2 INTRODUCTION ThisoneyearMSccourseinPsychologicalTherapyinPrimaryCare(Adults),commencingin January2016,willberunjointlybytheUniversitiesofStirlingandDundee,inpartnershipwithNHS EducationforScotlandandanumberofNHSPsychologyServicesacrossScotland.Oncompletion ofthecourseandfollowingagreedandappropriatesupervisionandcontinuingprofessional development,thetrainees,ofwhomtherewillbeananticipatedintakeof21inthe2016intakeyear, willbecompetentintheassessmentandtreatmentofadultpatientssufferingfromarangeof commonmentalhealthdisorderstypicallypresentinginprimarycaresettings.thereafteritis expectedthattraineeswillcontinueinsupervisedclinicalpracticeinordertoconsolidateanddevelop furthertheirclinicalskills. The12monthcoursehasastrong,butnotexclusive,emphasisoncognitivebehaviouraltherapyand ontheapplicationofscientificmethodstohealthcareandassessment.traineeswillbeemployed concurrentlybylocalnhsscotlandpsychologyservices,whereallclinicalplacementsinvolving superviseddirectpatientcontactwillbeundertaken. PARTNERS UniversityofDundee PsychiatryandBehaviouralSciencesisasectionoftheCentreforNeuroscience,Divisionof PsychologicalMedicine,withintheCollegeofMedicine,DentistryandNursingbasedonthe NinewellsHospitalcampus.Itreceiveda5ratinginthelastResearchAssessmentExercise.The researchactivitiesofthesectionexploredifferentaspectsoftheclinicalandbehavioural neurosciencesofpsychiatricandpsychologicaldisorderwithinthreestrategicthemes:affective Disorders,AddictionandDevelopmentandAgeing.Withineachofthesebroadthemes,Sectionstaff deliverresearch,educationandtrainingthatspansthebasicsciencesincludingsocial,psychological, geneticandneurobiologicalperspectives.thesectioncurrentlyhas13academicstaff.aclinical, researchandteachinggroupwithinaffectivedisordersisfocusedoncognitivebehaviouraltherapy (CBT).ThisgrouphasasignificanttrackrecordofteachingclinicalskillsinCBTandofconducting clinicalresearchontheefficacyofpsychologicaltherapyforanxietydisordersand,morerecently, depressionandpsychosis.thegroupisresponsiblefortheuniversityofdundee spostgraduate trainingcoursesincognitivebehaviouralpsychotherapyformentalhealthprofessionals. BoththeSectionandtheCBTgroupplaceastrongemphasisonclinicalresearchandteaching collaborationacrossprofessionaldisciplinesandacompetencywbasedapproachtoteachingclinical skills.theresearchactivitiesofthecbtgroupcurrentlyfocusonevaluatingtheeffectivenessof psychologicaltherapyforchronicdepression. FollowingconsultationwithNHSstakeholdersNEShassupportedthedevelopment ofanewroleforpsychologygraduatesinnhsscotlandasclinicalassociatesinapplied Psychology.TheMastersleveltrainingforthesenewrolesisdesignedtoequippsychology graduateswiththecompetencerequiredtodelivertheevidencewbasedpsychologicalinterventions requiredincircumscribedareasofpracticedefinedbyserviceneed.thedeliveryoftraininginvolves apartnership,brokeredbynes,betweentheuniversitiesprovidingtheacademiccomponentsofthe trainingprogrammesandthenhswhichsupportstraineesinsupervisedclinicalpracticeinthe workplace. TheMScinPsychologicalTherapyinPrimaryCarewasthefirstsuchprogramme,introducedin 2005.NES scommitmenttothistrainingroutecontinuesinto2015(2016intakeyear)withupto21 fundedplaceswithtraineesbeingsupportedhealthboardsacrossscotland. NEScommissionstrainingplacesfromtheUniversityonbehalfofNHSScotlandandsupportsthe educationalcostsi.e.feesandtravelcostsassociatedwithaccesstotraining.traineesare employedbylocalnhsboardsandmanageddirectlybypsychologyserviceswithinthoseboard areas.nesputsagreementsinplacewithemployingboardstounderpinfundingarrangementsfor trainees salariesandtheresponsibilitiesoftheemployerinrelationtotraining.thiscommitment fromnesinpartnershipwithnhsboardsandtheuniversitiesofstirlinganddundeeisasignalof