1 Cost for the Trip $125 if turned in by March 29th $150 if turned in by April 26th $175 if still space in the camp after April 26th Big House 2015 June Bellville, TX Big House is a summer mission trip for students who have completed 6th-8th grade. We spend 2 days out serving in the community and then worship with the host church on Sunday morning. The next page includes instructions to register for the trip. The pages behind that are the forms that need to be filled out. Contact Heather if you have any questions.

2 Big House Registration Instructions & Scholarship Information Registration Steps 1. Fill out and turn in all necessary paperwork. This includes bringing in your insurance card so that we can make a copy and having your medical release form notarized. 2. Pay at least the $50 deposit for the trip. This should be accompanied with a scholarship application where you can fill out a payment plan or list your need for a scholarship. You ll see more information about that below. 3. At this point your registration will be accepted as long as we are still meeting the required adult to student ratio that the trip requires. 4. If you are a student who does not regularly participate in Moody s student ministry, the parents and the student will need to schedule a meeting with Heather before registration will be finalized. Scholarship Requirements 1. Have all paperwork ready to turn in. 2. Scholarships will be offered to cover half of the cost of the trip. At the time paperwork is due, the deposit will need to be turned in along with the paperwork. After that, payment plans can be setup to finish paying the rest of your portion of the cost. 3. You must attend at least 10 student ministry programs between now and the beginning of summer. This includes Sunday school/church; Crossover, JV or Varsity; or any other special events that we have going on. 4. Participated in at least one of the fundraising events: pumpkin patch, Seaside Treasure Festival, Tacky Prom, Pancake Supper, etc. Meeting all of these requirements will give you priority in the giving of the scholarship funds. Those who cannot meet the requirements will be considered after the other students. A few other items 1. All trips must be paid in full before students can leave with us. We will not be able to take a student on a trip with an outstanding balance. 2. If a student chooses not to attend the trip, a refund will be given only if we can find another student to attend in their place. If it s early enough that the organization will offer a partial refund, we will pass that on to you. 3. If you receive a scholarship for a 2015 trip and do not attend without appropriate notice, you will not be considered for a scholarship in 2016 until the fee from the 2015 trip is paid back in full.

8 Big House 2015 Scholarship Application At Moody Memorial FUMC we don t want a lack of funds to keep you from attending an event. We are happy to arrange a payment plan or work out a scholarship of some sort for you. Please complete the following information to help us understand how we can best help you. We are limited on funds that we have to offer, so make sure that you get this turned in as soon as possible. All information is confidential and we will make every effort to help.
