1 Dear Georgia Tech Retiree, Open enrollment is the one opportunity provided each year to make changes to your existing benefit coverage. The 2014 benefit open enrollment period will be held November 11, 2013 through November 22, In order to participate in this open enrollment, you must have elected to continue coverage into retirement. During this period, eligible retirees and surviving spouses may only make changes to their current medical, dental or vision coverage, including electing a different plan, adding dependents up to age 26, dropping coverage, or reducing coverage levels. This year, the University System of Georgia (USG) is taking a new approach among all of its 31 institutions and retirees. Because the USG is offering more benefit plans, there are a number of changes to the 2014 benefit plan offerings. That s why it s even more important to review your options carefully. A dedicated website is available for retirees with all information accessible online at As you review your open enrollment packet and the online resources provided, please know that the benefits team is here to guide and assist you through this process. 2 IMPORTANT INFORMATION If you do not wish to make any changes to your existing insurance coverage for 2014, you only need to complete and return the USG Tobacco Certification form. Complete the USG Tobacco Certification for you and all dependents age 19 and older who are enrolled in medical coverage. There will be a $50 per person surcharge in addition to your monthly medical premiums for any enrolled tobacco user. You may only make changes to medical, dental or vision coverage if you are currently participating in a plan. For example, you may not enroll in a dental plan if you currently do not have dental coverage. Please review your beneficiaries at the bottom of your benefits statement. If you would like to change your beneficiary listing or allocation, you will need to complete the yellow Minnesota Life Beneficiary form, which is the new provider for A new rate tier will be introduced in 2014 for dental and vision insurance. If your current coverage is retiree + one child (or more), your new tier will be retiree + child(ren), instead of the family coverage. Make sure you provide the Social Security number for all covered dependents on your election form. MEDICAL PLAN CHANGES The University System of Georgia will continue to offer the following medical plans: RATES 1. Open Access POS 2. HSA Open Access POS (The High Deductible Health Plan) 3. BlueChoice HMO 4. Kaiser HMO All Blue Cross Blue Shield medical plan premiums (Open Access POS, HSA Open Access POS and BlueChoice HMO) will stay the same in The Kaiser HMO rate will increase approximately 4%. Please refer to the enclosed booklet for the new rates. PLAN DESIGN CHANGES Open Access POS Plan o Increase emergency room copays ($50 to $150) o Increase single/family out-of-network deductibles (from $400/$1,200 to $500/$1,500) o Increase single/family out-of-network annual maximums (from $2,000/$4,000 to $3,000/$5,000) o Out-of-network wellness visits will no longer be covered; it was previously covered at 60%. o ACA* requirement Change out-of-pocket maximum rules to include copayments for office visits and emergency room visits. o Prescription drug benefits - Increase brand name copays to $35 ($5 increase) HSA Open Access POS Plan o Increase participant co-insurance from 10% to 15% o Separate out-of-network deductible from in-network deductible and increase single/family out-ofnetwork deductible from $1,500/$3,000 to $2,500/$5,000 o Add prescription mail order option 2
3 BlueChoice HMO Plan o Increase Primary Care Physician (PCP) copay to $20 ($5 increase) o Increase Specialist copay to $25 ($10 increase) o Add prescription mail order option o ACA* requirement Add out of pocket maximum of $6,350/$12,700 Kaiser HMO Plan o Increase PCP copay to $20 ($5 increase) o Increase Specialist copay to $25 ($10 increase) o ACA* requirement Add out of pocket maximum of $6,350/$12,700 o Add two pricing tiers (Preferred and Non-Preferred) for both generic and brand prescription drugs Kaiser Senior Advantage o Senior Advantage Premium is decreasing due to copay increases o Increase PCP copay to $20 ($5 increase) o Increase Specialist copay to $25 ($10 increase) o Physical therapy visit will increase to $25 ($10 increase) o In-patient hospital stay will increase to $250 *Affordable Care Act NOTE: Tobacco Surcharge If you are enrolled in a USG medical plan, you must complete the enclosed USG