thevaluethatthesetraineesareseenasbringingtothenhsservices. InScotland,ajointinitiativebetweenScottishExecutiveHealthDepartment,NHSEducationfor ScotlandandtheQualityAssuranceAgencyforHigherEducationinScotlandhasproduceda SubjectBenchmarkStatement,whichdescribesthecharacteristicsandstandardsexpectedbothof theclinicalpsychologydoctorateandmastersleveltraininginappliedpsychology. ScottishGovernmentpolicyisdrivingredesigninmentalhealthservicestodeliveritstargetsand commitmentsformentalhealth,includingimprovedaccesstoevidencebasedpsychological therapies.thisisamultilevel,multidisciplinaryandmultiagencytask.thismscprogrammeis designedtoequipgraduatepsychologistswiththeknowledgeandskillstogetstartedassafeand effectivepractitionersinthatchangingservicecontext.thereareopportunitiesandchallengesin managingchange realopportunitiesforpsychologiststofulfilnewrolesofservicedeliveryand challengesinembeddingnewrolesinachangingworkforce.wehaverecentlyintroducedanolder

4 adulttrainingpathwaywhichwouldincludetraineesclinicalexperiencebeingweightedtowards adultswhoareover65. Thereisnoguaranteeofemploymentfollowingtraining.Howeverthemajorityofgraduateshave succeededinfindingrelevantjobsincludingclinicalassociatesandprimarymentalhealthworkers aswellasavarietyofotherroles. NEShasakeyroleinworkingwithallthekeystakeholderstoensureeducationremainsalignedto serviceandworkforceplanning i.e.toensurethatthisprogrammedeliversthelearningoutcomes thatitsaysitdoesandthatwe(employers,trainingprovidersandnes)worktogethertosupport transitionfromcoursetopracticeandsubsequentroledevelopment. Fromthestartthedrivingforcebehindthecoursehasbeenunmetclinicalneed. Commonmentalhealthproblemssuchasanxietyanddepressivedisordersare disabling,consumeagreatdealofhealthcareresources,mainlyinprimarycare,andtendtorecur. Todelivereffectiveclinicalservicesfortheseproblemswemusttrainmanymorepsychological therapiststhancurrentlyexistand,mostimportantly,wemusttrainpractitionersinhowtodelivera flexiblerepertoireofevidencewbasedtreatmentsthatareintegratedwithinthemultiwdisciplinary contextofmodernprimarycare. Thecourseteamhasextensiveclinicalexperienceofworkinginprimarycare,considerableexpertise inbothclinicaltrainingandresearchand,aboveall,acommitmenttoworkwithstudentstodeliver thebestpossiblequalityofcare. 5 HEALTHBOARDS NHSBorders PsychologicalServices:ServicestoAdultsWithMentalHealthProblems Thepsychologyserviceforadultswithmentalhealthproblemsisintegratedwithincombinedprimary andsecondarycarementalhealthteams.weseepeopleagedbetween18and70,withthevast majorityofreferralscomingfromgps.thesepresentawidevarietyofpsychologicalproblems rangingfromanxiety,depression,ocdandotherselfwcontrolproblemstothesequelaeoftrauma andabuse,psychosomaticdisordersanddifficultiesinadjustment. Thetherapeuticorientationoftheserviceismixed,bringingtogetheravarietyofapproaches includingcognitive,behaviouralandinterpersonalmodels.ouradultpsychologyestablishment comprises4.5wteclinicalpsychologists,3.0wteclinicalassociatesinappliedpsychologyanda 0.3wtecounsellor.Wehave1MScCAAPtraineeand3DClinPsychtraineesandprideourselvesin providingahighquality,variedtrainingexperience.thedepartmentvaluestheroleofcaapsand hasplayedaleadingroleindevelopinginnovativepostsforgraduatesofthemsc.currentlywehave 4CAAPsworkingwithintheservice,withoneworkinginanAddictionsService.Weofferexcellent opportunitiesforcaapstoplayakeyroleinthepromotionandpreventionofmentalhealthproblems inthewiderpopulationbcurrentlywehavecaapsworkingonanumberofprojectswithcolleaguesin thelocalhealthimprovementteamincludingabumptobabyevent,amen smentalhealthevent, andmatchfit.inaddition,caapsareresponsiblefordeliveringstressmanagementclassesandare currentlydevelopinginformationsessionsonarangeoftopicsincludinganxietymanagement, keepingwellinlaterlifeandocdwhicharebeingdeliveredinlocalcommunitysettings.caapsalso havemonthlypeersupportmeetingswherecasesandresearchpapersarediscussed. In addition, opportunities are available for specialist placements during training. There are also six CAMHS teams across Lanarkshire. All other services are hosted withinasinglelocality,butprovideapanwlanarkshireservice. 