See a doctor 24/7 with LiveHealth Online In 2016, BCBSGa members can talk face-to-face with a board-certified doctor through a mobile device or computer with a webcam at livehealthonline.com for less than a visit to the doctor s office! LiveHealth Online doctors provide medical advice for common concerns like the flu, colds, allergies, and more. They can also send a prescription directly to a pharmacy. It s faster, easier, and more convenient than a visit to an urgent care center. If you re enrolled in the Comprehensive Care or BlueChoice HMO plan, your copay will be $15. If you re enrolled in the Consumer Choice HSA, this option is often more cost-effective than alternatives like the ER, urgent care, or your primary care physician. For 2016, you ll continue to see a $75 monthly surcharge on your premiums if you use tobacco. You can make changes to your Tobacco Use Status at any time, and the change will be effective the first of the following month. However, there will be no refunds of the $75 surcharge. 2

For 2016, your healthcare options will remain: Consumer Choice HSA Provided by BCBSGa, this is the only healthcare plan that provides you with access to a Health Savings Account (HSA) see more information on page 5. NEW! To comply with the Affordable Care Act, the HSA contribution limit for family coverage will increase in 2016 to $6,750, and the out-of-pocket maximum for in-network coverage will decrease to $6,850. Moving to the Consumer Choice HSA for 2016? If you re moving to the Consumer Choice HSA plan and have an FSA now, make sure your Flexible Spending Account (FSA) balance is zero by December 31, 2015. Otherwise, you cannot open an HSA until April 1, 2016. Comprehensive Care This BCBSGa plan offers coverage for both in-network and out-of-network providers. Healthcare change for retirees age 65 and older USG remains committed to providing healthcare benefits to eligible retirees. Beginning in 2016, USG s Medicare-eligible retirees and their Medicare-eligible dependents will receive healthcare coverage in a new way through the Aon Retiree Health Exchange. The exchange enables eligible participants to compare coverage options and enroll in the plans that best fit their needs. Your pharmacy coverage CVS/caremark will continue to be the pharmacy benefit manager for employees enrolled in BCBSGa healthcare plans. Your BCBSGa ID card also includes your CVS/caremark information. Sign up for convenient mail service! Disability coverage Short-term disability (STD) After you ve been disabled for 14 days, this benefit replaces 60% of your weekly salary, up to $2,500 per week. Benefits may last up to 11 weeks. Evidence of Insurability (EOI) is required if you elect this coverage during Open Enrollment. Long-term disability (LTD) After you ve been disabled for 90 days, this benefit replaces 60% of your monthly salary, up to $15,000 per month. No EOI is required. LTD premiums will increase by 15% in 2016. Do you have a medical condition? If you purchase LTD insurance, you may not be eligible for LTD benefits for 12 months if you received treatment for that condition within three months of when your disability coverage begins. Life insurance Protect your family s income in the event of a death due to illness or accident. Basic life and AD&D You ll receive life and Accidental Death & Dismemberment (AD&D) insurance equal to $25,000 at no cost to you. Supplemental life insurance You can buy additional life insurance coverage from one to eight times your salary, up to a maximum of $2.5 million. Without having to provide EOI, you can elect up to the limit of the lesser of three times your salary or $500, 000 as a newly eligible employee Dependent coverage You can choose life insurance coverage for your dependents, too: Spouse coverage options of $10,000 to $500,000. EOI is required for amounts over $50,000. Child(ren) coverage options of $5,000, $10,000 or $15,000. No EOI is required. Additional AD&D insurance You can buy additional AD&D insurance in amounts of $10,000 to $500,000. Bonus! When you purchase supplemental life or additional AD&D insurance, you ll also receive beneficiary financial counseling, legacy planning services, legal services, and travel assistance. Protect those who matter Your USG healthcare, dental, vision, life, and additional AD&D benefits cover your eligible dependents: your legal spouse; your natural, adopted, or stepchild(ren) through the end of the month of their 26th birthday; and your disabled child(ren) with proof of disability. If you are adding new dependents to any coverage, you may be required to provide documentation of your relationship or your child s age. Examples include a marriage certificate, birth certificate, adoption certificate, and income tax returns. If both you and your spouse are eligible University System of Georgia employees, only one may elect to cover the other spouse and/or dependent children.