Tobacco Certification form for you and all dependents age 19 and older enrolled in your medical coverage. There will be a $50 per person surcharge in addition to your monthly medical premiums for any enrolled tobacco user. NOTE: Default Coverage Coverage will default to the same plan if no changes are made during open enrollment. MEDICARE MEDICARE CHANGES IMPORTANT: If you and/or your spouse will become Medicare eligible, age 65 after January 1, 2014, you are required to enroll in Medicare Part B during your eligibility period designated by the Social Security Administration (SSA). In 2014, USG medical plans will be Secondary Payer only. Failure to enroll in Medicare B during your eligibility period will result in you paying the total cost (USG/GT rate + retiree rate) for medical insurance. The total cost of medical insurance can be found on pages 5-6 in the enclosed 2014 Health Care Basics BoR Comparison Guide. Current retirees who only have Medicare A, will remain grandfathered in paying their current level of medical insurance premiums. Once you obtain your Medicare A & B card(s), you must submit a copy to the GT Benefits Department using the contact information below: o Mail to Georgia Tech Benefits, 500 Tech Parkway, Atlanta, GA o Or, fax to
4 MEDICARE AND HMOs For Medicare eligible retirees and/or dependents to qualify for the Kaiser Senior Advantage HMO Plan, you must have Medicare Parts A and B and must assign coverage to Kaiser. The Kaiser Senior Advantage HMO product will serve as the member s only health care plan. There will not be secondary benefits from Medicare. If you fail to qualify for participation in the Kaiser Senior Advantage Plan, you will be required to enroll in the Open Access POS Plan or the HSA Open Access POS Plan. If you are covered by the BlueChoice HMO plan, you must choose another plan when you become eligible for Medicare. The BlueChoice HMO does not offer a Medicare-eligible retiree health care plan. OPEN ACCESS POS RETIREE PARTICIPANTS ONLY The USG created the Express Scripts Medicare Part D plan for Medicare-eligible participants in the Open Access POS plan. Retirees in the Open Access POS plan will be automatically enrolled in the Express Scripts Medicare Part D plan. If you do not want to enroll in the Express Scripts Medicare Part D plan through the Open Access POS, you must either: (1) choose another USG health plan or (2) drop health insurance coverage. Please note, if you drop health coverage through the USG, you will NOT be allowed to reenroll at a later date. The Express Scripts Medicare Part D customer service phone number is DENTAL PLAN CHANGES The BoR MetLife and GT Delta Dental PPO plans will no longer be offered. The USG will provide two new dental plans through Delta Dental, which are comparable to the current 2013 MetLife and Delta Dental PPO plans. These two new plans will use the Delta Dental PPO network. o Delta Dental Base Plan o Delta Dental High Plan If you do not make any changes to your dental election and you re enrolled in the two plans that are going away, you will be defaulted to the most comparable plan which is the Delta Dental High Plan in The current GT DeltaCareUSA HMO plan will continue to be offered but referred to as Delta Dental HMO in 2014 (using the DeltaCare USA network). 4
5 LIFE INSURANCE CHANGES Minnesota Life will be the new vendor for the Basic, AD&D, and Supplemental life insurance plans. BoR CIGNA will no longer be offered. The rates will decrease under the new plans for Current plan participants should note the following: o Your current CIGNA Basic and/or Supplemental Life amounts will be grandfathered under Minnesota Life plan respectively. o BoR Retiree policy remains the same Hired before 11/1/80 = Calculation and AD&D drops at age 70 Hired after 11/1/80 = Maximum of $15,000 in coverage and no AD&D o The CIGNA Dependent Life amounts will be grandfathered. Dependents enrolled in this coverage will be in new, separate plans. Spouse life is $5,000/2.35 per month Child life is $5,000/0.50 per month NOTE: REVIEW YOUR BENEFICIARIES Due to the change in life insurance carrier, you will need to review your beneficiaries and allocation amounts. Please complete the enclosed Minnesota Life Beneficiary Form to make updates and submit to the Benefits Department for processing. VISION The USG will provide vision insurance through EyeMed, the current provider. There will be some plan changes, such as enhanced frame benefits and new lasik discounts. The monthly premium will decrease. 5
