ADR CATEGORY FOR WHICH CERTIFICATION IS REQUIRED (please tick appropriate box[es]) Type EX / II - For closed bodied or sheeted vehicles intended for the carriage of class 1 explosive sbstances and articles. Type EX / III - For vehicles with a closed, solidly constrcted body which is made of not readily flammable materials intended for the carriage of class 1 explosive sbstances and articles. Type MEMU - Mobile explosives manfactring nit - a nit, or a vehicle monted with a nit, for manfactring and charging explosives from dangeros goods that are not explosives. Type FL - Type OX - Type AT - For vehicles intended for the carriage of liqids with a flash point of not more than 61 C* or flammable gases in tank containers, portable tanks or MEGCs of more than 3000 litres capacity, fixed tanks or demontable tanks of more than 1000 litres capacity and for battery vehicle of more than 1000 litres capacity intended for the carriage of flammable gases. *exclding diesel fel complying with standard EN590:1993, gas oil and heating oil (light) UN 1202 with a flash point as specified in standard EN590:1993 For vehicles intended for the carriage of hydrogen peroxide, stabilised or hydrogen peroxide aqeos soltion stabilised with more than 60% hydrogen peroxide (Class 5.1 UN No 2015) in tank containers or portable tanks of more than 3000 litres capacity, or fixed tanks or demontable tanks of more than 1000 litres capacity. For vehicles other than those of type FL or OX, intended for the carriage of dangeros goods in tank containers, portable tanks or MEGCs of more than 3000 litres capacity, fixed tanks or demontable tanks of more than 1000 litres capacity and for battery vehicles of more than 1000 litres capacity other than those of type FL. For all tank and battery vehicles: Do yo intend carrying: a) Hydrogen (UN1966)? b) Carbon Dislphide (UN1131)? Is the tank made of reinforced plastic? TE: Demontable tanks - tanks which cannot be removed from the carrying vehicles when loaded. Tank containers and portable tanks - tanks which are designed to be removed from the carrying vehicle when loaded for transfer to another vehicle or mode of transport. Battery vehicles - vehicles which carry an assembly of receptacles interconnected by a manifold and permanently monted to the vehicle. MEGCs (Mltiple element gas containers) - nits containing a battery of elements linked together by a manifold and monted on a frame which can be removed from the vehicle when loaded. MEMU - A fll definition is contained in 1.2.1 of ADR International Accord. 3

7. DOCUMENTATION The following docmentation and / or declarations are reqired with the application. Failre to provide these cold delay the application. (TE: Brake, combstion heater and master switch control declarations are not reqired for ADR Type-Approved motor vehicles nless these systems have been modified after type-approval). TYPE EX / II - If applicable, sheet declaration(s). Combstion heater declaration. See parts 5 and 8. TYPE EX / III - Brake, body, combstion heater and master switch control declarations. See parts 3, 7, 8 and 10. For tanks and stability declaration, see part 9. TYPE MEMU - Brake, tank approval certificates, explosive compartment, combstion heater, proof of stability, master switch control declaration. See parts 3, 4, 7, 8, 9 and 10. TYPE FL - TYPE OX - TYPE AT - Brake declarations, tank approval certificates etc and combstion heater declaration. Proof of stability. Master switch control declaration. See parts 3, 4, 8, 9 and 10. As for Type FL or AT and in addition cab declaration (if not fitted with cab screen). See parts 3, 4, 6, 8 and 9. Brake declarations and tank approval certificates etc. Combstion heater declaration. PART 3 8. BRAKING REQUIREMENTS FOR TYPE EX/III, FL, OX, AT AND MEMU VEHICLES A brake declaration completed by the manfactrer will be reqired for certain vehicles which do not comply with ADR type approval at the time of first application for ADR inspection. Those vehicles are: a) a motor vehicle with a design gross vehicle weight exceeding 16 tonnes. b) a motor vehicle athorised to draw a fll drawbar trailer exceeding 10 tonnes design gross vehicle weight, or a centre-axle or semi trailer exceeding 10 tonnes design total axle weight. c) a fll drawbar trailer exceeding 10 tonnes design gross vehicle weight, or a centre-axle or semi-trailer exceeding 10 tonnes design total axle weight. DECLARATION - to be completed by manfactrer* *NB: Dealer declarations are not acceptable. TE: A separate declaration from the manfactrer is acceptable provided it clearly relates to the vehicle. I declare that: In the case of a vehicle referred to in (a) or (b) above the vehicle is eqipped with an antilock braking system of category 1 according to ECE Reglation No 13 Annex 13 or to Directive 71/320/EEC. In addition the vehicle is eqipped with an endrance braking system (retarder) that flfils the reqirements of ECE Reglation No 13 Annex 5 with a laden vehicle mass comprising the laden mass of the motor vehicle and its athorised maximm towed mass bt not exceeding a total of 44 tonnes. In the case of a trailer referred to in (c) above the trailer is eqipped with an antilock braking system of a category A according to ECE Reglation No 13 Annex 13 or to Directive 71/320 EEC. The following declaration will be reqired from the owner for every ADR application relating to the types of vehicle referred to above which are or which have previosly been sbject to a manfactrer s brake declaration or type approval: DECLARATION - to be completed by owner / spplying dealer I declare that no repairs or alterations have been carried ot which will invalidate the manfactrer s declaration or the vehicle ADR type approval which wold affect the performance of either: the antilock braking system, or the endrance braking system For motor vehicles only: The design gross vehicle weight is kg ; the design gross train weight is kg 5

