaches, other athletes and those in other cheerleading programs with respect and good sportsmanship I understand speaking back to my coaches, treating my coaches inappropriately, swearing or making inappropriate gestures could result in dismissal from the team. I agree to attend all practices to the very best of my ability, without being late. I understand being on a team requires commitment & my attendance is crucial. I understand health, fitness and general wellness are a major component of my sport, therefore a healthy lifestyle is strongly encouraged. I will call or advise my coaches in advance if I must miss practice and will give a reason for the absence. I understand being on an elite team means that practice and competitions must take priority over other events. I understand stealing, borrowing things without permission, damaging other s property or purposefully hurting others physically or mentally, and using illegal substances before or during practice, or before or during competitions is all prohibited behaviour and can constitute dismissal from the team. PARENTS I have read through the above with my child and she/he fully understands what is expected in the Super Cheer program and will comply by this code of conduct. I understand all team expectations, including attendance, scheduling and monetary expectations for the team my child will be joining. I understand the importance of my child attending all practices, being on time and Super Cheer s intention of teaching my child responsibility and accountability. I understand parents must not enter the gym as the safety our the athletes come first. I understand feedback is welcome but I will respect the coaches and their authority during practices and competitions by never confronting coaches at practice or competitions, and will take time to speak with them at an agreed upon time and place. I understand competition arrival times as designated by the coach must be respected in order to allow the coaches sufficient time to prepare the athletes for competition. Exceptions will apply for emergency situations only. I understand if my child cannot compete at certain competitions I must advise the head coach 2 months prior, unless for medical emergencies. I understand that my child may have minimal participation in routines following such absences due to the difficultly of re-working routines. I have read and understand the refund policy. If stop payments are placed on cheques to avoid the policy my account will be forwarded to a collections agency. I also understand there is a $25.00 charge for any cheque returned NSF with full replacement value in cash or certified cheque. I understand there will be late fees charged if I am late with payments for trips, etc. due to Super Cheer incurring late fees; unless I have spoken to Melissa Cecchini in advance. I understand I will be required to attend team meetings and must keep myself up to date on team functions, practices, etc, by using the Super Cheer website at www.supercheer.com as minimal paper will be distributed. I will be a positive role model for my child by showing respect and courtesy and demonstrating positive support for all players, coaches, parents and spectators at every practice, competition or other events. 2011 SUMMER SCHEDULE In the summer months while practices are not mandatory it is crucial for athletes to be at practice unless they have gone out of town for vacation. Being at practice is crucial for our teams success. Once the fall schedule starts all practices are MANDATORY. If an athlete is sick he/she is still required to be at practice to sit and watch.
