Subsequent to the production of the CAHPS for PQRS Survey (QAG V.1), it has been determined that there are specific survey material items that require content corrections. These items are identified below. This document was updated on November 20 th to add the first three items below and the item regarding revision of the Russian CATI Script to include Q57 Intro. 1. Section III, Program Requirements Survey Vendor Roles and Responsibilities, Page 13: A sentence was added at the end of the first Note to further clarify the cell size rule for reporting. The sentence reads: These instructions prohibit display counts of 1-10 or any numbers that allow the exact inference of a count of 1-10. 2. Section IV, Data Collection Protocol Phone Attempts, Page 32: A Note was added after the second paragraph to clarify call back attempts. The Note Reads: Note: It is permitted to call the beneficiary back one time after the sixth attempt if the beneficiary establishes a firm call-back date and time. The interviewer must establish the call-back with the beneficiary and not a member of the household. 3. Section IX, Data Analysis and Public Reporting Survey Vendor Analysis of CAHPS for PQRS Survey Data, Page 54: A Note was added at the end of the section to further clarify the cell size rule for reporting. 4. Revision to Appendices Revision to Appendix B, Survey Vendor Authorization Form, to include providing TIN if Group ID is not available. Clarification to Appendix J-1, Beneficiary Response Section, to provide additional information on how to code Q78d and Q78e for returned mail surveys Question 78d and 78e are not available as response options in the mail survey. When submitting data for a returned mail survey, code responses for questions 78d and 78e as M-Missing. NOTE: All other response options for the race question are to be coded in accordance with the protocols for multi-mark questions provided on page 38 of the CAHPS for PQRS Survey. The valid values for Q78d and Q78e are updated to include an asterisk and a corresponding note to address this clarification (please note that there are no revisions to the valid values to Q78d1 78d7 and Q78e1-78e4 and those items are not displayed in the table below). Revision to English Mail Survey Instructions The Example Q3 on the PQRS survey instructions that uses circles instead of check-boxes should have the Yes response option selected instead of the No response.