PART 4 9. TANK CERTIFICATES, ETC (TANK, MEMU AND BATTERY VEHICLES ONLY) The applicant mst normally provide the following with the application (copies are acceptable, bt originals mst be prodced on the day of the test): Tank Prodct List (first application only nless list has changed) TC2 or TV6 Tank Initial Approval Certificate Nmber: TC3 or TV8 Tank Periodical Test Certificate Nmber: Water Weight Test Certificate Nmber*: *Applicable for MEMU vehicles carrying solids / granlar /gelatinos sbstances that cannot be issed with a TC2 /TC3 Tank Lining Annal Condition Certificate (only applicable if carrying Bromine) Schedle of battery-elements if not listed at part 1 item 2 (battery-vehicles first application only) The following have not yet been issed bt will be prodced on the day of test: (see note 11 [b]) PART 5 10. SHEET DECLARATION FOR TYPE EX/II SHEETED VEHICLES To be completed by the vehicle or sheet manfactrer or spplier. TE: A separate declaration from the vehicle or sheet manfactrer or spplier is acceptable provided it clearly relates to the vehicle. I declare that the sheet manfactred or spplied by and if applicable marked with the following identification featre: is made of impermeable material and not readily flammable as proven by testing to the B1 level in standard DIN 4102 or eqivalent: BS5438, BS7837 or DIN 75200 are acceptable, as is a brn rate not over 100mm / min as tested according to ISO 3795:1989. DECLARATION - to be completed by owner / operator I declare that the sheet sed is the same as last year and has not been replaced or modified in sch a manner as to invalidate the original vehicle or sheet manfactrer s or spplier s declaration. If the sheet has been changed please obtain the declaration above from the vehicle or sheet manfactrer or spplier. 6

PART 6 11. CAB DECLARATION FOR ALL OX VEHICLES T EQUIPPED WITH A METAL SHIELD BEHIND THE CAB Declaration(s) completed by the manfactrer will be reqired at the time of first application for ADR inspection for all OX vehicles withot a metal shield behind the cab. DECLARATION - to be completed by manfactrer* *NB: Dealer declarations are not acceptable. TE: A separate declaration from the manfactrer is acceptable provided it is clearly related to the vehicle. I declare that only materials that are not readily flammable have been sed in the constrction of the driver s cab. The following declaration will be reqired from the owner for every ADR application relating to the type of vehicle referred to above. DECLARATION - to be completed by owner / spplying dealer I declare that no repairs, modification or painting of the cab have been carried ot in sch a manner as to invalidate the manfactrer s declaration. 7

