Yes STATEMENT OF INTEREST As a member of the CAC for several years now, I have a much better understanding of the community need that is being filled by REACH. In today's world, the gap between the "have's" and the "have not's" is growing at a rapid pace; the access to healthcare services has not been ignored by this growing trend. We are fortunate to have a resource such as REACH in our community. The fact that this organization focuses on mental health care as well as dental care and safety net sets it apart and further demonstrates REACH's commitment to making a difference In the counties served, especially as funding sources for these services are challenged. My career in the financial services industry has provided me with a solid background in business, finance, and investment principles. As a line of business leader at UMB, I am tasked with executing the strategic plan, driving financial results, improving associate engagement, and increasing client satisfaction. Additionally, I have served on non-profit boards as a voting member and in leadership roles. These opportunities have taught me the importance of collaboration as well as governance. It would be an honor to serve as a member of the REACH board of directors. ELECTED OFFICIAL STATUS: [radio-elected] If yes, please explain: CONFLICT OF INTEREST? No BACKGROUND CHECK: Convicted Felon? Employment Status: Employed Full Time Employer: Children International Job Title: Vice President of Finance Employer Address: 2000 East Red Bridge Road Kansas City, MO 64131 Employer Phone: 816-942-2000 May we call this employer as a reference? Yes STATEMENT OF INTEREST I wish to submit for your consideration my interest in serving as a member on the Board of Directors for the REACH Healthcare Foundation. For the past 15 years I have worked for Children International. I was the Controller for a majority of that time until recently when I was promoted to act as their VP of Finance. Children International provides health, dental, educational and other basic assistance to nearly 350,000 needy children living in 11 different countries worldwide. I have visited some of these children and feel compelled to help not only those children abroad, but those in our backyard as well. I have seen first-hand the impact poverty has on children and their families as well as the power the smallest of benefits can provide. I know that when I go to work each day that I am helping, albeit indirectly, a poor child overcome the bonds of poverty by eliminating financial barriers to better health and education. My family and I live in the Brookside area and I want to help change the lives of people living in the Kansas City area, as well. Currently I participate in a monthly reading program at the Children s Place, located at 59th and Main Street here in Kansas City. During that time I sit down and read one-on-one with a child that has been referred to the facility by a doctor to receive developmental and emotional support that they need. It is a truly rewarding to know that I am sharing in an experience that the children may otherwise not have outside of their time at the facility.

Name: Danielle Mitchell As part of my responsibilities as VP of Finance at Children International, I am responsible for preparing and presenting financial policies and strategies, budgets and results to our Board of Directors. I am very familiar with and enjoy that interaction with our Board members who are also members of the Kansas City community who have taken strong interest in our mission. I would like to support the REACH Healthcare Foundation in a similar manner visiting the program locations, increasing my understanding of the problems and lending my experience to aid in a solution to better the lives of the underserved in our community. I ask that you please consider me as a candidate to fill one of your vacant Director positions. ELECTED OFFICIAL STATUS: [radio-elected] If yes, please explain: CONFLICT OF INTEREST?