5.CLINICALACTIVITIESOFTHEAREAPSYCHOLOGYSERVICES 5.1AdultPsychologicalTherapies EachofthetenPsychologicalTherapyTeamsteamhasarangeofstaffemployedwithaConsultant Clinical/CounsellingPsychologistasTeamCoWordinator/Manager.Referralsarereceivedforclients across Tiers, and staff employ a variety of individual and group therapeutic approaches including CognitiveBehaviouralTherapy,InterpersonalTherapy,SchemaFocussedTherapy,Psychodynamic Psychotherapy, Behavioural Family Therapy and Person Centred Counselling. The PTTs work in close association with the CMHTs, and are active in training and supervising clinicians across the Services. Further specialist services are also provided in Psychiatric Rehabilitation, and Clinical HealthPsychology. 5.2PsychologicalTherapiesforAdultswithLearningDisabilities The department provides community based clinical input and psychological services to two multiw disciplinarycommunitylearningdisabilityhealthteamsandaninpatientassessmentandtreatment unit.thedepartmenthasamajorroleinprovidingautisticspectrumdisorderservicesandforensic services,inbothcommunityandinpatientsettings.staffundertaketrainingandconsultancywitha varietyofpublicsectorandvolunteerorganisations.thelearningdisabilityservice scentralbase housesavarietyofspecialistsincludingspeechandlanguagetherapy,psychiatry,physiotherapyand occupationaltherapy providingampleopportunityforcollaborativeworkingwithpersonsfromother disciplines.additionallythereareanumberofclinicallyrelevantresearchprojectsbeingcarriedout inthedepartment,includinginvolvementinanrctforemdrwithadultswithldandanopentrial ofuseofbftwithfamilieswherethereisapersonwithldusingthespecialistservices. 5.3AddictionPsychologyService TheAddictionsPsychologyService(APS)isaLanarkshireWwideservice.BasesareinCoatbridge, Hamilton,andMotherwell,withstafflinkedtolocalityaddictionteamsacrossLanarkshire. The APS has developed a model of service based on direct client contact, consultation, training, supervision, and support. A range of psychotherapeutic approaches are utilised, including CBT, psychodynamic, Gestalt, and solutionwfocused therapies. Much of the APS is communitywbased, although inwpatient contact is provided as appropriate. Referrals are managed via a steppedwcare system, allowing a threewlevel approach to assessment and treatment.there are a number of specialist posts in the Addiction Psychology Service including neuropsychology, and BBV. 5.4PsychologicalTherapiesforOlderPeople Psychological Therapies for Older People (PTOP) is a multiwprofessional areawwide service, with clinicalpsychologists,caaps,andseniorchargenursepsychologicaltherapists.ptopprovidesa service to ten Community Mental Health Teams for Older People, as well as to GPs, organic and functional inpatient units, day services, memory clinics and nursing/residential homes. The service alsoprovidesasubstantialamountofstafftrainingtootherprofessionalsandtopsychologistsfrom otherdepartmentswithinnhslanarkshire.placementsareofferedtotheedinburghandglasgow trainingcoursesforbothcoreolderpeople splacementsandmorespecialistadvancedplacements, includinginyoungonsetdementia. 5.5Tier3EatingDisorderSpecialistService(TESS).TESSisamultiWdisciplinaryteambasedat the Buchanan Centre, Coatbridge, providing a panwlanarkshire to individuals with severe eating disordersaged16w64years.managedbyaconsultantclinicalpsychologist,thereareopportunities forindividualcasework,multiwprofessionalworking,groupinterventions,workingwithcarers/families inadditiontoteaching/trainingotherprofessionals. 