Page 2 JOB DESCRIPTION AND TITLE: STROKE NURSE COORDINATOR JOB CLASS: 1285 REPORTS TO: PERFORMANCE & ACCREDITATION MANAGER QUALIFICATIONS: Bachelors or Associate Degree in Nursing required, MSN preferred. The Stroke Nurse Coordinator must be a NC licensed Registered Nurse. Recent nursing experience with BLS certification and a minimum of two years experience in Neurologic Critical and or Emergency care setting; evidence of advanced clinical knowledge, leadership abilities, and teaching and communication skills is preferred. Previous experience in case management, stroke care and knowledge of community resources, data collection and analysis is desired. Skills include: Proficiency in computer and office equipment, oral and written communication, ability to collaborate effectively as a team leadership in a dynamic and academic health care environment; flexibility to respond quickly to change; ability to prioritize work activities to achieve goals in a timely manner; problem solving, teamwork, consensus building and negotiations skills. JOB SUMMARY: The Stroke Nurse Coordinator is responsible for organizing and administering the stroke outreach programs. This position coordinates stroke education programs to agencies in the Onslow Memorial Hospital and Onslow County referral areas. The Stroke Nurse Coordinator provides the Stroke Risk Identification Screening Program to the public; develops, implements and maintains Beyond the Hospital Programs for stroke survivors. The Coordinator assures all documentation of outreach activities as required by various agencies. The Stroke Nurse Coordinator shall work collaboratively with patients and their families, health care providers, community groups, and schools to facilitate Stroke Care and prevention throughout the continuum of care. The Stroke Nurse Coordinator will coordinate regional stroke screening outreach activities that unitizes principles of program management; advanced clinical care and leadership to effectively meet customer needs; and the goals of the grant. Providing leadership and system planning for participating hospitals; coordinating community screenings, securing screening sites, personnel, volunteers and equipment. The Stroke Nurse Coordinator collaborates with hospitals and professional organizations to facilitate the development of the stroke program: assessing stroke community screening needs; participating in meetings as appropriate to assure effective communication; demonstrates effective interpersonal communication skills; provides expertise in the area of stroke specialization; consults with health care institutions and community agencies to meet the needs of the patients, volunteers, and care providers. Stroke Nurse Coordinator shall develop and promote a standard of care consistent with decreasing inpatient resource utilization, and contribute to service excellence at the hospital and outpatient level in accordance with the goals established in the guidelines of the Kate B. Reynolds Charitable Trust, NC Stroke Association and NC Collaborative Stroke Registry and Onslow Memorial Hospital policies and procedures. The Stroke Nurse Coordinator position is grant funded and limited to up to a three-year employment.

Establishes and implements educational action plan for healthcare staff and community at large for Stroke care and prevention management. Works with community to establish support systems and identify at risk Stroke patient population. VIII. COMMUNICATION Communicates Stroke program progress and outcomes to Performance Improvement committee, NC Stroke Association & NC Stroke Registry. Communicates patient status and needs to healthcare providers and other assisting agencies. Communicates plan of care for Stroke Care to patients and families in a clear and understandable manner. IX. JOB KNOWLEDGE Establishes and maintains the structure and processes necessary to provide Stroke Care services in Onslow County through a three-pronged approach, Risk Identification, Acute care EVB standardized guidelines for practice delivery and Beyond the Hospital Program. Coordinates community activities aimed at addressing the needs of the defined patient population. Monitors and completes follow-up on all acute care outpatient and inpatient stays for the defined patient population. Provides home visits/follow-up Calls as a necessary mechanism of assessment and follow-up and as requested by any healthcare provider. Establishes and maintains effective relationships with the medical community, Community Health Improvement Project, Caring Community clinic, public health department, school, families, church groups, and other community agencies as necessary. Coordinates the Stroke Team at OMH.

Page 8 Coordinates and develops a community-wide stroke education program. Establishes and maintains a clinical information system with the capacity to identify, classify, and track the defined patient population. Through the PI Monitors, NC Collaborative Stroke Registry, NC Stoke Association and the American Heart Association s GWTG. Promotes a standard of care that initiates cost savings and decreases inpatient resource utilization. Provides patient services, utilizing advanced clinical skills, that target Stroke primary and secondary prevention through effective education throughout the stroke continuum of care to include inpatient, outpatient, rehab and additional resources services. Initiates and/or participates in program development/strategic planning. Adheres to Hospital and Department confidentiality policies. ADDENDUM: As a part of the job description of all employees of the Onslow County Hospital Authority, the employee may be directed to render services within the scope of his/her job description to an affiliate of the Onslow County Hospital Authority pursuant to an arrangement between the two entities.
