REGULATORY REFORM (FIRE SAFETY) ORDER 2005 FIRE RISK ASSESSMENT TEMPLATE GUIDANCE This guidance dcument has been prduced t assist the Respnsible Persn t cmplete a Fire Risk Assessment using the template prvided by Glucestershire Fire & Rescue Service. The template has been prvided t assist the Respnsible Persn with the cmpletin f a Fire Risk Assessment fr their premises. Glucestershire Fire & Rescue Service accept n respnsibility fr the suitability r sufficiency f the added cntent f the cmpleted dcument. In rder t prduce a suitable and sufficient risk assessment fr yur premises, it shuld be used in cnjunctin with a series f guide bks published by the Department fr Cmmunities and Lcal Gvernment (CLG). These bks prvide additinal infrmatin (type f premises specific) in rder t assist thse preparing fire risk assessments, giving detailed infrmatin n risk assessments and ther issues. Regulatry Refrm (Fire Safety) Order 2005 The Regulatry Refrm (Fire Safety) Order 2005 became law in Octber 2006 and intrduced significant change t wrkplace fire safety respnsibilities. As well as simplifying the legislative regime by bringing all fire safety legislatin tgether int ne Order, it intrduced the need fr emplyers, building wners and ccupiers as 'respnsible persns' t carry ut, implement and maintain a fire safety risk assessment. All nn-dmestic premises, including the cmmn r shared parts f blcks f flats r huses in multiple ccupatin are cvered by the Order, and may be inspected by their lcal Fire and Rescue Authrity. Under the Order, Fire and Rescue Authrities have a statutry duty t ensure cmpliance and enfrce the requirements where necessary. What are the main requirements f the Order? The respnsible persn is required t: Carry ut a fire risk assessment identifying the risks and hazards. Cnsider wh may be especially at risk. Eliminate r reduce the risk frm fire as far as is reasnably practical and prvide general fire precautins t deal with any residual risk. Take additinal measures t ensure fire safety where flammable r explsive materials are used r stred. Create a plan t deal with any emergency and, in mst cases, dcument yur findings. Review the findings as necessary In carrying ut a risk assessment the respnsible persn may decide that given the nature f the premises r the peple invlved, they may need t appint ne r mre cmpetent persns t assist them. The level f necessary cmpetence is nt prescribed in the Order. It recgnises that the extent f cmpetency will vary accrding t the nature and cmplexity f the premises invlved. The Fire Safety Order requires that yu give effect t arrangements fr the planning, rganisatin, cntrl, mnitring and review f the preventive and prtective measures. These are the measures that have been identified as a cnsequence f a risk assessment as the general fire precautins yu need t take t cmply with the Fire Safety Order. Yu must recrd the abve arrangements if: Yu emply five r mre emplyees in yur undertaking (regardless f where they are emplyed); A licence under ther legislatin is in frce; r An alteratins ntice is in frce requiring a recrd t be kept.

Where des it apply? The Order applies t virtually all premises and cvers nearly every type f building, structure and pen space. Fr example: Offices and shps Premises that prvide care Cmmunity halls The cmmn areas f huses in multiple ccupatin including cmmn fire warning systems etc. Pubs, clubs and restaurants Schls Tents and marquees Htels, B&Bs, guest huses, hstels and self catering accmmdatin Factries and warehuses Stables What des this mean fr me? The main effect f the changes is a mve twards greater emphasis n fire preventin in all nn-dmestic premises, including the vluntary sectr and self-emplyed peple with premises separate frm their hmes. Respnsibility fr cmplying with the Fire Safety Order rests with the 'respnsible persn'. In a wrkplace, this is nrmally the emplyer r any ther persn wh may have cntrl f any part f the premises, e.g. the ccupier r wner. If there is mre than ne respnsible persn in any type f premises, they must take all reasnable steps t cnsult with ne anther regarding matters f fire safety. If yu are the respnsible persn yu must carry ut a fire risk assessment which must fcus n the safety in case f fire f all 'relevant persns'. It shuld pay particular attentin t thse at special risk, such as the disabled and thse with special needs, and must include cnsideratin f any dangerus substance likely t be n the premises. The fire risk assessment will help yu identify risks that can be remved r reduced and t decide the nature and extent f the general fire precautins yu need t take, t prtect peple against the fire risks that remain. The fire risk assessment shuld cnsider dangerus substances that are used r stred in yur premises, nly t the extent necessary t determine the adequacy f the general fire precautins (as defined in Article 4 f the Fire Safety Order) and t advise yu accrdingly. If dangerus substances are used r stred in yur premises, yu shuld ensure that a risk assessment f the relevant wrk activities has been carried ut t enable yu t cmply with the Dangerus Substances and Explsive Atmspheres Regulatins 2002. This fire risk assessment must cnsider special, technical r rganisatinal measures that are required t be taken r bserved in cnnectin with the use r strage f any dangerus substance.