5.6Child,Adolescent,andFamilyServices(CAMHs) AllpsychologistsworkingwithinChildandFamilyServicesarebasedwithinmultiWdisciplinaryteams withtheexceptionofthepaediatricpsychologyteam.therearesixgenerictier3camhsteams, eachprovidingservicestoaspecificgeographicalarea:cumbernauld/airdrie,coatbridge/bellshill, Motherwell/Wishaw,Hamilton,ClydesdaleandEastKilbride.TherearealsofourspecialistCAMHs servicesofferingplacements:primarymentalhealthteam,learningdisabilitiescamhsteam,cayp (CAMHsforAccommodatedYoungPeople),andPaediatricPsychologyandNeuropsychology.MultiW disciplinary teams generally consist of Clinical Psychologists, Child and Adolescent Psychiatrists, ChildandAdolescentMentalHealthCliniciansandSocialWorkers.ClinicalPsychologistsworkingin CAMHs meet together regularly for professional management, CPD and Research and Audit meetings. 5.7ForensicMentalHealthPsychologyService TheserviceisbasedinHamiltonbutcomprisesamixtureofinpatient(lowWsecure)andcommunity working across Lanarkshire. Primarily, patients have a severe and enduring mental illness and usually a forensic history. The psychology service provides input as part of a multidisciplinary approach to care and treatment. Therapeutic work includes groupwbased interventions as well as formulationwdrivenindividualworkthatmightincludeinterventionsforpsychosis,personalitydisorder oroffencewfocussedwork.forensicriskassessmentisanotherkeyaspectofourroleandclinicians are trained and supervise others in the use of structured risk assessment tools (e.g HCRW20 and RSVP).Thereareopportunitiestoprovideteachingandtrainingtootherprofessionals/agencies,and to offer consultancy and supervision to others delivering lowwintensity psychological interventions. The service continues to evolve in line with national developments providing scope for trainee involvementinservicedevelopmentatalocallevel.theservicehasgoodlinkswithotherforensic mentalhealthservicesthroughoutscotland. 5.8ManagedClinicalNetworkforStroke Aspecialistteam,basedinCoatbridge,offersassessmentandrehabilitationservices 5.9ChronicPainTeam Based at Wishaw General Hospital, clinics are offered in the Buchanan Centre, Coatbridge. The serviceoffersassessmentandrehabilitationincludingagroupbasedpainmanagementprogramme. 5.10EVA(EndViolenceandAbuseagainstWomen) Based in Coatbridge, but working across Lanarkshire, this is a service offering training to other services and agencies, as well as offering information, advocacy, counselling and psychological assessmentandinterventiontowomenwhohavebeenorarevictimsofabuse. 5.11Traumatic/AcquiredBrainInjury This service provides psychological assessment and rehabilitation to those with traumatic brain injuries. 10

6.TEACHING,TRAININGANDRESEARCHACTIVITIES Staff within Psychological Services have close links with Glasgow, Edinburgh, Caledonian, and Stirling universities, and a servicewwide research group has been established to provide encouragement,consultationandsupportforpsychologystaffandtraineesplanningandconducting research.weareactivelyinvolvedindevelopingresearchproposalsfortrainees.wehaveamajor commitmenttoteachingonallthecoursesfromwhichwetaketrainees. Staffalsoprovideteachingandclinicaltrainingtoavarietyofotherprofessionalsandstaff,including nurses,healthvisitors,gps,traineepsychiatrists,juniormedicalstaff,careproviderstaff,adoptive andfosteringparents,socialworkers,midwiferystaffandalliedhealthprofessionals. 7.TRAVELANDSPECIALARRANGEMENTS InallNHSBoards,servicesareofferedacrossawidegeographicalareaandplacementswillrequire travel to various clinical bases and locations within the NHS Board region.