PART 7 12. BODY DECLARATION FOR TYPE EX/III AND MEMU VEHICLES A declaration completed by the manfactrer will be reqired at the time of first application for ADR inspection. DECLARATION - to be completed by manfactrer / body bilder or spplier TE: A separate declaration from the manfactrer is acceptable provided it clearly relates to the vehicle. I declare that in the case of a Type EX/III motor vehicle or trailer first sed prior to 1 Jly 2005: The body is solidly constrcted of not readily flammable materials and the materials sed for the lining are incapable of prodcing sparks. The inslating and heat resisting properties of the body are at least eqivalent to those of a partition consisting of a metal oter wall lined with a layer of fire proofed wood 10mm thick, or the body is of a constrction which ensre no flame penetration of the wall or hot spots of more than 120ºC on the inner wall srface will occr within 15 mintes from the start of a fire likely to occr from the operation of the vehicle. No materials have been sed in the constrction of the body which wold be likely to form dangeros componds with the sbstances carried. I declare that in the case of an EX/III motor vehicle or trailer first sed on or after 1 Jly 2005: The materials sed for the lining are incapable of prodcing sparks. The materials sed for the body and its lining are made from heat and flame resistant materials at least 10 mm thick and which comply with classification B-S 3-d 2 of EN 13501-1:2002. No materials have been sed in the constrction of the body which wold be likely to form dangeros componds with the sbstances carried. I declare that in the case of a MEMU vehicle or trailer of any age: Where the constrction of the special explosive compartment incldes metal it is lined with heat and flame resistant material which comply with classification B-S 3-d 2 of EN 13501-1:2002. The following declaration will be reqired from the owner for every ADR application relating to a type EX/III or MEMU vehicle or trailer: DECLARATION - to be completed by owner / spplying dealer I declare that no repairs, modifications or painting of the body or MEMU special explosives compartment have been carried ot in sch a manner as to invalidate the manfactrer s declaration. 8

PART 8 13. COMBUSTION HEATER DECLARATION (applicable to all ADR vehicles with a Combstion Heater fitted) A declaration will be reqired at the time of first application for ADR inspection for vehicles identified above which are not sbject to ADR Type Approval or which have had the combstion heater system modified after Type Approval. DECLARATION - to be completed by vehicle or combstion heater manfactrer or installer TE 1: A separate declaration is acceptable provided it clearly relates to the vehicle. TE 2: The scope of this declaration does not inclde ADR specific control system fnctions which may be modified by others to comply with Directive 2001/56. I declare that the combstion heater and its basic installation is in accordance with the relevant technical reqirements of Directive 2001/56 as amended. Additionally for a type FL motor vehicle I declare that the heat exchanger of the combstion heater is resistant to the redced after-rnning cycle of 40 seconds for the time of its normal se. PART 9 14. DECLARATION OF STABILITY A declaration completed by the manfactrer is reqired at the time of first application for inspection for vehicles in the categories below: 3 for type FL, OX, AT tank vehicles with a capacity over 3m and with a test pressre less than 4 bar first sed after 30 Jne 2004 DECLARATION - to be completed by the tanker manfactrer I declare that the tank stability of this tank-vehicle has been assessed against the technical reqirements of ECE Reglation 111 either by physical test or calclation and complies with those reqirements. for MEMU vehicles of any capacity or for EXIII tank vehicles / trailers with a capacity over 3m DECLARATION - to be completed by the MEMU manfactrer I declare that the grond contact width of the vehicle axles exceeds 90% of the centre of gravity height of the flly laden vehicle and eqipment. 3 9

PART 10 15. DECLARATION OF SUITABLITY OF BATTERY MASTER SWITCH CONTROLS (applicable to EX/III, FL and MEMU motor vehicles first sed after 30 Jne 2005 with electrically operated battery master switch controls) A declaration completed by the manfactrer is reqired at the time of first application for inspection for type EX/III, FL and MEMU motor vehicles identified above which are not sbject to ADR type-approval or where master switch control systems have been modified after Type Approval. DECLARATION - to be completed by the vehicle manfactrer or converter I declare that the control system for the battery master switch complies with ADR section 9.2.2.5. PART 11 16. TANK REPAIRS AND MODIFICATIONS For all tank and battery vehicles, have any repairs or alterations or damage likely to affect their safety occrred to the tank or battery or their eqipment since the last ADR examination? If yes, is a Certificate of Exceptional Examination carried ot nder ADR 6.8.2.4.4. after the repairs, alteration or damage available on the VCA tank database? If applicable, which Department for Transport Appointed Inspection Body issed the Certificate of Exceptional Examination? 10

