www.accidentmeds.com Personal Injury Clients may receive an AccidentMeds Pharmacy Card from: A Medical Provider/clinic Personal Injury Attorney Medical Provider / clinic Many clinics have pre-printed AccidentMeds cards that they distribute to personal injury patients. These cards are not activated and cannot be useduntil the personal injury attorney authorizes by submitting a Lien or Letter of Protection.

www.accidentmeds.com When a clinic gives a card to your client, they send AccidentMeds a REQUEST FOR ACTIVATION. This form has the information we need to contact your office and request the Lien / LOP in order to activate the card. A fax is immediately sent to the attorney office requesting the Lien / LOP be returned. FAX to (855)795-4367 Card is activated and ready to be used If the Lien / LOP is not received within 24 hours, we will follow up by phone and fax to confirm if you would like to activate the card.

www.accidentmeds.com Attorney Card Requests You have two methods to provide AccidentMeds cards to your clients. 1. We can mail you hard-copy cards (see example) that you can keep on hand and provide to clients when needed 2. You can use the website www.accidentmeds.com to print out a paper card for the client As long as you use the website to Assign Card to a client, both methods offer you the same ongoing card management features.

Once you click : System will prompt you to select User Name and Password Important Note: The default user name will be the input first and last name with a period and no spaces between names. Example: John.Lawyer If you wish, you may change your user name at this time. Create and enter a password System will prompt you to re-enter password Confirm by selecting Create Account Click on upper left corner of screen to return to the Home Page

Once boxes are checked System will automatically present the next screen

Complete all fields Click on SUBMIT

If client is detected as a possible duplicate, you will see this message: IMPORTANT: If there is an existing (active or expired card) for this client, DO NOT issue another card to client. One card per client per date of injury. This is for your tracking and billing records. If you wish to re-open an expired card (not deactivated) card, you can do this in the Manage Card tab. To request re-open of a deactivated card or to request a card limit increase, contact us by phone.

Sample paper card The Card ID is a nine digit number; representing a 5 or 6 digit unique number preceded by zeros

Don t forget to: Congratulations! You have now completed the process. www.accidentmeds.com Program Features Each card has a maximum balance (normally $1000). When the client is nearing the limit, your office is notified via fax. You can request an increase as needed. For better customer service, this is a soft balance as there are endless combinations of drugs and dosages that your client may present at the pharmacy and we cannot fill partial prescriptions. Cards expire in 90 days unless requested otherwise. When a client is nearing the expiration, your office is notified. Medications covered on the program are generics only and must be related to the accident. Formulary and sample charges of common drugs are available on the website Our Pharmacy Benefit Management company controls and tracks prescriptions for abuse or misuse. Clients can check whether their card has been activated online or with the toll-free number (less calls to your office)

www.accidentmeds.com Billing and Settlements Soon you ll be able to request and receiving invoices via phone or web, but in the meantime, contact us (888)795-4367. Invoices are faxed to you as we receive them. There is normally 1-2 week delay before we receive the bills form the pharmacy and are able to immediately provide them to your office. If you have an emergency billing situation, contact us and we ll help as we can. Reduction requests may be faxed to (855)795-4367 Payment is to Key Health. AccidentMeds is the pharmacy program for Key Health. Remit information will be on each invoice.

www.accidentmeds.com Manage Cards You can extend a card for another 30-60 days online at any time If a case has closed, notify us to deactivate the card so that the client cannot continue to use the card. You can do this online. If the card has already expired, it will clearly indicate this. Request a card limit increase by calling our office, but this feature will also be available on the website very soon.
