Definitions As used in this list: * Targeted Case Management is a type of service a person may receive when seeking mental health treatment. Working together, our staff and the individuals they serve, will identify resources in the community that will be helpful in promoting recovery. Individuals are then linked to these additional resources. ** Mental Health Services are provided for people with severe emotional and mental distress. They may include group therapy, individual therapy, and targeted case management. All services are geared toward improving one's quality of life and experiencing recovery. *** Psychiatric Inpatient Hospitals are facilities that specialize in the 24 hour care and treatment of people greatly effected by a serious mental disorder. **** Cultural Specific Services are services that are offered in languages other than English or that are designed to address cultural differences. Initial screenings and assessments are provided to adults and seniors. Minors seeking non-crisi mental health resources will be given information about how to access services through their geographic provider of children s mental health services. The Assessment Center provides assistance with community services and referrals to determine proper service placement. The center includes specialized assessment, and referral for substance abuse as well as **mental health services. Life Skills Groups focusing on establishing basic skills for emotional regulation and interpersonal effectiveness, as well as depression recovery groups are offered on a weekly basis for anyone wishing to participate. clients from the following cultural groups: veterans, older adults, Hispanic American, African-American, LGBTQ, women, gang members, faith traditions. KCMH Northeast 3715 Columbus 661-868-7199 8 a.m. - 5 p.m. Brief Therapy Clinic Ave. Bakersfield, CA 93306 Spanish languages Adult Services The Northeast Brief Therapy Clinic (NBTC) is an ACA team whose primary goal is to provide brief, recovery-oriented therapy. Case management and medication management is provided on a limited basis, as needed basis. The NBTC serves adult clients by assisting them with moderating their mental health symptoms and impairments, then re turning to the community supports for continued services. The client s natural strengths and resources will be explored and expanded and those who do require medication management will be referred back to their primary providers after reducing the impairments and symptoms that necessitated specialty mental health services. Specific services include brief recovery oriented assessments, nursing assessments, brief solution-focused therapy and case management, skill-building groups, and medication management. clients from the following cultural groups: veterans, blind/visually impaired, hearing impaired, older adults, Native Americans, Hispanic, African American, LGBTQ, women, gang members, and faith based services. KCMH Crisis Case 3715 Columbus 661-868-7151 8 a.m. - 5 p.m. Management Ave. Outreach, Adult Bakersfield, CA Wraparound, Freise 93306 8 a.m. 12 pm Saturday & Hope House Sunday Liaisons (Freise only) Spanish Adult Services, Wraparound Services Crisis Case Management Outreach (CCMO) is the brief intermediary between initial crisis contact and appropriate KCMH and community services. Individuals served are new to the KCMH System Of Care (SOC). Referrals are received from inpatient psychiatric units. Services include screening, discharge planning, intensive *targeted case management, and linkage. Adult Wraparound assists KCMH SOC providers to discharge individuals from inpatient units, and provide brief intensive outpatient treatment to augment that of existing KCMH teams. Services are provided in the greater Bakersfield area. The Freise Hope House Liaisons provides screening and linkage to the Freise Hope House. Services are provided for individuals living at the facility, and 30 days post discharge from the facility. Services include screening, hospital discharge planning, intensive *targeted case management, and community linkage. Referrals are received from inpatient psychiatric units, the psychiatric evaluation center and KCMH outpatient teams. CCMO, AWA, and the Freise Hope House Liaisons have bilingual (Spanish speaking) staff available. Services are provided in Spanish for monolingual and bi-lingual Spanish-speaking individuals. clients from the following cultural groups: Asian Pacific Islander, Hispanic, African American, LGBTQ, women, veterans and faith based services.