TES GENERAL 1. Applications can be made pto 93 days and not less than 10 working days before inspection is reqired. Applications mst be accompanied by valid Tank Test Certificates etc (see part 4 and note 10), and any declarations that may be reqired. 2. Separate applications shold be made for each vehicle (ie motor vehicle, trailer or semi-trailer) for which an ADR certificate is reqired. 3. Applications shold be sent to the Driver & Vehicle Standards Agency, Tass, ADR Section, Ellipse, Padley Road, Swansea SA1 8AN. The application shold be accompanied by a crossed cheqe or postal order (payable to DVSA) for the appropriate fee, or the nmber of the prepaid DVSA accont from which the fee shold be taken shold be indicated at section 5 of this form. If sing a prepaid DVSA accont please ensre the signatory in section 5 is a delegate and able to draw from the accont. If yo wish to pay by credit / debit card, please provide the card holder s name and telephone nmber at section 5 of this form. 4. If a vehicle is reqired to have a first test at the same time as the ADR inspection, the appropriate test application shold be attached to this application. 5. If a motor vehicle has not yet been registered this shold be stated against registration mark. If a trailer does not bear a chassis nmber or other permanent mark, application for one shold be made to the Driver & Vehicle Standards Agency at Swansea. If a trailer already has a Ministry Identification nmber this shold be given against trailer ID No. 6. If modifications have been carried ot to the vehicle since it was last plated by DVSA, details mst be given and, if applicable a VTG10 (Notifiable Alteration) form shold be enclosed with this application. Frther details regarding notifiable alterations can be fond at www.gov.k/government/pblications/ details-of-notifiable-alterations-or-application-for-a-change-of-plated-details-of-a-goods-motor-vehicle 7. If yo are not able to keep an appointment for a test, let the test station know as soon as possible - fees may be forfeited for late cancellation or failre to attend. 8. Frther details regarding the carriage of dangeros goods can be fond at www.gov.k/specialisttests-for-lorries/the-adr-test 9. If this is not the first application for this vehicle, a photocopy of both sides of the crrent or most recent ADR1C certificate mst be enclosed with the application. The crrent ADR1C certificate mst be presented at the time of test for validation. If the ADR1C has been lost this shold be declared in writing and retrned to DVSA if fond. 10. Drawbar and semi-trailers containing dangeros goods mst be drawn by a towing vehicle also certified to the relevant standard even if they do not themselves carry dangeros goods. The only exception to this is pre-2003 explosives carrying drawbar combinations where the towing vehicle does not need certification if the explosives are only carried on the trailer. TANK AND MEMU VEHICLES 11. a) The tank initial approval certificates shold be sent along with the application. A photocopy is acceptable; however, the original mst be prodced at time of test. If the original has any featres which are not obvios on the copy and which lead to a change having to be made to the prepared approval certificate a retest fee may be payable before the release of the new certificate. PDF versions of tank docments are now accepted electronically from approved tank inspection bodies at: adrbooking@vosa.gov.k 11

TANK AND MEMU VEHICLES (contined) b) In the case of a new tank, MEMU vehicle or battery vehicles only in exceptional cases an application may be made before the tank initial approval certificate is issed. Before an approval certificate can be prodced the tank initial approval certificate nmber mst be advised to DVSA at part 4, section 9. The original tank initial approval certificate mst be prodced at the time of test. c) Tank periodical certificates shold be sent with the application. Please provide both the crrent and preceding certificate, if available. A Copy is acceptable, however an original crrent certificate mst be prodced at the time of the test. The original TC2/3 is only reqired to be seen once at the ADR test following their isse by the Tank Inspection Body. d) For first tests or if any details have changed: tank and MEMU prodct lists and for battery vehicles (where it is not possible to list all the elements in part 1, section 2) a schedle of the battery element serial nmbers shold be spplied with the application. e) Tank Codes and any Special Provision Codes shold be reqested from the tank manfactrer or insrer to complete part 1, section 2 of this form. f) Tank-vehicles for which stability declarations are reqired bt not available may be certified for ADR by Britain nder mltilateral agreements signed by orselves and other participating contries provided proof can be frnished that the tank was hydralically tested prior to 1 Janary 2005. Certificates granted where no stability declaration has been made will be endorsed and may not be accepted by other contries nless they are also signatories to that mltilateral agreement.