GLOUCESTERSHIRE FIRE & RESCUE SERVICE FIRE RISK ASSESSMENT TEMPLATE The Glucestershire Fire & Rescue Service Fire Risk Assessment Template is divided int sectins as fllws: Frnt page General Infrmatin Identified Fire Hazards and Primary Cntrl Measures Arrangements fr Evacuatin Prvisin f Elements f Fire Safety as Secndary Cntrl Measures Management f Fire Safety Remedy Actin Plan The Respnsible Persn must ensure that each sectin is fully cmpleted with additinal details included where necessary. The lcatin f any further written infrmatin such as General Evacuatin Plans, Persnal Emergency Evacuatin Plans, Persnal Staff Training Recrds, Testing and Maintenance Recrds fr alarms/lights etc, shuld be cntained within the Fire Risk Assessment. Frnt page This sectin gives infrmatin as t wh is the designated Respnsible Persn fr the premises under the Regulatry Refrm (Fire Safety) Order 2005 and infrmatin regarding wh carried ut the assessment, the date f cmpletin and any subsequent reviews. General Infrmatin This sectin gives basic infrmatin abut the premises itself (Number f flrs, size, cnstructin and usage). Include details and numbers f ccupants especially at risk frm fire (i.e. sleeping, disabled, lne wrkers, yung persns, elderly, infirm etc). The fire safety measures required within yur premises will be whlly determined by thse likely t ccupy it!

Identified Fire Hazards and Primary Cntrl Measures It is imprtant that each sectin is fully cmpleted, including the identified hazards, existing cntrl measures, and if any additinal cntrl measures are subsequently required. CLG guidance bks will assist in the identificatin f mst generic hazards and cntrl measures which may be relevant t yur premises. An example is as fllws: ELECTRICAL SOURCES OF IGNITION Fixed installatin peridically inspected and tested? (Every 5 years) Prtable appliance testing carried ut n a risk assessed basis? Suitable plicy in place regarding the use f persnal electrical appliances? Suitable limitatin and management f trailing leads and adaptrs? Arrangements fr Evacuatin The determined strategy f evacuatin within yur premises is paramunt t the safety f all ccupants. Any evacuatin strategy must be carried ut withut Fire Service interventin. If Persnal Emergency Evacuatin Plans (PEEPS) are in use, then details f these shuld als be made reference t. Prvisin f Elements f Fire Safety as Secndary Cntrl Measures Primary Cntrl Measures are taken t prevent, as far as reasnably practical, a fire frm starting. Secndary Cntrl Measures are taken t ensure that if a fire des start; all ccupants are alerted and evacuated quickly and safely as pssible. It is therefre imprtant that yu have suitable: Fire warning and detectin; Means f escape (design f, travel distances, number f exits, etc) Cmpartmentatin (fire resisting walls, glazing and drs); Emergency lighting; Signage; Fire extinguishers CLG guidance bks prvide mre detailed infrmatin as t the minimum required standards. Fire warning and detectin systems are installed t British Standard 5839. Sme examples f grading are as fllws: Type L5: Life safety generally when specific fire engineering slutins r where PI insurance is required. Type L4: Life safety system, same as a manual system plus smke detectin n escape rute. Type L3: Life safety system, same as a manual system plus smke detectin n escape rute and heat r smke detectin in adjacent rms. Type L2: Life safety system, same as L3 but additinal detectin prvided in fire hazard/risk f ignitin areas (i.e. kitchens, sleeping areas and ther specified areas.) Type L1: Life safety system, detectin prvided in all areas, including vids and unused spaces. Fire Inspecting Officers may require t see any relevant certificates required fr a fire alarm installatin, these are as fllws: Installatin Certificate Cmmissining Certificate Alteratin Certificate (if apprpriate) Test Certificate Management f Fire Safety This sectin determines respnsibility fr different elements f fire safety within the premises. It als shuld include infrmatin regarding initial and remedial fire safety training prvisin fr members f staff and infrmatin prvided t external cntractrs, testing and maintenance regimes fr all fire safety measures and subsequent recrding requirements. Remedy Actin Plan This sectin shuld be used t make a written recrd f any additinal actin/cntrl measures identified when carrying ut the initial r subsequent review f yur Fire Risk Assessment. (i.e. things t d list!) Remedial actin shuld be priritised accrdingly and remedied as necessary. Please nte, Fire Inspecting Officers fcus n this sectin t determine whether any issues they may identify during any subsequent fire safety audit, have previusly been identified during the riginal risk assessment and suitable steps taken t remedy by the Respnsible Persn.
